Content ArticleA number of associations and societies across the UK have come together to provide guidance on the safe switching of warfarin to direct oral anticoagulants (DOACs). This needs to be undertaken in a phased manner over the 12 week cycle of INR monitoring to protect the supply chain for ALL patients. As highlighted in the guidelines, many patients are not suitable for a switch from warfarin to DOACs and further guidance will be available shortly to support services providing care for these patients.

It is the result of a two-year effort to develop an evidence-based collection of interventions and case studies exploring how primary care organisations and practitioners engage patients and families in improvement work and in their personal safe care. The resource includes a user's guide and is accompanied by a deep environmental scan that informed the development of the work.

To understand how and when the NHS can reach net zero we established an NHS Net Zero Expert Panel, reviewed nearly 600 pieces of evidence submitted to us and conducted extensive analysis and modelling. The targets set are as ambitious as possible, while remaining realistic; and are supported by immediate action and a commitment to continuous monitoring, evaluation and innovation. The aim is to be the world’s first net zero national health service.

Content ArticleThis progress report outlines the contribution of Academic Health Science Networks (AHSNs) during the first year of the NHS Patient Safety Strategy. It looks at the impact it has had in improving safety in hospitals, maternity and neonatal units, care homes and the community. It also describes how AHSNs and the Patient Safety Collaboratives they host have responded to the COVID-19 pandemic, supporting programmes on tracheostomy safety and the use of pulse oximeters to safely monitor patients at home. Examples featured in the report include tools to spot and manage patients at risk of serious illness in hospitals and care homes, discharge safety bundles, and award-winning projects to support premature babies and their families.
