    Few empirical studies have directly examined the relationship between staff experiences of providing healthcare and patient experience. Present concerns over the care of older people in UK acute hospitals – and the reported attitudes of staff in such settings – highlight an important area of study. Maben et al. examine the links between staff experience of work and patient experience of care in a ‘Medicine for Older People’ (MfOP) service in England.

    In his newsletter today (The Top 10 Dangers of Digital Health), the medical futurist, Bertalan Meskó, raises some very topical questions about the dangers of digital health. As a huge advocate of the benefits of digital health, I am aware of most of these but tend to downplay the negative aspects as I generally believe that in this domain the good outweighs the bad. However, as I was reading his article, I realised that it was written very much from the perspective of a clinician and, to some extent, a healthcare organisation too. The patient perspective was included but not from a patient safety angle. Many of the issues that he raises do have significant patient safety issues associated with them which I’d like to share in this blog.

    In this study, Mansab et al. examine the COVID-19 community triage pathways used by Singapore, Japan, USA and UK, specifically comparing the safety and efficacy of national online ‘symptom checkers’ used within the triage pathway.Their results suggest that whilst ‘symptom checkers’ may be of use to the healthcare COVID-19 response, there is the potential for such patient-led assessment tools to worsen outcomes by delaying appropriate clinical assessment. The key features of the well-performing symptom checkers are discussed in the paper.

    NHS Solent share their policy on healthcare workers screening and immunisation. The primary purpose of this policy is to reduce the risk of transmission of infection (as far as reasonably practical) from an infected healthcare worker-to-patient. The main known risks of infection through bloodborne virus in the clinical setting are from hepatitis B, hepatitis C and HIV. This measure is not intended to prevent those healthcare workers from working in the NHS but rather to restrict them from working in clinical areas where their infection may pose a risk to patients in their care and by early diagnosis; allows them to manage their own health.

    Healthcare organisations strive to improve patient care experiences. One way is to use one-on-one provider counselling (shadow coaching) to identify and target modifiable provider behaviours. Quigley et al. examined whether shadow coaching improves patient experience across 44 primary care practices in a large urban US health centre. They found that shadow coaching improved providers' overall performance and communication immediately after being coached. However, these gains disappeared after 2.5 years. Regularly planned shadow coaching "booster" sessions might maintain or even increase the improvement gained in patient experience scores, but research examining additional coaching and optimal implementation is needed.
