    This guide by the Patient Information Forum (PIF) provides practical support for translating health information. It offers tips on overcoming key challenges and links to useful resources. It is mainly focused on foreign language translation, but the principles can also be applied to British Sign Language and Braille. Research shows that in the UK, up to a million people cannot speak English well or at all, and these people have a lower proportion of good health than English speakers. Providing culturally appropriate, translated health information can help people manage their own health and take part in shared decision making.     This report by the Harmed Patients Alliance (HPA) explores the needs of injured patients and their loved ones for independent advocacy, advice and information when they have been involved in patient safety incidents that are believed to have led to harm. It examines the extent to which this is available or resourced, and aims to stimulate and inform a national discussion about this issue in England among key stakeholders. It looks at the historical context and the moral and economic arguments and implications of resourcing these kinds of services.

The goal was to look at ways specialists could support GPs so they could reduce the number of outpatient appointments patients have to attend, without compromising care.     Patient safety remains a global challenge for society today; in high income countries, it is estimated that one patient in ten is subject to adverse events while receiving hospital care. This article by Laís Junqueira, Quality, Patient Safety and Innovation Manager at Elsevier, in The Journal of mHealth looks at how enabling safer healthcare decision-making could reduce the burden of avoidable harm. Junqueira highlights the need to recognise that non-analytic and implicit decisions occur in healthcare systems, and that these have an impact on patient safety. He argues that as healthcare systems evolve, there must be an increased focus on the importance of an environment that fosters safe decision-making.

    In this study, Aniza Ismail and Norhani Mazrah Khalid assessed the baseline level and mean score of every domain of patient safety culture among healthcare professionals at a cluster hospital in Malaysia and identifed the determinants associated with patient safety culture. The study found that healthcare professionals at the cluster hospital showed unsatisfactory patient safety culture levels. Most of the respondents appreciated their jobs, despite experiencing dissatisfaction with their working conditions. The priority for changes should involve systematic interventions to focus on patient safety training, address the blame culture, improve communication, exchange information about errors and improve working conditions.

    Sodium valproate is a medication used to treat epilepsy, bipolar disorder and migraines, but it can cause birth defects, learning disabilities and developmental problems in babies if taken during pregmamcy. This video by Central and North West London NHS Foundation Trust discusses the various effects of using valproate, including the potential harmful effects the medication can have on unborn foetuses. It features a conversation between a pharmacist and patient discussing the need for a valproate pregnancy prevention programme if the patient is to be prescribed valproate.

    The NHS Patient Safety Conference, in partnership with Patient Safety Learning, is a long-standing virtual and in-person event series that has welcomed over 1500 NHS professionals through its doors. In February 2021, further updates and changes were made to the NHS Patient Safety Strategy. The most significant strategy update is the new commitment to address patient safety inequalities, with a new objective added to the safety system strand of the strategy. This event series provides a timely platform to discuss these changes. Key event topics are run across 3 key pillars: Insight Adopt and promote fundamental safety measurement principles and use culture metrics to better understand how safe care is. Use new digital technologies to support learning from what does and does not go well, by replacing the National Reporting and Learning System with a new safety learning system. Introduce the Patient Safety Incident Response Framework to improve the response to an investigation of incidents and implement a new medical examiner system to scrutinise deaths. Improve the response to new and emerging risks, supported by the new National Patient Safety Alerts Committee Share an insight from litigation to prevent harm. Involvement Establish principles and expectations for the involvement of patients, families, carers, and other lay people in providing safer care. Create the first system-wide and consistent patient safety syllabus, training, and education framework for the NHS. Establish patient safety specialists to lead safety improvement across the system. Ensure people are equipped to learn from what goes well as well as to respond appropriately to things going wrong. Ensure the whole healthcare system is involved in the safety agenda. Improvement Deliver the National Patient Safety Improvement Programme, building on the existing focus on preventing avoidable deterioration and adopting and spreading safety interventions. Deliver the Maternity and Neonatal Safety Improvement Programme to support a reduction in stillbirth, neonatal and maternal death, and neonatal asphyxia brain injury by 50% by 2025. Develop the Medicines Safety Improvement Programme to increase the safety of those areas of medication use currently considered the highest risk.     Three years since we launched the hub, our award-winning platform to share learning for patient safety, we have seen it grow in members, content and impact. To date, the hub has received over 565,000 visits and had over 1 million page views. It now has over 3,400 members from 80 countries working in over 1,000 different organisations, and offers 7,500 knowledge resources, viewed by people from 221 countries. We continue to highlight serious patient safety issues, celebrate patient safety achievements, provide ‘how to’ resources on good practice and offer a safe space for staff and patients to share their experiences and discuss challenges. In this blog, we would like to celebrate just some of the work we are especially proud of and highlight where we’ve been making the case for change and the many ways the hub is making an impact.
