Along with organisations everywhere, we at Patient Safety Learning have had to adapt how we work due to the COVID-19 pandemic. Though our working environments and areas of focus have changed, our goals as a charity have not. We continue to be an independent voice, committed to working in partnership to improve patient safety.

    The Medication Errors Group was established in 2017 under the leadership of Dr Brian Edwards and Professor Ian Wong.. Ensuring the safe and effective use of healthcare products is a key objective for those who work in pharmacovigilance, as well as all other stakeholders, including patients and caregivers. One key element of that is working to reduce and mitigate medication errors and other irrational use of drugs. The mission of the Medication Errors Group is To provide an opportunity for International Society of Pharmacovigilance (ISoP) members, and like-minded collaborators, to network globally in a professional and neutral environment, to share evidence and solutions that can ideally eliminate and/or at least reduce the number and severity of medication errors. This will enable continuous improvement in the use of medicines whilst supporting the interests of patients and their families, and healthcare professionals.

    The involvement of patients in decisions about their care is essential to help effectively manage long-term conditions and help achieve the best possible patient outcome. ‘Shared decision making’ ensures that individuals are supported, by their healthcare partners, to make decisions about their care or treatment, that are right for them. Shared decision making is included in the NHS Constitution, and AbbVie, a research-driven biopharmaceutical company. hopes to help continue raising the profile of patient involvement. AbbVie wants to see shared decision making be widely adopted across NHS services. To support this ambition, AbbVie held the Shared Decision Making Showcase in Parliament on the 10 March 2020. A year on from the publication of the NHS Long Term Plan, and Universalised Personalised Care Plan – which established ambitious targets to put shared decision making at the heart of patient care – the showcase provided a platform for patient groups, NHS Trusts, and healthcare providers to share their innovative work. The projects showcased ranged from MS, arthritis, Lymphoma, Autism, Hepatitis C, kidney dialysis and more—all focused on empowering patients to take an active part in decisions about their treatment and care. AbbVie has supported a number of organisations and projects to develop and enable shared decision making including two of the showcased initiatives: a survey of treatment decision experience amongst psoriasis patients and the work of the Patient Information Forum to develop and implement the Perfect Patient Information Journey.

    The Institute for Healthcare Improvement (IHI)-convened National Steering Committee for Patient Safety (NSC) has released a National Action Plan intended to provide US health systems with renewed momentum and clearer direction for eliminating preventable medical harm. Safer Together: A National Action Plan to Advance Patient Safety draws from evidence-based practices, widely known and effective interventions, exemplar case examples and newer innovations. The plan is the work of 27 influential federal agencies, safety organisations and experts, and patient and family advocates. The plan provides clear direction that health care leaders, delivery organisations, and associations can use to make significant advances toward safer care and reduced harm across the continuum of care.

    The shared commitment and responsibility uniting everyone within and outside of healthcare during the COVID-19 has been unparalleled. Prior to the pandemic, this type of shared commitment has been discouragingly lacking for other major healthcare concerns such as patient safety. Reasons for this include organisational leaders who are incentivised to focus on activities essential for reimbursement and quality measurement rather than those involving the promotion of safety culture and implementation of systems-based approaches to improve safety, compounded by lack of clear ownership and accountability to solve long-standing safety challenges. The COVID-19 pandemic is leading to several ongoing impacts on the healthcare delivery system, many of which have patient safety implications. We are witnessing negative effects from delays in care from patients not seeking (or unable to seek) healthcare, patients with complex chronic conditions not having ongoing ambulatory care and new types of diagnostic errors. However, we are also witnessing some early short-term positive effects in selected safety areas where the COVID-19 pandemic has provided a new glimmer of hope.     The Prioritisation Matrix is a structured visual tool to help you decide which improvement ideas to test first and how to focus your activity and energy. It works best in a collaborative environment and can help to build buy-in and communicate why you have chosen to test certain ideas before others. They come in many different forms, but the simplest and easiest to use is the 2 x 2 matrix. The axis of the matrix are usually labelled to enable you categorize the priority of each change idea. Usually the horizontal axis is labelled with a concept such as “effort” or “willingness to adopt”. The vertical axis is usually “impact” or “value”.

    Asking a simple yet powerful question: ‘what matters to you?’ can give us, as healthcare professionals, vital insights into the lives of our patients, not always captured in routine assessments.     The Inaugural Australian Patients for Patient Safety Workshop, held over 3 days in Perth from July 7 ‐9, 2009, brought together a group of 40 health consumers, many of whom had experienced medical error or health system failure, health providers and health policy makers from around Australia.   Participants were selected for their efforts as change agents who have worked proactively to improve the safety of health care in Australia and their desire to further improve safety in health care, in partnership. Participants came together to build trust, functional working relationships grounded in mutual respect and appreciation of what each brought to the field of patient safety and to form strategies and action plans for improving patient safety in Australia. The core of those strategies and action plans is the Perth Declaration for Patient Safety.

    Staff at C.S. Mott Children’s Hospital and Von Voigtlander Women’s Hospital, Michigan are adopting a new approach to safety. By picking up near misses, close calls, deviation off protocol and investigating each one via a daily huddle, they are able to enable change system wide.

    Earlier this year, the World Health Organization declared 17 September the first World Patient Day and presented it as an opportunity to speak up for patient safety. A week or so beforehand, health leaders from across the world had met in Salzburg, Austria, at the request of Salzburg Global Seminar and the Institute for Healthcare Improvement (IHI) to explore ways of improving the measurement of patient safety. The Lucian Leape Institute, an initiative of the IHI, led the convening and content curation. Participants of Moving measurement into action: designing global principles for measuring patient safety agreed that there is no single measure that allows all stakeholders in all settings to assess the past, current, and future safety of their system. Participants agreed a system of measures must be carefully designed to assess the safety of patients throughout their health journey. The conversations in Salzburg have helped establish eight global principles for the measurement of patient safety. They feature in this new document, Salzburg Statement on Moving Measurement into Action: Global Principles for Measuring Patient Safety.

    As COVID-19 spread throughout the world, clinicians and researchers rapidly published guidance and data and shared their experiences in the hope of understanding the virus better. Their shared purpose was to keep more patients safe from becoming acutely unwell or dying. While the initial focus was on treating the hospitalised, one Trust was also thinking ahead to the challenging recovery many would face.
