    Lawmakers say minorities may disproportionately suffer from long-term symptoms of coronavirus infection. A pair of Democratic House members asked the Centers for Disease Control and Prevention (CDC) in a letter Tuesday to release data on the number of Americans who suffer lingering symptoms of coronavirus infection, including breakdowns along race, gender and age. The National Institutes of Health and the CDC have launched detailed studies of Long Covid, but those examinations are expected to take years. In the meantime, policymakers lack good information about how many people in the United States and worldwide suffer from long-term, debilitating effects of the disease. “People suffering from Long Covid have been ignored and overlooked for far too long. Collecting and publishing robust, disaggregated demographic data will help us better understand this illness and ensure that we are targeting lifesaving resources to those who need them most,” said Rep. Ayanna Pressley, who co-signed the letter Tuesday to the CDC.“We’re calling on the CDC to publicly report this data because that which gets measured gets done — and we can’t have an equitable recovery from this pandemic without it.” Read full story Source: The Washington Post, 25 January 2022

    This report presents maternal mortality rates in the USA for 2020 based on data from the National Vital Statistics System. Maternal mortality rates, which are the number of maternal deaths per 100,000 live births, are shown in this report by age group and race and Hispanic origin. In 2020, 861 women were identified as having died of maternal causes in the United States, compared with 754 in 2019. The maternal mortality rate for 2020 was 23.8 deaths per 100,000 live births compared with a rate of 20.1 in 2019. In 2020, the maternal mortality rate for non-Hispanic Black women was 55.3 deaths per 100,000 live births, 2.9 times the rate for non-Hispanic White women (19.1). Rates for non-Hispanic Black women were significantly higher than rates for non-Hispanic White and Hispanic women. The increases from 2019 to 2020 for non-Hispanic Black and Hispanic women were significant. The observed increase from 2019 to 2020 for non-Hispanic White women was not significant.

    The Healthy Data e-consultation is a joint initiative run by the Belgian project 'Towards the development of a national health data platform' (AHEAD) and the European initiative 'Towards a European Health Data Space' (TEHDAS). Its aims are: to listen to citizens and patients’ views on health data secondary use and sharing, and on the role that they would like to play in the management and use of their health data. to increase citizen awareness, engagement and empowerment on the topic, so that everyone can develop informed opinions and take an active role in the use of their health data.  Anyone can sign up and share their views on the following questions: What should your health data be used for? Under which conditions should your health data be used? How would you like to be informed and involved in the reuse of your health data? What other ideas do you have on health data reuse?

    The number of 12-hour waits in accident and emergency departments rose by 27% in one month to reach record levels in January amid warnings overcrowding is harming an increasing number of patients. Official monthly performance data prompted the Royal College of Emergency Medicine’s president to warn that the problems facing emergency departments were getting “worse and worse”, while pointing out the real number of 12-hour A&E breaches is likely higher than official data records. The figures also revealed the waiting list had hit a new high of 6.1 million, while the number of two-year breaches also rose a record level. Trusts recorded 16,558 patients last month waited 12 hours or more in an emergency department from decision to admit to being discharged or admitted. This was up from 12,986 in December. RCEM president Katherine Henderson warned on Twitter: “This is [decision to admit] plus 12 – a concept which must be retired as a performance metric. We should have 0-12 hour data. You cannot fix a problem if you [are] unwilling to face up to what it actually is. We estimate reality is 20 x more.     People with dementia, particularly care home residents early in the pandemic, were “disproportionately” vulnerable to fatal Covid infections, according to a new report. The analysis, commissioned by the Scottish Government, also found that excess deaths involving dementia during 2020 were “almost wholly” attributable to Covid. Of the 2,154 deaths where both dementia and Covid were listed on the death certificate, 95 per cent had Covid as the main underlying cause. This contradicts previous suggestions that a rise in dementia deaths early in the pandemic may have been linked indirectly to the virus as a result of “lockdown distress” or an increased use of potentially harmful sedation in elderly people confused by restrictions. The report also found that 73% (1,577) of those who died with both Covid and dementia mentioned on their death certificates had passed away in care homes. Henry Simmons, chief executive of Alzheimer’s Scotland, said their loved ones had been “torn apart by grief and loss” and that the report “raises many more questions as to why so many people with dementia living in care homes quickly became victims of Covid-19”.     The Atlases of Variation help to identify unwarranted variation and assess the value that healthcare provides to both populations and individuals. This is produced in collaboration with PHE, NHS England and RightCare and many other organisations. Products include Compendium atlases and themed atlases for areas such as Diagnostic Services and Liver Disease. A defining aspect of the atlases is that each of the indicator’s maps, column chart and box-and-whisker plot is accompanied by text which provides: the context for the indicator, a description of the variation and trend data, options for action and a list of evidence-based resources to support action. Interactive Atlases services can be accessed via the NHS England website.

    A delayed discharge is when a patient no longer requires hospital care but isn’t able to leave. Earlier this winter, hospital trusts in England were asked to work with local partners to cut delayed discharges by half by the end of January.     Non-communicable diseases (NCDs) including cardiovascular disease, cancer, chronic respiratory disease and diabetes, are leading causes of morbidity, disability and mortality in the WHO European Region, causing nearly 90% of all deaths and 67% of premature deaths. The World Health Organization (WHO) Regional Office for Europe has released the WHO Europe NCD Dashboard, which provides graphical data on NCDs in the 53 Member States of the Region and makes comparisons between them. The dashboard enables analysis of a country’s health situation and its progress towards meeting NCD health targets. It includes standardised data from national and international registries and surveys collected by countries, WHO and other international organisations

    Improving patient safety culture (PSC) is a significant priority for OECD countries as they work to improve healthcare quality and safety—a goal that has increased in importance as countries have faced new safety concerns connected to the COVID-19 pandemic. Findings from this OECD benchmarking work in PSC show that there is significant room for improvement.

    Patient safety, staff moral and system performance are at the heart of healthcare delivery. Investigation of adverse outcomes is one strategy that enables organisations to learn and improve. Healthcare is now understood as a complex, possibly the most complex, socio-technological system. Despite this the use of a 20th century linear investigation model is still recommended for the investigation of adverse outcomes. In this review, Isherwood and Waterson use data gathered from the investigation of a real life healthcare near incident and apply three different methodologies to the analysis of this data. They compare both the methodologies themselves and the outputs generated. This illustrates how different methodologies generate different system level recommendations. The authors conclude that system based models generate the strongest barriers to improve future performance. Healthcare providers and their regulatory bodies need to embrace system based methodologies if they are to effectively learn from, and reduce future, adverse outcomes.

    A major IT incident at an acute trust is disrupting treatment for eye patients after a significant data loss, it has emerged. Sandwell and West Birmingham Hospitals Trust chief executive Richard Beeken revealed to his trust’s board that a data loss incident in December had “impacted on staff and patient care” after disrupting 20 systems across the organisation. Recovery of the full data set for patients receiving treatment at the Birmingham and Midland Eye Centre is still under way, and some have had operations postponed. Despite the incident, ophthalmologists are continuing to see the majority of patients, Mr Beeken said, telling HSJ: “[Numbers affected] are being kept to a minimum through the extraordinary efforts of the clinical team who are putting in extra hours to reassess each patient’s needs.” Scanning continues in the majority of cases and the trust is pressing on with recovery work for all historic images and patient contact details, though leaders believe the chances of 100% data recovery are “still slim”.     Mapping variation is an invaluable tool for understanding how our healthcare system is providing care. Maps of variation in care, derived from information routinely gathered by the health system, show how healthcare use differs across the country and raise important questions about why this variation might be occurring. The aim is to prompt further investigation into whether the observed variation reflects differences in people’s healthcare needs, in the informed choices they make about their treatment options, or in other factors. Each Atlas includes data, maps, graphs, clinical commentaries and recommendations for each chapter. These can be viewed or downloaded from links on the Atlas website.

    Reporting to the National Reporting and Learning System (NRLS) is largely voluntary, to encourage openness and continual increases in reporting to facilitate learning from error. Increases in the number of incidents reported reflects an improved reporting culture and should not be interpreted as a decrease in the safety of the NHS. Equally, a decrease cannot be interpreted as an increase in the safety of the NHS. This report covers the early stages of the COVID-19 pandemic in England, from April 2020 through to the end of March 2021, when cases had declined rapidly. The number of incidents reported from April 2020 to March 2021 was 2,109,057, and represent a small decrease of 6.1% compared to April 2019 to March 2020 (2,246,622).

    This International Patient Summary roadmap (G7-IPS) supports the G7 commitment to deliver on the rights of patients to have access to their health information, and through using open and interoperable standards it enables this information to be used at the point of treatment or care. The roadmap outlines the component parts required for implementation and the standards which will be used to ensure alignment and interoperability across the G7 community. Although developed by the G7 countries, other countries, should they wish to, will be able to adopt the same principles and use the open and interoperable resources.

AHRQ’s digital healthcare research (DHR) programme funds research to create actionable findings on what and how digital healthcare works best for these critical stakeholders in healthcare. Now more than ever, the DHR programme is focused on supporting crucial research that identifies how the various components of the ever evolving digital healthcare ecosystem can best come together to positively influence healthcare delivery and create value for its key stakeholders: patients, clinicians, and health systems. This ecosystem includes clinical, contextual, and patient-generated health data as well as the tools used to manage and apply these data, such as advanced analytics and data visualisations. The application of these data can result in new knowledge, which can take the form of computable clinical guidelines and decision support. The DHR program continues to fund research on how these ecosystem elements and the actors who create and use them can best support the quality and safety of healthcare.

    'Kicking the Hornet’s Nest' is a documentary that looks at power morcellation, a popular gynaecologic procedure used to perform hysterectomies. The documentary demonstrates how the practice has been inadvertently spreading cancer in patients for decades. It includes first-person testimonies and archival footage and follows two married, Harvard-affiliated whistle-blowers who have been personally impacted by the procedure, as they campaign to expose the controversial practice and prevent future needless deaths.

    Digital technology is transforming every industry including healthcare. Digital and data have been used to redesign services, raising citizen expectations about self-service, personalisation, and convenience, and increasing workforce productivity. The pandemic has accelerated the shift to online and changed patient expectations and clinical willingness to adopt new ways of working. In addition, it facilitated new collaborations both in the centre of the NHS and wider local health and care systems. Together, these changes have enabled previously unimaginable progress in digitally enabled care pathways. The goal of this review is to build on this progress and ensure the national NHS (defined as NHS England and NHS Improvement (NHSEI), NHSX (X) and NHS Digital (D)) can lead the transformation of the wider healthcare system, supporting integrated care systems (ICSs) to deliver better citizen health.
