    New figures has revealed serious incidents have occurred in NHS trusts where the wrong bodies of patients were released to families or lost, damaged or kept the organs or bodies of babies without family permission. According to data from the Human Tissue Authority, these incidents have reduced over the years, however, the errors still affected more than 100 families in the latest year to March 2021. Incidents such as these are extremely distressing for the friends and family of the deceased patient, but also for the staff working in the mortuaries involved, who try their best to uphold the highest standards of patient care in often difficult circumstances. The fact that the mortuary profession is not a profession regulated by statute – as other professions such as laboratory staff and paramedics are – makes the maintenance of these quality standards more difficult than it needs to be."     Concerns have been raised amid allegations that hospital managers have told minority ethnic staff to adopt 'Western work names' as their original names were 'too difficult to pronounce'. In a letter from the Care Quality Commission, published in the trust's July board papers, workers at University Hospitals Bristol and Weston Foundation Trust told inspectors about their experiences during an unannounced visit in June. “This is not acceptable, individuals can only truly thrive in a work environment where they feel safe as themselves and belong rather than having to ‘fit in’.” Wrote the CQC’s head of hospital inspections Amanda Williams.     Union leaders have said cleaners, porters, catering assistants, security guards and other healthcare staff employed to work in NHS hospitals should also receive the same pay rise as NHS staff. Recently, most NHS staff were given a pay rise of 3%, but now Unison have said those working privately in the NHS may be at risk of not getting the same increase in pay. "NHS staff have the benefit of a national pay system, but those not directly employed are missing out, often because of complex contracting arrangements, penny-pinching practices and the hard-nosed pursuit of profit. Staff in the NHS work on one site as one team, from maintaining clean and safe wards to ensuring patients are fed and cared for.     In April of last year, many people in America came out and cheered for the healthcare workers fighting to save lives during the height of the coronavirus pandemic, but now, nurses across the US are holding strikes due to staff shortages and inadequate equipment amid the pandemic. “Most of us felt like we went from heroes to zeroes quickly,” says Dominique Muldoon, a nurse for more than 20 years at Saint Vincent’s hospital in Worcester, Massachusetts. Muldoon, co-chair of the local bargaining unit has also said nurses are going home crying in their cars, working through breaks and staying up late just to get the work done as demand for patient care has increased. “You’ll end up staying late or working through your break trying to fit the workload all in, but ultimately become so frustrated, because eventually you keep trying to overcompensate and cannot keep up with it."     Concerns over bullying and discrimination have been raised in a survey of hundreds of doctors at a major hospital trust, HSJ can reveal. University Hospitals of North Midlands Trust’s medical staff committee carried out a survey of its doctors earlier this year, after bullying concerns were raised by members of the British Association of Physicians of Indian Origin at the trust. A summary of the survey findings, published in a newsletter sent to all doctors at the trust last week and seen by HSJ, showed more than two-thirds of the 348 respondents claimed to have experienced bullying, harassment or victimisation at work. Nearly 80% said they had witnessed bullying or harassment, while 50 per cent of respondents said the bullying and harassment was due to race or ethnicity. Fifty-five per cent of those answering the survey also said they had not reported concerns as they had “no confidence in the investigative process within the organisation”.     More needs to be done to bring maternity units at a city's two main hospitals up to scratch, inspectors have said. In 2020 the Care Quality Commission (CQC) found serious concerns at Nottingham University Hospitals NHS Trust and labelled the units "inadequate". A new report concluded the trust still has "some areas to address". In October a coroner said the death of Wynter Andrews minutes after she was born was "a clear and obvious case of neglect". Nottinghamshire assistant coroner Laurinda Bower also revealed a 2018 whistle-blowing letter from midwives to trust bosses outlining concerns over staffing levels as "the cause of a potential disaster". In the same month "in response to concerns raised... and coronial inquests", the CQC carried out an unannounced inspection at the hospital and found some staff had not completed training and "did not always understand how to keep women and babies safe", and issued a warning notice over its concerns. Its latest report, based on an inspection in April, found improvements in the way women at risk of deterioration were identified and found documentation and monitoring had improved. However the CQC found a disconnect between online and paper record-keeping and said there were multiple systems in place that led to duplication and errors at times.     Despite under-reporting, health workers (HWs) accounted for 2-30% of the reported COVID-19 cases worldwide. In line with data from other countries, Jordan recorded multiple case surges among HWs. This study from Tarif et al. looked at infection prevention and control risk factors in HWs infected with Covid-19. Study findings confirmed the role of hand hygiene as one of the most cost-effective measures to combat the spreading of viral infections.

    This Healthcare Safety Investigation Branch (HSIB) investigation aims to help improve patient safety in relation to administering high-strength insulin from a pen device to patients with diabetes in a hospital setting. As its ‘reference case’, the investigation uses the experience of Kathleen, a 73 year old woman with type 2 diabetes who received two recognised overdoses of insulin while she was in hospital. On both occasions she became hypoglycaemic, received medical treatment, and recovered. Patient Safety Learning has published a blog reflecting on some of the key patient safety issues highlighted in this report.

    One of the reasons why patient safety may be put at risk during healthcare interventions is a lack of staff adherence to patient safety guidelines. There could be a relationship between staff’s adherence to patient safety guidelines and their perceived level of reward for their work and/or motivation. This study from Asmoro et al. examined the relationship between reward and adherence to patient safety guidelines, and between motivation and adherence to patient safety guidelines, among nurses working in emergency departments (EDs) in Indonesia. They found that ensuring ED nurses are motivated for their work by offering rewards – such as a decent salary, a supportive workplace environment and career progression opportunities – is important to enhance their adherence to patient safety guidelines.

    Pretty soon there won’t be a trust without an associate director or even board level director fully dedicated to all things equality, diversity and inclusion; relatively new senior roles that must have a purpose, job description and performance indicators. They will spend energy on yet more strategies, start from the top and hope something trickles down. Or they could start where the work is done, and build the tools to make equality, diversity and inclusion (EDI) everyone’s responsibility. Trusts are full of people passionate about EDI. So many roles, so many champions. They meet, share stories, and champion the importance of EDI. All this busyness typically outside a governed frame without the necessary reporting, investigating, actions, outcomes, learning, and measurable improvement. To normalise EDI and make it everyone’s responsibility will involve enabling reporting of EDI incidents, investigating it, taking action, and learning from it, writes Dr Nadeem Moghal in an article for HSJ.

    Bullying, discrimination and harassment between healthcare workers can have an impact on how well individuals do their job, and may therefore lead to an increase in medical errors, adverse events and medical complications. This systematic review in BMJ Quality & Safety aimed to summarise current evidence about the impact on clinical performance and patient outcomes of unacceptable behaviour between healthcare workers.

    This editorial in BMJ Quality & Safety looks at the risks to patient safety posed by negative interpersonal interactions between healthcare professionals. The authors review a recent study on the subject by Linda Guo et al that revealed how and when these negative behaviours from staff may have an impact on patient outcomes and clinical performance. They highlight the huge scale of the impact of unacceptable behaviours, arguing that it is even greater than evidenced in Guo et al's research. They also highlight that there are other, largely unexplored impacts on healthcare workers, patients and their families when they are exposed to unacceptable interactions.
