Content ArticleAn open letter has been published in the Nursing Times from infection prevention and control experts, together with interested and concerned individuals and organisations, about the restrictions enforced in nursing, care and residential homes. Restrictions are being imposed in relation to COVID-19 across too many nursing, care and residential homes in the UK and beyond, in the name of infection prevention and control. A number of experts in this field, led by Jules Storr, independent global health consultant and former Infection Prevention Society (IPS) president, summarise in an open letter why infection prevention and control should be an enabler not a barrier to safe, compassionate human interaction in nursing, care and residential homes. By adding their voice their intention is to accelerate action to end this uncompassionate treatment of people in homes as well as for their families and other loved ones.

Content ArticleThe Queen’s Nursing Institute has published a resource for community nurses caring for people living with COVID-19. Commissioned by NHS England and NHS Improvement, ‘Living with COVID-19 (Long Covid) and Beyond’ provides information to support nurses working in community, care homes and primary care and also to the wider multi-disciplinary team including clinical knowledge, care responses and skills when caring for people during their recovery and rehabilitation. It is predicted that there will be a ‘new wave’ of physical, mental and emotional health challenges as individuals enter recovery from COVID-19 infection – and for some this is combined with issues resulting from the social and economic impact of lockdown, such as isolation and unemployment. The resource also aims to help nurses assist people, families, carers and employers to work towards managing post COVID-19 symptoms, regaining everyday life activities and returning to independent living wherever possible. It includes information on physical care, psychological and neuro-psychological care, social impact and features several case studies.

Content ArticleDrawing on a 2010 analysis of the reform and costs of adult social care commissioned by Downing Street and the UK Department of Health, this paper from Glasby et al., published in the Journal of Social Policy, sets out projected future costs under different reform scenarios, reviews what happened in practice from 2010-19, explores the impact of the growing gap between need and funding, and explores the relationship between future spending and economic growth. It identifies a ‘lost decade’ in which policy makers failed to act on the warnings which they received in 2010, draws attention to the disproportionate impact of cuts on older people (compared to services for people of working age) and calls for urgent action before the current system becomes unsustainable.

Content ArticleSuperabsorbent polymer gel granules are used to reduce spillage onto bedding and clothing when patients use urine bottles or vomit bowls, or when staff move fluid-filled containers (eg washbowls or bedpans). If the gel granules are put in the mouth, they expand on contact with saliva risking airway obstruction. This National Patient Safety Alert requires any organisation still using these products to protect patients by introducing strict restrictions on their use.

Delirium awareness and management of delirium superimposed dementia

Learning from frontline staff is crucial, now more than ever. Prior to a predicted second wave hitting us, the government and leaders must listen to what has gone well but, most importantly, not so well for both staff and patients. Martin is a passionate nurse working on a covid unit and wants to promote learning to ensure patient and staff safety. This initially started as a way of connecting and not feeling alone but what Martin has found is that there are many voices that need and want to be heard but just don’t know how to speak up and out. In all of the interviews the healthcare professionals wanted to remain anonymous which is indicative of their fear of reprisals from their organisation. In this first interview, Martin interviews a new student district nurse who has been working within the community in the South West. Their role involves supporting care homes with end of life care and assisting in keeping people with long term conditions at home.
