PRSB: 'About Me' standards
Patient Safety Learning posted an article in Patient-centred care
The About Me standard helps people share information about what is most important to them with health and care professionals so that staff can provide better, more person-centred care whenever and wherever it is needed. About Me information may include things like how best to communicate with the person, put them at ease during treatment, their spiritual or religious beliefs, or what arrangements to make for family or pets if they are hospitalised. The Professional Record Standards Body (PRSB) has published a standard outlining how About Me information should be documented and shared in health and care records. #CareAboutMe aims to raise widespread awareness of the About Me standard and the improvements it can make to the quality of care administered in health and care, as well as the positive impact it can have on people’s quality of life and health. PRSB wants to help every person share information about ‘what matters to me’ by using PRSB’s About Me standard. The goal is to help professionals provide better care and for people to experience lasting benefits to their health and wellbeing.e hospitalised.- Posted

Content ArticleHandover in healthcare settings can be a time when the risk of error and harm is increased. This blog summarises the results of global survey that asked the opinions of healthcare workers on the safety of handover. It highlights ten key points raised by the results: Handover causes frequent errors and patient safety incidents Handover errors can cause serious harm to patients Most people think they are better than average at handover The longer you’ve been around, the scarier handover appears Different types of handovers have a similar safety profile The safety of handover is a problem all over the world Most practitioners use manual or informal systems to support handover EPR systems are not up to the job of supporting handover Staff need more training, and we need more time Healthcare leaders want better electronic systems The results of the survey have been published in Preprints.

Content ArticleBefore a patient undergoes a medical procedure such as surgery, they need to give their consent, having been informed about the details of the procedure and associated risks and benefits. However, in spite of the development of clear guidance on informed consent, many patients still feel nervous or unsure about asking questions and raising concerns during the consent process. Beginning with a personal account that highlights the importance of a thorough and meaningful consent process, this article in The BMJ by Tessa Richards, Associate Editor, examines the issues faced by both doctors and patients in achieving truly personalised informed consent.

Content ArticleIn this interview for Pharmacy Update Online, Patient Safety Learning's Chief Executive Helen Hughes talks about how the hub was established to provide free, easily-accessible information about patient safety for everyone. "By everyone we mean literally everyone–the hub was designed by and for clinicians, patient safety experts, patients, family members, policy makers, academics–everyone. We wanted a knowledge repository, all in one place, that people could find easily and use to inform their campaigning, their work, their education.” Helen describes how the hub's audience and reach has grown over the three years since it was launched—the hub has had a million page views from people in more than 200 countries, and 450,000 unique users. Although it was started as a UK-based resource, over time more people around the world have found out about it. Helen also discusses Patient Safety Learning's work to make patient safety a core purpose of healthcare, and the vital nature of patient involvement in patient safety.

Content ArticleThis blog on the NHS England website looks at how Written Medicine, a service that provides bilingual medication information, is helping to reduce healthcare inequalities and medical errors in London. Written Medicine’s software allows pharmacies and hospitals to translate and print medication information, instructions and warnings. Drawn from a dataset of 3,500 phrases, printed labels are available in fifteen different languages. The bilingual labels help patients take ownership of their treatment, giving them a better understanding of how to take their prescribed medication. The solution is helping to reduce errors, improve medication adherence and enhance patient safety and experience. The blog also looks at the experience of London North West University Healthcare NHS Trust (LNWH) using Written Medicine. A 2019 audit showed that the service was valued by patients and highly successful in increasing medication adherence through empowering patients.
