    Following the blistering verdict last week of the independent review into the General Medical Council's (GMC) handling of the notorious 'laptop' case, which highlighted the "worrying trend" of ethnic minority doctors facing disproportionate regulatory action, the GMC has launched a new resource 'hub' to support doctors facing racism at work. A new dedicated area on the GMC website offers advice on how to address racism in the workplace, and sits alongside its existing dedicated whistleblowing webpage as the latest of 12 areas in an 'ethical hub' that brings together resources on how to apply GMC guidance in practice, focussing on areas doctors often query or find most challenging, and helping to address important ethical issues. Announcing the launch, the GMC said: "Tackling discrimination and inequality continues to be an urgent priority for health services." It added: "The GMC has committed to working with organisations to drive forward change, setting targets on tackling inequality." Its equality, diversity, and inclusion targets set last year aimed, inter alia, "to eliminate disproportionate complaints from employers about ethnic minority doctors, by 2026, and to eradicate disadvantage and discrimination in medical education and training by 2031". In March this year it published its first progress report, which showed that the gap between employer referral rates for ethnic minority doctors and international medical graduates, compared with white doctors, had "reduced slightly".     Everyone has the right to come to work without fear of racism. This resource from the General Medical Council (GMC) provides advice on how our guidance principles on non-discrimination apply when tackling racism. Where racist behaviour occurs among colleagues and patients, we recognise the fear that many doctors have of reporting these incidents. It signposts a range of support channels and highlights the duties we expect of doctors in senior positions in tackling and rooting out discrimination where it arises. It includes case studies from doctors and others on their experiences, advice and best practice.

    The only two female ambulance chief executives in the country have said there is something ‘deeply wrong’ with the culture in ambulance services. Helen Ray, the chief executive of the North East Ambulance Service Foundation Trust, said women working in the ambulance service “accept [inappropriate] banter, they accept sexualised behaviour from their male colleagues, and from patients, and they think it is okay”. She stressed “it is absolutely not [okay]” and said women must be given “safe spaces for talking and speaking up about that”. “There is something deeply wrong with the culture in the ambulance service”, she told the NHS Confederation’s Health and Care Women Leaders Network event on Tuesday. Siobhan Melia, interim chief executive of South East Coast Ambulance Service, said when she joined the trust four months ago on secondment from Sussex Community FT, it felt like she had “landed on a different planet”.     Racism is unacceptable and it has no place in health and care. But we know that it exists and that the impact on staff can be devastating. All registered professionals have responsibility under the Nursing and Midwifery Council (NMC) Code to challenge discriminatory behaviour, creating an environment where people are treated as individuals and with dignity and respect. This resource is firmly rooted in our professional Code and it is designed to support nurses, midwives and nursing associates, providing advice on the action you can take if you witness or experience racism. It also supports those in leadership roles to be inclusive leaders. This document provides practical examples of how, as nursing and midwifery professionals, you can recognise, and challenge racial discrimination, harassment, and abuse. It also highlights other useful resources and training materials that will support you to care with confidence. This document is a resource for individuals at all levels. This resource does not replace existing NHS England policies and procedures for speaking up and managing racism. It is a resource to support best practice in line with organisational policies and procedures.

    Further funding cuts to the NHS will unavoidably endanger patient safety, an NHS leader warned last week after the chancellor’s promise of spending cuts of “eye-watering difficulty”. Matthew Taylor, the chief executive of the NHS Confederation, said his members were issuing the “starkest warning” about “the huge and growing gulf between what the NHS is being asked to deliver and the funding and capacity it has available”. The warning came as figures showed that paramedics in England had been unavailable to attend almost one in six incidents in September due to being stuck outside hospitals with patients. Service leaders say wait times for A&E and other care are being exacerbated by an acute lack of nurses, with a record 46,828 nursing roles – more than one in 10 – unfilled across the NHS. "Patients are presenting more unwell," says a GP from South Wales, "Wait times in A&E have become unmanageable, so we’re seeing patients who have waited so long to be seen they’re bouncing back to us. Things we can’t deal with, like injuries and chest pain. We tell them they have to go back to A&E. "Abuse of surgery reception and admin staff began last year and it’s just scaled up from there. We’ve had staff members who have been verbally and physically threatened and we’re struggling to recruit and retain staff – people are hired and quit in a couple days. A lot of people are going off sick with stress."     This study in BMJ Open Quality examines aspects of workplace culture, employee motivation and leadership behaviours that support continuous learning and improvement, in an effort to measure the transition to high reliability. It reports on the development of two scales (trust in team members and trust in leadership) in a US children’s hospital which was seeking to assess progressive movement towards a ‘culture of safety'. The scales were designed to measure two cultural conditions fostered by the five high reliability principles and a composite measure on local learning activities.

    They play a vital role in society, but workers in adult social care – who are mostly women – are among the lowest paid in the UK and experience poor working conditions. This report by The Health Foundation analyses national survey data from 2017/18 to 2019/20 to understand rates of poverty and deprivation among residential care workers in the UK. It then compares these rates to other sectors including health, retail, hospitality and administration. The analysis demonstrates that: over a quarter of the UK’s residential care workers lived in, or were on the brink of, poverty. Nearly 1 in 10 experienced food insecurity. Around 1 in 8 children of residential care workers were ‘materially deprived’, meaning they may not have access to essential resources such as fresh fruit and vegetables or adequate winter clothing. the prevalence of poverty and deprivation in residential care is similar to hospitality, retail and administration. But residential care workers experienced much higher rates than most workers – and were at least twice as likely to experience poverty and food insecurity than health workers. Their dependent children were nearly four times as likely to experience material deprivation than children of health workers. The report highlights that political and economic conditions have changed since the data they looked at was collected, meaning that the situation is likely to have worsened for many social care workers. The poorest households in the UK are being disproportionately affected by sharp rises in inflation and poverty is set to increase. The report also highlights chronic underfunding in the social care sector, particularly in England, and calls on the new Government to make it a priority to ensure social care workers are paid fairly.

    The main corridor of an acute hospital has been closed to patients and staff and turned into a ‘makeshift ward’, in what sources describe as an ‘absolutely unprecedented’ situation. The move by Aintree Hospital comes after staff clashed with paramedics last week about whether ambulance patients could be brought into the crowded emergency department. One staff member, who wished to remain anonymous, said: “It’s exceptional for this to happen, but I can see it happening more over winter. It’s a rock and a hard place… either you wait in the ambulance if the queue is too long, or you wait in the main hospital corridor. Neither option is ideal.” Dr Adrian Boyle, president of the Royal College of Emergency Medicine, said: “Across the country, the urgent and emergency care system is in unprecedented crisis. Emergency medicine teams and our paramedic colleagues are doing their very best to deliver effective care in exceptionally difficult circumstances. Circumstances like these require ICB leaders to engage, take control of the situation and accept their responsibility.     The workforce is healthcare’s most precious resource. Hospitals and health systems are committed to supporting mental well-being and improving access to behavioural health screenings, referrals and treatment when the workforce needs it. This new American Hosptial Association guide, Suicide Prevention: Evidence-Informed Interventions for the Health Care Workforce, identifies three drivers of suicide: stigma, limited access to behavioural health resources and treatment, and job-related stressors. The guide offers a curated list of 12 evidence-informed interventions that hospitals and health systems can implement to reduce the risk of suicide among healthcare workers. Hospitals and health systems should choose the interventions and metrics that work for their organisation based on their own needs and available resources to customise a pathway to suicide prevention for their employees.

    An ambulance service rated ‘inadequate’ by the Care Quality Commission has set out a wide-ranging improvement plan, including ‘civility training’ for senior leaders and ensuring board members hear a mix of ‘positive and negative’ stories from patients and staff. South Central Ambulance Service has been moved into the equivalent of “special measures” by NHS England, in the wake of the Care Quality Commission report in August which criticised “extreme positivity” at the highest levels of the organisation. This means 3 out of only 10 dedicated ambulance service trusts in England are now in segment four of NHSE’s system oversight framework, the successor to special measures. The other ambulance services in segment four are East of England and South East Coast. In a damning inspection report published in August, the care watchdog said that leaders were “out of touch” and staff had faced a “dismissive attitude” when they tried to raise concerns. One staff member told inspectors: “When sexual harassment is reported it seems to be brushed under the carpet and the person is given a second chance. Because of this, a lot of staff feel unsafe, unsupported and vulnerable when coming to work.” An improvement plan summary published at the start of last month included a large number of priorites and actions, including to “ensure [a] mix of positive and negative patient/staff stories are presented to [trust] board meetings” – an apparent attempt to address CQC concerns that its positive outlook could feel “dismissive of the reality to frontline staff”.     In this blog, Saffron Cordery, Interim Chief Executive at NHS Providers, examines progress on the Government's manifesto pledge to build 40 new hospitals in England by 2030. Known as the New Hospital Programme (NHP), many of these projects are facing serious delays, with seven of the 40 not yet having a completion date. In a recent survey by NHS Providers, nearly two in three leaders said delays to the programme affected their ability to deliver safe and effective patient care, with all those facing delays reporting cost implications. Saffron highlights the opportunity the NHP presents to boost healthcare and renew services, and argues that the impact on communities will be huge if the new hospital plans are scrapped.

    On 12 October, 11:00-12:00 CET, the ETUI is hosting a webinar on cancer risks in healthcare workers: identification of Hazardous Medicinal Products (HMPs). In the healthcare sector, 12.7 million workers across the EU are potentially exposed to Hazardous Medicinal Products (HMPs). While these drugs are vital in the treatment of different diseases (for example, cancers and psoriasis), they can also pose health risks to those exposed to them at work such as nurses, pharmacists and cleaners. The ETUI has identified 121 HMPs commonly used in the healthcare sector which can cause cancer or reproductive disorders in professionals exposed to them on a daily basis. Download the report here As the Carcinogens, Mutagens & Reprotoxic Substances Directive (CMRD – Dir (EU) 2004/37/EC) has been recently revised to specifically cover HMPs, the ETUI list of HMPs is timely to raise awareness about these risks in the healthcare sector and help employers use the European guidelines on the safe management of HMPs to be published soon by the European Commission.     Talking General Practice speaks to Dr Helen Garr, medical director of NHS Practitioner Health, the NHS service that looks after doctors and dentists - and also other NHS staff - who are experiencing mental ill health. In this conversation, Helen talk about the impact that pressures on the NHS are having on doctors’ wellbeing and how this is affecting GPs in particular. Helen also explains what doctors and other NHS staff can do if they are suffering from burnout, how to prevent burnout, what people can do if they are worried about a colleague and how NHS Practitioner Health supports doctors who seek help from the service. She also outlines how she thinks the NHS could change to help ensure better mental health for doctors and other staff.

    Record numbers of nurses are quitting the NHS in England, figures show. More than 40,000 have walked away from the NHS in the past year - one in nine of the workforce, an analysis by the Nuffield Trust think tank for the BBC revealed. It said many of these were often highly skilled and knowledgeable nurses with years more of work left to give. And the high number of leavers is nearly cancelling out the rise in new joiners that has been seen. There were just 4,000 more joiners than leavers in the year to the end of June. But a Department of Health and Social Care spokesman said progress was being made and the government was already halfway to meeting its target to increase the numbers of nurses working in the NHS in England during this Parliament by 50,000.     A nurse has told how she almost quit her job this month after a patient shouted out racist slurs for hours on a recent night shift. Beverly Simpson, who works as a nurse in a care home in Derry, Northern Ireland, said she was left angry and broken after a patient repeatedly used derogatory racist language and told her ‘to go back to her own country’. Ms Simpson reported the incident on 4 September to managers, who are now investigating. In the meantime, she has called on all employers and peers to do more to protect staff from racism that she says she encounters every week. "I have been a nurse for almost 30 years, but that night made me feel like I wanted to quit,’ she told Nursing Standard. ‘I was just worn down with it all. I’m human, I am hurt, but I still have to go back and treat this patient, be professional and hold my head up high." "It’s all very well a black nurse standing up and saying it is wrong, but we need allies. There needs to be training and protocols on what to do in these situations.     Join Kayleigh Barnett, Senior Improvement Advisor at Aqua who will share her experience in using Appreciative Inquiry methods to create additional value for learners in a quality improvement (QI) programme aimed at aspiring senior leaders. Appreciative Inquiry is increasingly used as the basis for building a structured learning process and this session will present a case study, and provide practical ideas for you to consider. Ensuring that Appreciative Inquiry processes are included in any part of an organisation can also contribute to psychological safety. Psychological safety is the belief that you won’t be punished or humiliated for speaking up with ideas, questions, concerns, or mistakes. Kayleigh has worked at Aqua for six years and is the Delivery Lead for Appreciative Inquiry. She is an accredited Appreciative Inquiry Practitioner from the International Academy of Appreciative Inquiry. Her other areas of work are quality improvement and human factors. The case study she will present has also been featured in the September edition of the Appreciative Inquiry Practitioner journal. Register
