Content ArticleThis survey, a collaboration between the International Society for Quality in Healthcare (ISQua) and the International Hospital Federation (IHF) was designed to frame the WHO Global Consultation on Patient Safety, which was held from 24-26 February 2020 to kick off the development of the Global Patient Safety Action Plan. Already then, the pandemic-to-be was affecting various regions, before striking health systems worldwide. The question of patient safety is a critical one in the discussion about COVID-19: hygiene and hospital-acquired infections, non-suitable hospital architecture, delayed surgeries and procedures, lack of personal protective equipment (PPE) and much more affected the safety of patients as well as of health workers, to whom the World Patient Safety Day 2020 is dedicated. In February 2020, the IHF disseminated a short survey on national safety plans to its Full Members, hospitals’ national/regional representatives. At the same time, ISQua disseminated their survey asking how well incident reporting is in place, and if the outcomes improve the 'no blame no shame' approach to their Individual and Institutional Members. The surveys were repeated in July 2020 to see if the onset of COVID-19 had made any positive or negative changes to the responses.

Content ArticleIn 2013, the Centers for Medicare and Medicaid Services announced that it would begin levying penalties against hospitals with the highest rates of hospital-acquired conditions through the Hospital-Acquired Condition Reduction Program. Whether the programme has been successful in improving patient safety has not been independently evaluated. Sheetz et al. used clinical registry data on rates of hospital-acquired conditions in 2010–18 from a large surgical collaborative in Michigan to estimate the impact of the policy. They concluded that the programme did not improve patient safety in Michigan beyond existing trends. These findings, published in Health Affairs, raise questions about whether the program will lead to improvements in patient safety as intended.

Content ArticleIn Northern Ireland (NI), leg ulcer clinical guidelines were developed by CREST (Guidelines for the Assessment and Management of Leg Ulceration) in 1998 and although never updated were superseded by NICE guidelines in 2006. Leg ulceration affects approximately 1% of the population of the UK, with a further 400,000 people experiencing recurrence. The aim of this audit was to assess the standard of care provided to patients with lower leg ulceration and to understand who provides care and where this care is provided.

In this guideline, written by the Association of Anaesthetists, recommendations include that there should be a named lead consultant in each department of anaesthesia who is responsible for liaising with their trust’s infection prevention and control team and occupational health department to ensure best antimicrobial practice is maintained in all areas of anaesthetic practice.
