    In this podcast from General Broadcast, Clinical Lead Daimon Wheddon and the Norfolk and Norwich University Hospital's Dr Caroline Barry, sit down to talk about ReSPECT. The ReSPECT process provides a written record of patients' treatment preference and focuses on treatments to be considered as well as those that are not wanted or would not work. People with a complete ReSPECT form do not require a separate DNACPR form although it is important to be aware that a patient can have a ReSPECT form and still be in favour of cardiopulmonary resuscitation. ReSPECT aims to inform decision making for ambulance clinicians when the patient may lose capacity to make decisions for themselves.

    Having read an article about a critically ill intensive care patient terrified when they couldn’t understand what the healthcare team were saying through their personal protective equipment (PPE), Rachael Grimaldi, an NHS anaesthetist, was inspired to create a simple communication tool: CARDMEDICTM.   Update from Rachael: Since writing this blog, we have been accepted onto The Hill Accelerator Program, run by Oxford University, Oxford University Hospitals NHS Foundation Trust and Oxford Brookes University. We have also been selected to be a part of the Healthcare UK Digital Health Offer for Export for 2020. Healthcare UK is a joint initiative of The Department for International Trade (DIT), The Department of Health and Social Care (DHSC) and NHS England, which promotes the UK healthcare sector to overseas markets. We now have over 30,000 users in over 100 countries, alongside 11,500 app downloads.  Version 2 of the app is out now, with a 'free notes' section so the patient and healthcare staff can communicate directly. Text can either be dictated or typed into the notes box, the language auto-detected and then translated into one of 10 languages available and read aloud to the patient if necessary. We are working on enhancing accessibility through the addition of sign language videos and images / illustrations / makaton signs.  An independent academic evaluation by University of Brighton using simulated patients demonstrated 25% increased confidence in understanding a healthcare worker in PPE with CARDMEDIC than without. Overall, confidence improved by 28% to 95%. Results likely significantly higher for those unwell/communication needs.

It is based on over 120 interviews with former Intensive Care patients and family members at different stages of recovery. This website is aimed at: patients who have been on intensive care relatives and families who have been affected by their loved one being in intensive care.

    This resource has been developed by a group of multi-disciplinary health professionals at Lancashire Teaching Hospitals. Not every patient will experience all of these things, but they are more likely to if they have been in intensive care for more than a few days. Most of this guide is written for patients but there is a section specifically for relatives and visitors. By reading the guide, relatives will learn what a patient's recovery may involve and it will give them the answers to some of the questions they may have.

    CARDMEDIC was inspired by a news article on a patient surviving COVID-19 after an admission to a UK Intensive Care Unit (ICU), where he described feeling terrified of not being able to understand what his healthcare providers were saying, due to the limitations of communicating through Personal Protective Equipment (face masks, visors, hoods etc). CARDMEDIC is a collection of communication flashcards designed to break through the Personal Protective Equipment (PPE) barrier, improving transfer of vital information from frontline healthcare professionals to unwell and critically ill patients. They are also used as an upskilling training tool, for example by healthcare professionals and volunteers working outside their usual realms of practice. They are simple and succinct, using basic language to share information and describe the plan of action. Set out in alphabetical order, it should be easy to find what you’re looking for.   You may wish to use the “HELLO MY NAME IS…” card at the start of every patient interaction to introduce yourself and set the scene for using the flashcards. The flashcards can be used in electronic format on either the patient’s or hospital’s phone / tablet / smart device, or printed, laminated, annotated and re-used – write on, wipe off.  CARDMEDIC are continually expanding their database, so please get in touch with suggestions or comments, as well as ideas for further flashcards.

    This report published by Carers UK looks at carers’ recent experiences of hospital discharge under the discharge to assess model. It reveals the devastating toll on carers where it is clear that they have been left with unacceptable levels of caring responsibilities which are unsafe in some situations. This has placed intolerable stress upon carers and has had negative outcomes for people needing care and support. A very clear thread from carers’ experiences shows that carers have not been involved, consulted or given the right information in order to care safely and well. If carers are considered to be partners in care, then, like health and care professionals, they need access to relevant information to help them support a person needing care safely.

    An NHS-Led Provider Collaborative is a group of providers of specialised mental health, learning disability and autism services who have agreed to work together to improve the care pathway for their local population. They will do this by taking responsibility for the budget and pathway for their given population. The Collaborative will be led by an NHS Provider who remains accountable to NHS England and NHS Improvement for the commissioning of high-quality, specialised services. These Collaboratives aim to ensure that people with specialist mental health, learning disability and autism needs experience high quality, specialist care, as close to home as appropriately possible. They seek to enable specialist care to be provided in the community to prevent people being in hospital if they don’t need to be, and to enable people to leave hospital when they are ready. This webpage explains the role of NHS-Led Provider Collaboratives and includes case studies that demonstrate how they are helping to transform specialised mental health services.

    This report published by the National Confidential Enquiry into Patient Outcome and Death (NCEPOD) looked to identify and explore remediable factors in the clinical and organisation of the physical healthcare provided to adult patients admitted to a mental health inpatient setting.  The report suggests that a physical healthcare plan should be developed when patients are admitted to a mental health inpatient setting. Other key messages aimed at improving care include calls to: formalise clinical networks/pathways between mental health and physical health care; involve patients and their carers in their physical health care, and use admission as an opportunity to assess and involve patients in their general health, and include mental health and physical health conditions on electronic patient records.

    In this blog for the Citizens Commission on Human Rights, Jane London shares her account of how electroconvulsive therapy (ECT) has affected her life since early adulthood, resulting in severe memory loss and heart problems. Jane shares how her physical medical problems including appendicitis and a severe heart attack were put down to depression, and how she was nearly forced to have ECT against her will in England in 1966. When her abusive marriage ended in 1968 and Jane returned to Australia her mother insisted she have ECT, despite her depression being temporary and related to her marriage ending. After 14 treatments, Jane left the treatment facility and received talking therapy to help her recover. Jane talks about the dramatic adverse effect ECT has had on the rest of her adult life.

Don't Turn Away tells the story of "an increasingly brutal turning away from the most abused and damaged people who struggle to survive within our complacent society."
