    Having surgery can be a daunting experience for most people. Staff at the Princess of Wales Hospital in Bridgend, Wales, have recognised this, especially in their patients with complex needs. The reasonable adjustments that they have put in place to ensure their patients receive a bespoke, calming, safe experience won them an NHS Wales Award in 2016 in the Citizens at the Centre of Service Redesign and Delivery category.

It is a national project involving many different organisations which are helping to stop the over use of these medicines. STOMP is about helping people to stay well and have a good quality of life. Psychotropic medicines affect how the brain works and include medicines for psychosis, depression, anxiety, sleep problems and epilepsy. Sometimes they are also given to people because their behaviour is seen as challenging. People with a learning disability, autism or both are more likely to be given these medicines than other people. These medicines are right for some people. They can help people stay safe and well. Sometimes there are other ways of helping people so they need less medicine or none at all.

    The VIP scheme is supported by the learning disability liaison team in the acute hospital to improve health outcomes. The scheme promotes reasonable adjustments, person centred surgical care pathways, accessible information and decision making care plans to support those who may lack the capacity to make health decisions. It also includes VIP champions and promotes evidence based training. The scheme is supported by the Wakefield Patient Experience Group, made up of people who have a learning disability.  The VIP scheme has improved health outcomes of patients with learning disabilities at Mid-Yorkshire Hospitals Trust. The scheme, all though multi-faceted, has one common aim; achieving equality for patients with a learning disability.

    People with learning disabilities, autism or both and their families and carers should be able to expect high quality care across all services provided by the NHS. They should receive treatment, care and support that are safe and personalised and have the same access to services and outcomes as their non-disabled peers. But we know some people with learning disabilities, autism or both encounter difficulties when accessing NHS services and can have much poorer experiences than the general population. Several inquiries and investigations have found that some NHS trusts and foundation trusts are failing to adequately respect and protect people’s rights, with devastating consequences for them and their families. Also, skills deficits in the NHS workforce mean people’s needs are sometimes misunderstood or responded to inappropriately. As a result of these failings, people with learning disabilities, autism or both are at risk of preventable, premature death and a grossly impoverished quality of life. With system partners, NHS Improvement, have developed four standards that trusts need to meet; doing so identifies them as delivering high quality services for people with learning disabilities, autism or both. These standards are supplemented by improvement measures or actions that trusts are expected to take to make sure they meet the standards and deliver the outcomes that people with learning disabilities, autism or both and their families expect and deserve. These four standards are: 1. respecting and protecting rights 2. inclusion and engagement 3. workforce 4. specialist learning disability services.

    Paula McGowan is a Multi Award-winning Activist who following the preventable death of her teenage son Oliver, has dedicated her life to campaigning for equality of Health and Social Care for Learning Disabled people and Autistic people. She is an Ambassador for several charities and organisations. Paula launched a parliamentary petition asking for all doctors and nurses to receive mandatory training in Learning disability and Autism awareness. She ferociously argued that autism must be included. On 22 October 2018, her petition was debated and gained cross party support. As a direct consequence Government announced that all NHS and Social Care Staff would receive The Oliver McGowan Mandatory Training in Learning Disability and Autism.     Difficulty in swallowing—known as dysphagia—is a serious problem for some adults with learning disabilities and, in serious instances, can lead to death. Improving the safety of people with dysphagia is essential, and introducing individual patient management guidelines can reduce the risks associated with this potentially life-threatening condition. This document from the NHS National Patient Safety Agency outlines the issues facing adults with learning disabilities who have dysphagia and introduces support materials that can provide practical help for these people. The tools can be adapted for local use and for any adult who has dysphagia.

    Primary care services are the front door to the NHS - they are the first port of call when we feel unwell and the main coordinator of care when we are living with health conditions. The primary care team have an important role in making people feel welcomed, listened to and taken seriously. Yet we often hear examples about people who have not had their communication needs met within primary care.     The information that care homes submit to CQC about the deaths of people in their care is published on a weekly basis as part of the Office for National Statistics (ONS) reporting on deaths. The ONS data is not broken down by whether the person who died had a disability. Supported by ONS, the Care Quality Commission (CQC) has completed a targeted piece of analysis to better understand the impact of coronavirus (COVID-19) on people with a learning disability, some of whom may also be autistic, and how the number of deaths during this period compares to the number of deaths last year. This analysis looked at all deaths notified to CQC between 10 April and 15 May from providers registered with CQC who provide care to people with a learning disability and/or autism (including providers of adult social care, independent hospitals and in the community), and where the person who died was indicated to have a learning disability on the death notification form.

    NHS investigators are to meet the family of a young, autistic man - left starving and desperately thirsty in hospital while waiting for a delayed operation. Mark Stuart spent five days in agony and died following a catalogue of failings by NHS staff. His parents say they have been battling for answers for four years.  These are the harrowing events that came days before the needless, avoidable death of Mark Stuart. Mark was a young man with autism.
