    This study in JAMA Network Open sought to determine whether limited English proficiency (LEP) is associated with not reporting a history of cardiovascular disease among patients with angina symptoms. The study found that patients reporting symptoms of angina were more likely to not report having cardiovascular disease if they had limited English proficiency. This discrepancy may be because of higher rates of undiagnosed cardiovascular disease or lower awareness of such diagnoses among individuals with LEP. The authors highlight the importance of finding effective communication strategies for people with LEP in order to make prevention and treatment for cardiovascular disease more effective.

    Surekha Shivalkar was a 78-year-old woman who was scheduled for elective total hip replacement revision surgery. Following surgery she suffered a cardiac arrest and subsequently died. The conclusion of the inquest was that died from multi-organ failure and complications arising during anaesthesia and hip revision surgery, which led to hypotension and hypoperfusion in a woman with ischaemic heart and chronic obstructive pulmonary disease. In his report, the Coroner raises concerns about the lack of a use of a formal risk assessment tool prior to her surgery, communication failures between the orthopaedic surgical team and the anaesthetist and the departure of the Senior Consultant surgeon prior to the surgeries conclusion.

According to Good Surgical Practice, all surgeons should regularly attend morbidity and mortality meetings as a key activity for reviewing the performance of the surgical team and ensuring quality.

    In October 2014, the Royal College of Surgeons in Edinburgh launched a UK-wide education campaign to get patients moving in the run-up to surgery. Addressing this costly and avoidable matter, the campaign asks patients to speak with their surgeon or GP to work out an exercise plan that suits their condition and the type of operation they will undergo.

    This study in the British Journal of General Practice aimed to examine the relationship between empathy and patient-reported satisfaction, consultation quality, and patients’ trust in their physicians. It also sought to determine whether this relationship is moderated by a physician’s gender. The authors found that doctors self-reported more gender differences in measures of empathy than were observed in external measures, which included a facial recognition test, observations and a Synchrony of Vocal Mean Fundamental Frequencies (SVMFF), which measures vocally coded emotional arousal. SVMFF significantly predicted all patient outcomes, and could be used as a cost-effective proxy for relational quality.

    A shared lingo can create cohesion in “the unit,” but James M Hodgetts and colleagues ask whether this is exclusionary and puts patient safety at risk. They collected the slang terms used by others and ourselves in the intensive care unit (ICU) over six months and collated these terms and expressions (those that can be committed to print) in a mini-dictionary of ICU slang, and some are used in UK medicine more broadly. They hope that the dictionary may be of use and possibly a source of amusement—and that it causes ICU staff and other healthcare workers to reflect on the suitability and inclusivity of the language they use with colleagues.

    The Royal College of Anaesthetists is launching a campaign to prevent future deaths from unrecognised oesophageal intubation following a recently received coroner’s report where an oesophageal intubation took place and was not recognised in time to save the life of the patient. The coroner’s report highlighted the critical importance of human factors in safe anaesthetic practice.     Technical developments tend to grab the headlines in health care. Predictive analytics, telemedicine, electronic health records — technology is rightly seen as a transformative force in health delivery. But it’s not the only one. At Rotterdam Eye Hospital, hospital administrators have found that through their ongoing design-thinking programme, lower-tech measures can also improve health care. Simple measures such as building a more intuitive website, replacing harsh fluorescent lighting and cold linoleum floors with softer lighting and wood parquet, and giving children and pediatric ophthalmologists matching T-shirts have reduced patient fears. Addressing patients’ fears is important because fear can make an eye operation difficult or even impossible. Moreover, less fear translates into greater patient satisfaction.     In this blog for National Voices, Sue Brown, CEO of the Arthritis and Musculoskeletal Alliance, argues that using the word 'elective' when referring to elective surgery is misleading, and downplays the seriousness of waiting for a long time for treatment or surgery. She looks at the impact of waiting too long for surgery such as joint replacement on the lives of patients. Intense, long term pain and loss of mobility can lead to deteriorating mental health, isolation from friends and family and job loss, among other things. Patients needs support while they wait for surgery, and Sue outlines what she believes is needed to support patients who have had community and secondary care delayed: Design support with those with lived experience – ask what is important to them. Use the things we know can help, like social prescribing and health coaching – individual or group personal support. Use the voluntary and community sector who have a wealth of experience in supporting long term condition management – people need to know they are not alone and get support from others in the same situation.

    On the 15 May 2020, John Skinner was admitted to Watford Hospltal suffering from a tonic clonlc seizure. He had a background of cannabis usage and a subdural empyema in 2020 that had left him with epilepsy. On arrival at hospital he again had another tonic clonlc seizure and focal seizures. The Junior doctor Instructed to administer the drug sought advice from a more senior doctor as to the dose to be administered. As a result of a failure In verbal communication between the doctors, aggravated as both were masked, a dose of 15 mg/kg was heard as 50 mg/kg and an overdose was administered. He was given 3600 mg of phenytoln. He arrested within 16 minutes and died and could not be revived.

    This article in The BMJ discusses the consequences for practising doctors of the 2015 Montgomery v Lanarkshire Case. The case was brought by Nadine Montgomery, a woman with diabetes and of small stature, after she delivered her son vaginally and experienced complications during the birth which resulted in her son having cerebal palsy. Her obstetrician had not disclosed the increased risk of this complication in vaginal delivery, despite Montgomery asking if the baby’s size was a potential problem. The Supreme Court ruling in her favour established that a patient should be told whatever they want to know, not what the doctor thinks they should be told.
