    Public Policy Project's second phase of its integrated care policy programme will centre around the ICS Delivery Forum. The Delivery Forum will build on the foundations of the ICS Roadshow, continuing to connect key health and care stakeholders at a localised debate and networking. However, the Delivery Forum will focus on developing impact-driven, results-orientated insights. Working in close collaboration with ICSs and crucial providers, PPP is creating a programme that highlights exactly how ICSs are making place based, personalised care a reality and the impact this is having on individual citizens and communities. Rather than discuss issues such as health inequalities in broad framing and terminology, the Delivery Forum will ask local systems leaders and stakeholders to demonstrate exactly how integrated care systems can affect change in key health and care challenges. The Delivery Forum will also provide a helping hand to ICBs grappling with workforce challenges and service pressures by connecting system leaders with on the ground innovators and industry experts. Audiences will consist of local ICS leaders and representatives from the NHS, social care, primary care and local government, as well as community workers and social prescribing specialists. Further, the ICS Delivery Forum will engage with patients and end-users to gather first-hand experience of receiving care within a system in dire need of reform. This event is free for the Public Sector– including, NHS Organisations, Local Authorities, academics, researchers, non-for-profit and third sector providers and charitable sectors. Register

    The focus of GGI’s April health and care leaders webinar is systems partnerships and the Hewitt report, in conversation with Rt Hon Patricia Hewitt, a former Secretary of State for Health and now a Chair of an ICB. The Hewitt Report on systems partnerships and governance will really frame how boards will lead during this next crucial year for the NHS. With an election next year, all NHS board members will be navigating their way through financial and performance pressures of the likes never seen before. How the new ICBs behave will impact every single NHS board. In a ‘Chatham House Rule’ session this will be a valuable chance to understand the forces at play and what individual NHS directors need to understand in terms of context and direction.     This webinar shares the findings of a co-production project in Nottingham and Nottinghamshire Integrated Care Board (ICB) to remove barriers to shared decision making. The partners in the project were the ICB’s Personalised Care Team, the My Life Choices lived experience panel, the Patient Information Forum (PIF), and us, the Patients Association. The project was one we highlighted during Patient Partnership Week last year; you can learn more about it before attending this webinar by watching the recording of the Partnering with patients and communities - what's happening in ICSs session. Over the course of six co-production meetings, we developed simple resources to support patients and professionals to have better shared decision making conversations. This webinar shares the findings of the project. Speakers will discuss practical solutions to help patients and professionals get the most from limited appointment times which can be applied nationally. Register

    Communities are playing an increasingly important role in improving health and meeting the wellbeing needs of people locally, highlighted in part by their role in the response to the Covid-19 pandemic. Integrated care systems (ICSs) need to recognise the role communities can play in improving and sustaining good health, and as part of this they need to seek greater involvement with local voluntary, community and social enterprise (VCSE) groups at the place and neighbourhood level, where the link local communities is at its strongest. This conference will provide an opportunity to discuss the impact of community-led and person-centred approaches to improving health and wellbeing, and to explore what more can be done to build on community interventions, assets and solutions that developed as a response to the pandemic. It will also consider the challenges of demonstrating value and of working with communities to assess need and provide services. You will hear from community groups who have worked with others – including their ICS, local health system or local authority – to develop a collaborative approach to tackling health inequalities.

    The event will be hosted by Lord Bethell, Member of the House of Lords and the former Health Minister, who will provide the opening remarks. Health Tech Alliance Chair Dame Barbara Hakin will welcome attendees and chair the event. The event will consist of opening remarks, a keynote address, networking and drinks. This event series follows the successes of our previous Conferences, originating in January 2020 where the launch of the Artificial Intelligence in Health and Care Award was announced. Our previous keynote speakers include the then-Health Ministers Matt Hancock, Lord Bethell and Lord Kamall, Jeremy Hunt, and Dr Sam Roberts, Chief Executive of NICE. In 2023 we are adding this Networking Reception to address the need for dialogue between politicians, the health system, and industry in helping the NHS to fix much of its current challenges.     From July 2022, all NHS trusts providing acute and mental health services will need to join a provider collaborative, with these collaboratives forming a universal part of the provider landscape. Working within a challenging NHS environment – struggling with record high waiting lists and a limited workforce – provider collaboratives offer an opportunity to make efficiencies whilst improving service delivery. As providers move from a mindset of competition to one of collaboration, they must come together to deliver better services and improve care pathways. However, the purpose and form of these collaboratives can vary considerably across England and important decisions remain over the governance and accountability arrangements of these new collaboratives.     Local public health systems have recently been tested to an unprecedented degree. The pandemic response since March 2020 has necessitated the scaling up of diagnostic and testing capacity, enhanced collaborative working across the health system and a rapidly assembled nationwide vaccination programme. This free online event from the King's Fund will provide an opportunity to share experiences and lessons from the public health response to the pandemic, including how local systems have mobilised and collaborated, the barriers faced by those designing and delivering services and priorities for ensuring the NHS and wider public health system are equipped to tackle any future public health crisis. Recognising the link between inequalities and pandemic-related health outcomes, our expert panel will also explore ways to increase vaccine uptake in deprived communities and share data across local areas to inform the response. This discussion will consider the role of innovation and those involved at a place and system level in the response to emerging health security concerns and the challenges that remain for integration between health system partners.     Population health is an increasingly clear and important priority for the health and care system and is key in addressing health inequalities. This is clear in the NHS Long Term Plan, in the 2022 Health and Care Act, and in the current and likely future policy landscape. A population health-led approach to health and care aims to improve physical and mental health outcomes, promote wellbeing and reduce health inequalities across an entire population. Whether you’re working in an integrated care system (ICS), primary care network, a national body, community group, or you’re someone with lived experience it is likely that you have been thinking about your own role in population health and how best you can help to improve the health and care of the people in your local area.     Breaking down the barriers between organisations is key to the successful development of integrated care systems (ICSs), and the underpinning digital transformation that their introduction demands. Digital transformation can become the foundation of partnership working across health, social care, local government, and wider partners – including those in the voluntary, community and social enterprise, and private sectors – as place-based approaches to delivering care develop. This session from The King's Fund will explore what is being done to create collaborative digital strategies at ICS level that enable practical and flexible ways of working between partners. It will discuss how best to harness and use the data the system already holds, and how partnerships can move beyond barriers around data sharing, co-ordination and workforce capacity. Register

    Is it time to change the way England's healthcare system is funded? Is the English system in need of radical structural change at the top? I've been prompted to think about this by the article about the German public health system on the BBC website: https://www.bbc.co.uk/news/health-62986347.amp There are no quick fixes, however we all need to look at this closely. I believe that really 'modernising' / 'transforming' our health & #socialcare systems could 'save the #NHS'. Both for #patients through improved safety, efficiency & accountability, and by making the #NHS an attractive place to work again, providing the NHS Constitution for England is at the heart of changes and is kept up to date. In my experience, having worked in healthcare for the private sector and the NHS, and lived and worked in other countries, we need to open our eyes. At present it could be argued that we have the worst of both worlds in England. A partially privatised health system and a fully privatised social care system. All strung together by poor commissioning and artificial and toxic barriers, such as the need for continuing care assessments. In my view a change, for example to a German-style system, could improve patient safety through empowering the great managers and leaders we have in the NHS. These key people are held back by the current hierarchical crony-ridden system, and we are at risk of losing them. In England we have a system which all too often punishes those who speak out for patients and hides failings behind a web of denial, obfuscation and secrecy, and in doing this fails to learn. Vast swathes of unnecessary bureaucracy and duplication could be eliminated, gaps more easily identified, and greater focus given to deeply involving patients in the delivery of their own care. This is a contentious subject as people have such reverence for the NHS. I respect the values of the NHS and want to keep them; to do this effectively we need much more open discussion on how it is organised and funded. What are people's views?

    The idea of patient feedback as an essential tool for improving the safety of services is a familiar one. In recent years there has been a more fundamental shift towards recognising patients not just as commentators on the safety of the healthcare they experience, but as contributors to improving the safety of care. In this blog, Kate Eisenstein, Director of Strategy at the Parliamentary and Health Service Ombudsman (PHSO) looks at the ways in which patients and their families contribute to safe care. She also highlights the fact that in many cases, their voices are still being ignored, with catastrophic consequences for individual patients and the system as a whole.

    Patients need to be involved in all aspects of the design and delivery of healthcare and to make quality improvements that prevent harm. The Patient Safety Commissioner website shows examples of where working in partnership with patients and families, listening to patients’ voice and acting upon their concerns have made positive changes.

    The World Health Organization (WHO) launched the Global Knowledge Sharing Platform for Patient Safety (GKPS) at the World Patient Safety Day 2023 Global Conference on 13 September 2023. GKPS is an online and public platform to facilitate systematic collection and sharing of patient safety knowledge by stakeholders in different geographic regions, economics and cultural settings. It promotes the sharing of best practices related to the theme of each World Patient Safety Day for implementing and learning, as well as sharing of experience in enhancing patient safety.

    The focus of this year's World Patient Safety Day is engaging patients in “recognition of the crucial role patients, families and caregivers play in the safety of healthcare”. In this Comment in the Lancet, Jane O'Hara and Carolyn Canfield outline how supporting patients and families to be partners in care safety is both a logical and moral imperative. That is, we need to do it for safer care, but we also should do it because safer care relies on relationships, reciprocal trust, and collaboration.

    Patients and families are key partners in diagnosis, but there are few methods to routinely engage them in diagnostic safety. Policy mandating patient access to electronic health information presents new opportunities, and in this study, researchers tested a new online tool—OurDX—that was codesigned with patients and families. The study aimed to determine the types and frequencies of potential safety issues identified by patients with chronic health conditions and their families and whether their contributions were integrated into the visit note. The results showed that probable Diagnostic Safety Opportunities (DSOs) were identified by 7.5% of paediatric and adult patients with underlying health conditions or their families. Among patients reporting diagnostic concerns, 63% were verified as probable DSOs. The most common types of DSOs were patients or families not feeling heard, problems or delays with tests or referrals and problems or delays with explanation or next steps. In chart review, most clinician notes included all or some patient/family priorities and patient-reported histories. The researchers concluded that OurDX can help engage patients and families living with chronic health conditions in diagnosis. Participating patients and families identified DSOs and most of their OurDX contributions were included in the visit note.

    How we treat each other at work has an enormous impact on how teams perform—with potentially fatal consequences if you work in healthcare. Chris Turner, consultant in emergency medicine and founder of Civility Saves Lives, reveals the shocking impact of rudeness in the workplace. He highlights the importance of understanding the complex realities of practice and communication between healthcare professionals in different team environments, if we are to learn from patient safety incidents.

    There is widespread variation in the instance and quality of meaningful patient involvement across the 42 Integrated Care Systems (ICSs) of NHS England. This is seen throughout the structures, policies and processes of the ICSs, from the omission of patient representatives on decision-making bodies—such as Integrated Care Boards (ICBs)—to the neglect of clear consultation when decisions are made concerning a patient’s care. This report present the results of research and analysis conducted by the Medical Technology Group (MTG). It shows that where a patient lives is the biggest determinant to whether they are involved in their care meaningfully, or at all. It makes recommendations for the Government, NHS England and ICS's on the approach that should be taken to ensure meaningful patient engagement.

    The aim of this study in the journal Pediatrics was to explore the impact of rudeness on the performance of medical teams. Twenty-four NICU teams participated in a training simulation involving a preterm infant whose condition acutely deteriorated due to necrotizing enterocolitis. Participants were informed that a foreign expert on team reflexivity in medicine would observe them. Teams were randomly assigned to either exposure to rudeness (in which the expert’s comments included mildly rude statements completely unrelated to the teams’ performance) or control (neutral comments). The videotaped simulation sessions were evaluated by three independent judges (blinded to team exposure) who used structured questionnaires to assess team performance, information-sharing and help-seeking. The authors concluded that rudeness had adverse consequences on the diagnostic and procedural performance of NICU team members. Information-sharing mediated the adverse effect of rudeness on diagnostic performance, and help-seeking mediated the effect of rudeness on procedural performance.

    The State of Integrated Care Systems 2022/23 examines the progress that local systems have made, and opportunities for further development. The NHS Confederation’s ICS network collected the views of 47 integrated care board chairs and chief executives, and integrated care partnership chairs, in the spring, as well as holding roundtables. The results found they are generally positive about relationships – with 88% agreeing that “partners within my ICS are working collaboratively”. However, when asked about whether a range of organisations in their area “have the requisite level of resourcing and maturity to deliver the ambitions outlined in your integrated care strategy”, there were signs of concerns about primary care networks, provider collaboratives and place-based partnerships, all of which saw less than 50% of leaders agree.
