Content ArticleHappier teams provide better care to patients. It is now accepted that good culture in the NHS is crucial to ensure that patients receive high quality care and better outcomes. As teams work to improve systems and processes, it is important that teams better understand their own culture to identify what works well and what can be improved. Each maternity and neonatal department in the collaborative is invited to undertake the SCORE survey locally. The survey is an internationally recognised way of measuring and understanding culture that exists within organisations and teams. It is an anonymous, online tool that teams can use to assess their culture. It provides an overview but also detail in specific focus areas such as communication and staff burn out. Once the survey has been completed, the results are provided to that team alone for them to use to start conversations internally about what and how they would like to improve culture. The results are not shared with anyone else and will never be used for bench marking or performance management. The patient safety collaborative also assists with the debriefing the results of the survey to staff.

Content ArticleIn response to growing pressures on healthcare systems, the advanced clinical practice (ACP) role has been implemented widely in the UK and internationally. In England, ACP is a level of practice applicable across various healthcare professions, who exercise a level of autonomy across four domains, referred to as the four pillars of practice (education, leadership, research and clinical practice). A national framework for ACP was established in 2017 to ensure consistency across the ACP role, however current ACP governance, education and support is yet to be evaluated. This study aimed to analyse data from a national survey of the ACP role to inform the development and improvement of policies relating to ACP in the National Health Service (NHS) in England.

News ArticleOne in four doctors in the NHS are so tired that their ability to treat patients has become impaired, according to the first survey to reveal the impact of sleep deprivation on medics during the coronavirus pandemic. Growing workloads, longer hours and widespread staff shortages are causing extreme tiredness among medics, leading to memory problems and difficulty concentrating, according to the report by the Medical Defence Union (MDU), which provides legal support to about 200,000 doctors, nurses, dentists and other healthcare workers. The survey of more than 500 doctors across the UK, carried out within the past month and seen by the Guardian, uncovered almost 40 near misses as a direct result of exhaustion. In at least seven cases, patients actually sustained harm. Despite encouraging signs the Omicron wave may be fading, doctors admitted the constant pressure of the past 22 months spent fighting coronavirus on the frontline was taking a toll on their technical skills and even their ability to make what should be straightforward medical decisions. Medics admitted for the first time sleep deprivation was causing real harm to patients in the NHS. Almost six in 10 doctors (59%) reported their sleep patterns had worsened during the pandemic. More than a quarter (26%) of medics admitted being so tired that their ability to treat patients was “impaired”. Sleep deprived doctors concerned about patient safety

Over a third of doctors say they feel sleep deprived on at least a weekly basis and over a quarter have been in a position where tiredness has impacted their ability to treat patients, a new survey by the Medical Defence Union (MDU) has found. The UK's leading medical defence organisation carried out the survey among its doctor members. Of 532 respondents one in four doctors (26%) said tiredness had affected their ability to safely care for patients, including almost 40 near misses and seven cases in which a patient actually sustained harm. In addition, six in ten respondents said their sleep patterns had worsened slightly or significantly during the pandemic. Dr Matthew Lee, MDU chief executive, said: "Doctors and their healthcare colleagues are running on empty. Our members have come through a period of immense pressure caused by the pandemic and it is affecting all aspects of their life, including sleep patterns. Previous studies have shown that fatigue can increase the risk of medical error and affect doctors' health and wellbeing. In our survey, side effects doctors reported due to sleep deprivation included poor concentration (64%), decision making difficulties (40%), mood swings, (37%) and mental health problems (30%). "Taking regular breaks is vital in the interests of doctors and their patients yet in our survey, three in ten doctors got no breaks at all during the working day despite many working long shifts. In addition, 21% didn't have anywhere to go such as a staff room, or quiet area, to take a break. "Pressures on frontline healthcare workers are likely to get worse for doctors in the coming weeks. At a time of considerable staff absence in the NHS it is more important than ever that those staff who are fit to work are properly supported so they can care for patients safely." UK survey suggests 1.3 million have Long Covid

About 1.3 million people in the UK have Long Covid symptoms lasting more than four weeks after an initial infection, an Office for National Statistics survey suggests. Of those, 892,000 (70%) first caught the virus at least 12 weeks ago and 506,000 (40%) at least a year ago. The survey asked nearly 352,000 people to record their own symptoms. There is no universally agreed definition of long Covid and different studies use varying definitions. The ONS survey, over four weeks in November and December 2021, suggests, of those with Long Covid: 51% have fatigue 37% have loss of smell 36% have shortness of breath 28% have difficulty concentrating. University of Exeter senior clinical lecturer Dr David Strain said: "The stark warning here is that, based on this, in the previous waves, over 800,000 people have their day-to-day activities significantly affected over three months after catching Covid and nearly a quarter of a million report this has a dramatic impact on their quality of life. "As we continue to see case numbers of Omicron rise, we must be wary that our reliance purely on hospitalisations and death as a measure of the risk from Covid could grossly underestimate the public-health impact of our current Covid strategy." Content ArticleHernias are one of the most common surgical diagnoses, and general surgical operations are performed. The involvement of patients in the decision making can be limited. The aim of this study was to explore the perspectives of patients around their hernia and its management, to aid future planning of hernia services to maximise patient experience, and good outcomes for the patient. A SurveyMonkey questionnaire was developed by patient advocates with some advice from surgeons. It was promoted on Twitter and Facebook, such as all found “hernia help” groups on these platforms over a 6-week period during the summer of 2020. Demographics, the reasons for seeking a hernia repair, decision making around the choice of surgeon, hospital, mesh type, pre-habilitation, complications, and participation in a hernia registry were collected. Hernia repair is a quality of life surgery. The survey found that whether awaiting surgery or having had surgery with a good or bad outcome, patients want information about their condition and treatment, such as the effect on aspects of life, such as sex, and they wish greater involvement in their management decisions. Patients want their surgery by surgeons who can also manage complications of such surgery or recommend further treatment. A large group of “hernia surgery injured” patients feel abandoned by their general surgeon when complications ensue.

This involved a nationally representative survey and three two-day citizens' juries that took place in Glasgow, Manchester and Liverpool. The results of the study demonstrate significant public support for proposals that aim to tackle health inequalities through improvements to living and working conditions. There is more limited support for proposals targeting individual behaviour change,

Content ArticleWomen are entitled to clear information on the risks and benefits of different options in order to make informed decisions about the birth of their babies. Rates of induction are rising. One in three pregnancies is induced in Great Britain, according to most recent data. Earlier this year Patient Information Fortum (PIF) members raised concerns about availability of information to support decision-making on induction of labour. PIF responded by collaborating on a survey with maternity charities including Tommy’s, Bliss and Birthrights. The results are sobering and show there is much to do to put personalised care and shared decision making into practice in maternity care.

Republic of Ireland: Most HSE staff felt negative change at work during pandemic

The vast majority of HSE staff in the Republic of Ireland felt supported during the COVID-19 pandemic but more than half felt there has been a negative change in their working environment, a new survey has found. Staff across the health service were asked about their work, and responses from almost 13,000 staff showed a mixed impact since the pandemic with staff saying they were more enthusiastic about their job than in 2018 but were less optimistic about their future in the health service. Three in 10 said they had been subject to assault from the public in the past two years. One in three felt more positively towards the HSE since before the pandemic began. The survey found there had been an increase in the satisfaction with the level of care delivered since 2018 but almost 4 in 10 felt the service delivered was deteriorating. There was a strong sense of job security among staff, but satisfaction levels have fallen back on the previous survey three years ago. A third said they were dissatisfied at present. Despite the fact that an anti-bullying taskforce was set up after the previous survey, the same number of staff reported experiences of being bullied by a colleague as in 2018. Responding to patient alarms in single patient rooms

This survey for health and care staff looks at how quickly staff are aware of alarms emitted by bedside monitoring equipment in single patient rooms, and their ability to respond. Doors to single patient rooms are often kept shut for long periods of time for reasons of privacy, dignity and (at the moment especially) infection control. With the UK Government targeting a growth in the proportion of NHS hospital rooms which have a single bed, is this a risk to the health and wellbeing of patients? This is not a specific issue where data is collected, so an online survey has been created to gather feedback and opinions.- Posted

Content ArticleThe Royal College of Obstetricians and Gynaecologists (RCOG) is undertaking a project to look at the impact of the pandemic on gynaecology waiting lists. The work will look at the size and scale of waiting lists and at the impact this is having on those who are waiting longer for diagnosis and treatment, and on the wider health service. The College would like to hear from people whose gynaecology care has been affected by the pandemic. This might mean you have struggled to get an appointment with or a referral from your GP to see a gynaecologist, waited longer for tests or a scan to receive a diagnosis of a gynaecological condition, or waited longer for treatment or surgery. The College wants to understand your experiences of having a suspected or diagnosed gynaecological condition that isn’t cancer, as ‘benign’ gynaecological conditions are managed differently in the NHS. This could be for conditions such as endometriosis or fibroids, heavy periods, incontinence or recurrent urinary infections – anything you’d be referred to a gynaecologist for. News ArticlePatient safety in the NHS in England is being put at “unacceptably high” risk, with severe staff shortages leaving hospitals, GP surgeries and A&E units struggling to cope with soaring demand, health chiefs have warned. The health service has hit “breaking point”, the leaders say, with record numbers of patients seeking care. Nine in 10 NHS chief executives, chairs and directors have reported this week that the pressures on their organisation have become unsustainable. The same proportion is sounding “alarm bells” over staffing, with the lack of doctors, nurses and other health workers putting lives of patients at risk. Sajid Javid, the health secretary, has come under fire for recently claiming, at a No 10 press conference, that he did not believe the pressure on the NHS was unsustainable. But the survey of 451 NHS leaders in England finds the health service already at “tipping point”. The results of the poll, conducted by the NHS Confederation, which represents the healthcare system in England, Wales and Northern Ireland, show that 88% of the leaders think the demands on their organisation are unsustainable, and 87% believe a lack of staffing in the NHS as a whole is putting patient safety and care at risk. Matthew Taylor, the chief executive of the NHS Confederation, said: “Almost every healthcare leader we’ve spoken to is warning that the NHS is under unsustainable pressure, and they are worried the situation will worsen, as we head into deep midwinter, unless action is taken. Content ArticleThe Ipsos Global Health Service Monitor is an annual study that explores the biggest health challenges facing people today and how well-equipped people think their country’s healthcare services are to tackle them. It ran the survey in 30 countries between 30 August and 3 September 2021. The survey found that public perceptions of healthcare services have not been adversely affected by the pandemic, according to our 30-country survey. Britons are generally happy with the quality of healthcare but are acutely aware of the challenges facing healthcare services.

This survey looks at the experiences of people who stayed at least one night in hospital as an inpatient. The results show that, generally, people’s experiences of inpatient care were positive and overall differences between COVID-19 and non-COVID-19 patients were small, suggesting that care provided was consistent. Most people said they were treated with respect and dignity, had confidence and trust in the doctors and nurses that treated them and observed high levels of cleanliness. Survey findings were less positive, however, for areas of care including people’s experiences of receiving emotional support, information sharing and hospital discharge. Content ArticleBurnout is a serious problem for clinicians as well as the patients who rely on them for safe care, and the challenge has only been compounded by the stresses and trauma of the pandemic. A recent study by Pearl et al. showed that healthcare administrators could use a single survey item to see how their clinicians are doing. The question it asked was, “Are there individuals at your work location who are so burned out that the quality or safety of research, clinical care, or other important work product is impacted?” The respondents’ perception of the impact of burnout on quality safety of healthcare was self-reported using a 5-point system, ranging from 1 (“no burnout or it doesn’t impact safety and quality”) to 5 (“a serious impact on quality and safety”). This nonproprietary, single-item burnout-impacting safety scale showed a sensitivity of 82% using 4 on the scale as a cutoff (“there is quite a bit of impact of burnout on safety and quality”), indicating this tool may be effective in helping determine what healthcare providers may be at high risk for safety events affecting patients.

News ArticleLeaked results from a national survey of NHS staff has revealed a sharp drop in those who believe their health and wellbeing is being supported by their employer. The People Pulse is a national, monthly survey launched in 2020. It enables provider and commissioner organisations to monitor the NHS workforce’s health and wellbeing. According to a snapshot of the results recorded between May and August seen by HSJ, there was a drop of 9.6 percentage points in “perceptions of wellbeing support”, with “positivity” sitting at 57.3%. Almost a quarter of the survey respondents reported a “negative” experience of health and wellbeing support. The survey results also revealed almost a third of respondents said they wanted to speak up about a specific issue during the pandemic, especially on issues of staff safety, health and wellbeing, but they did not because they feared repercussions or believed nothing would happen. Major survey reveals ‘best and worst’ A&Es for patient satisfaction

A survey of almost 50,000 patients by the Care Quality Commission (CQC) found people’s experiences of emergency departments improved in 2020, compared to the last time the poll was conducted in 2018. On a scale of one to 10, the regulator found 33% of patients scored their overall experience as 10, compared to 29% in 2018. Eighty-eight per cent of patients scored their care at six or higher, compared to 85% three years ago. However, overall satisfaction levels declined at around 20 providers. Ted Baker, CQC’s chief inspector of hospitals, said: “This year’s survey shows some encouraging improvements with trust and confidence in clinicians, perceptions of cleanliness and overall experience all performing better than in previous years… “However, the scope for further improvement remains.
