    I recently wrote a blog for the hub on my experience as a theatre scrub nurse in private healthcare, and what happened to me when I reported a surgeon for dropping an instrument on the floor and reusing it without sterilising it. Following the Paterson Inquiry, I see many similarities in the behaviour and the culture of surgeons and staff in operating theatres. I'd like to share my thoughts.

    Dr Joanna Poole is an Anaesthetic trainee and a Doctors Association UK (DAUK) member. After sharing a blog on Twitter about wanting to quit medicine which went viral, Joanna has been inundated with messages from fellow doctors who have found themselves in a similar situation. Now, Joanna has been invited to share her experiences with multiple Royal Colleges and Joanna is collating the responses she has received anonymously in the hope this will inspire a kinder NHS for our doctors. Joanna is a force for change and is a real example for what grassroots doctors can achieve when they speak up.

    The NHS is Britain’s greatest treasure. Yet it still harbours a culture of hierarchy where bullying, harassment and appalling training environments can go unchallenged. The Doctors Association UK (DAUK) believe that bullying, and discouraging victims from speaking up, goes hand in hand with a blame culture. Often doctors are shamed into silence, and don’t realise other doctors are struggling just as much as they are. Morale is at an all time low in the NHS, with rates of burnout and sadly, even physician suicide on the rise. DAUK are teaming up with the Royal Colleges as part of a wider NHS anti-bullying alliance and are encouraging doctors to speak about their experiences.

    Medical errors are the third leading cause of death in the United States. Putting patients first — listening to their own and their families’ concerns — can help eliminate medical errors altogether. A patient-centric approach encourages patients to communicate their ‘gut feelings’ when something seems wrong, thereby working to end the pervasive and dangerous culture of silence and fear in hospitals.

    “It’s not something we talk about or that everybody experiences to the same degree but I think most of us are affected, be it subconsciously or consciously by antiquated, competitive, hierarchical values. Revered doctors are those that work above and beyond the hours they are paid for, that come in even when they are sick, that prioritise work over their families, over sleep and their own health. Doctors that are kind and compassionate but that don’t allow themselves to be affected by their experiences.     The Right Honourable Sir Anthony Hooper was asked by the General Medical Council (GMC) on 5 September 2014 to conduct an independent review of how the GMC engage with individuals who regard themselves as whistleblowers. The terms of reference were: “To conduct a review of how the General Medical Council handles cases involving individuals who regard themselves as whistleblowers and who have appropriately raised concerns in the public interest. These are individuals: whose fitness to practise is being investigated or determined under the General Medical Council (Fitness to Practise) Rules 2004; or who have reported such a concern to the GMC.” This is the report by the Right Honourable Sir Anthony Hooper to the GMC presented on the 19th March 2015.

    ‘In Safe Hands’ is an interactive guide produced by Health Education England (HEE) who is responsible for delivering education and training that supports safer clinical practice across the NHS. This guide has been produced in response to the recommendations made in the 2016 report ‘Improving Safety Through Education & Training’.

    This cross-sectional study in BMJ Quality & Safety aimed to assess patient comfort in speaking up about problems during hospitalisation, and to identify patients at increased risk of having a problem and not feeling comfortable speaking up. The authors assessed the responses of 10,212 patients at eight hospitals in Maryland and Washington to the question, "How often did you feel comfortable speaking up if you had any problems in your care?" The study found that 48.6% of respondents indicated that they had experienced a problem during hospitalisation. Of these, 1,514 (30.5%) did not always feel comfortable speaking up. The authors concluded that creating conditions for patients to be comfortable speaking up may result in service recovery opportunities and improved patient experience.

    This information sheet produced by South Australia Health's Safety and Quality Unit describes how patients and staff can work together to make sure that if clinical deterioration occurs, it will be acted upon in a timely and effective manner. The information also applies to carers, family members, friends or the patient’s appointed responsible person. It includes information relating to deterioration during an emergency department visit or hospital stay, and at and after discharge.

    In this blog, Jessica Behrhorst, Senior Director for Patient Safety at the Institute for Healthcare Improvement (IHI), discusses challenges staff face in creating a safety culture, such as fear of negative consequences and thinking they will not be taken seriously. She highlights the importance of acknowledging these fears and building positive group norms in order to engage staff. She also highlights the role of root cause analysis in addressing fears about speaking up.

He draws attention to damaging discrepancies between written policy and actual procedure.
