    The NHS Confederation, NHS Providers, the Academy of Medical Royal Colleges, National Voices and the Richmond Group of Charities have penned a joint letter to the new Prime Minister warning that without urgent action on key priorities the NHS risks being trapped in a relentless cycle unable to meet rising patient need and demand. The five organisations, which together represent NHS leaders, clinicians and patients, are calling on the new government to take rapid action to address five key priorities in the short term.  These priorities are: Workforce Social care Capital funding  The impact of the cost of living crisis and inflation Strengthening the voice of people living with ill health in decision making.

    Annual NHS Staff Survey results show an average decline in those feeling optimistic about their career within the NHS. One area of concern, in particular, was the amount of NHS workers who felt they were not adequately recognised and rewarded. This poses a threat to the healthcare provider’s People Promise. As part of the NHS’ People Promise, one of the key values include: ‘we are recognised and rewarded. At a time when much of the NHS is suffering from staff shortages, it is vital to find solutions to boost employee morale. Following this report from Each Person, the NHS has highlighted two key areas that need to be addressed to combat staff dissatisfaction: continuing their advocacy for increased investment and support to raise staff numbers; and relieving points of pressure to foster a positive working culture across the organisation. Consequently, NHS trusts have taken positive steps to ensure that their employees feel more appreciated for their hard work through rewards and recognition.

    The UK has fewer hospital beds than almost any other European comparator and we can ill afford any loss of hospital capacity. While Covid has undoubtedly worsened performance, crowding in emergency departments was a problem before the pandemic. There are frantic attempts to shore up battered and fragile rotas and rota coordinators are scratching their heads. Meanwhile there has been a steady increase in the number of staff off work with Long Covid. While many have been flexible and accommodating to try to maintain their services, there is increasing burnout and uncertainty as to when all this will end. The workforce needs to feel valued and supported, writes Adrian Boyle, a consultant in emergency medicine. There needs to be an acknowledgment that the system is broken.

    In a television studio in Stoke-on-Trent last month, Liz Truss and Rishi Sunak traded blows over everything from credit card economics to Channel migrants to the accessories chain Claire’s. The list of issues the pair clashed over was dizzyingly long. There was one glaring omission, however. In the hour-long debate there was not a single mention of the NHS – despite being engulfed in its biggest ever crisis. The NHS now shares the same traits as many of those relying upon it to keep them alive and well: it is elderly, has multiple comorbidities, and is in dire need of emergency care. Summer has left it on its knees. Worse is expected this winter. “The new prime minister will inherit an NHS in its worst state in living memory,” says Matthew Taylor, the chief executive of the NHS Confederation, which represents the healthcare system in England, Wales and Northern Ireland. “There is no escaping that the NHS is in a state of crisis.”

    The Patients Association was formed over fifty years ago. Since then, it has listened to patients concerns and spoken out on their behalf. Not long after the Patients Association took up its role, legislation was enacted by the government to establish the Parliamentary and Health Service Ombudsman (PHSO). Both organisations have similar values and agendas, intended to help and support the public, the difference being, one is an independent charity, the other a government body afforded all the power and legislation to act with credibility. However, sadly the Patients Association has no confidence that the PHSO will carry out an independent, fair, open, honest and robust investigation. The Ombudsman is frequently quoted as saying patients who suffer harm or poor care in hospitals are failed by a “toxic cocktail” within the health service, whereby complaints go unheard and lessons unlearned. The Ombudsman states: ”We are the last resort for complaints about the NHS. We listen to individual complaints and where things have gone wrong, help to get them put right.” The Patients Association, in partnership with the families of those who have contributed to this report, challenge that statement. Nearly 50 years after the PHSO was established, it is time for real and robust change, not just promises and more recommendations. The Patients Association have a clear request to the Government and Public Administration Select Committee-read our patients stories, listen to their concerns, consider our conclusions, recommendations and finally, hold the PHSO to account for its action.

    “I will deliver on the National Health Service” was as much detail as the new prime minister offered on health policy in her victory speech on Monday. One idea floated during her campaign, that £13bn of new funding earmarked for the NHS should be diverted to social care, might once have won plaudits. But such is the crisis now engulfing hospitals that the moment has passed. Next week, a ballot opens that could see nurses in England and Wales go on strike for the first time. A record 132,139 – or almost 10% of all NHS posts – were vacant in June. This shocking rise in the number of NHS vacancies is a symptom of the government’s dangerous neglect, says the Guardian newspaper in its Editorial.

But with healthcare rates of avoidable harm stubbornly not reducing at the rate we all want, for the benefit of patients we need to strive to achieve more. This extends to how we source, supply and monitor the use of healthcare equipment and products. Procurement and supply chains can be complex and may involve many organisations, with patient safety concerns manifesting themselves in a range of diverse ways. Using value-based procurement (VBP) is one way we can achieve safer care.

    In March 2018, the Secretary-General of the United Nations launched a global call to action for WASH in all healthcare facilities, noting that healthcare facilities are essential tools in reducing disease, and that without basic WASH services, healthcare facilities can instead contribute to more infections, prolonged hospital stays and preventable deaths, including of mothers and babies. This call was answered in a May 2019 World Health Assembly resolution calling on countries to conduct comprehensive assessments of WASH and IPC in health care facilities, and to take steps to improve WASH and IPC conditions where necessary. In May 2022, the World Health Assembly passed a resolution calling for WHO to draft a global strategy on infection prevention and control. The World Health Organization (WHO) and the United Nations Children’s Fund (UNICEF), through the WHO/ UNICEF Joint Monitoring Programme for Water Supply, Sanitation and Hygiene (JMP) release progress updates on WASH in households, WASH in schools and WASH in health care facilities every two years. This 2022 update presents national, regional and global estimates for WASH in healthcare facilities up to the year 2021, with a special  focus on the linkages between WASH and infection prevention and control (IPC).  Achieving universal access to WASH in health care facilities requires political will and strong leadership at both national and facility levels, but is highly cost-effective, and would yield substantial health benefits.

    Earlier this year in March, a nurse from Vanderbilt University, RaDonda Vaught, was found guilty of criminally negligent homicide and gross neglect of a patient. In 2017, Vaught gave 75-year-old Charlene Murphey the incorrect medication. Murphey died as a result. Charlene Murphey’s tragic death highlights the failures of healthcare organisations and their leadership to be trustworthy as well as a fractured and weakened accountability system for patient safety in the United States.

    The dangers of health care in Britain have been long understood. Systematic data collection of the hazards of health care can be traced back at least to the time of Florence Nightingale's publications in the 1860s. This short paper from Susan Burnett and Charles Vincent, outlines the evolution of patient safety and trace its development and progress over the last 10 years in Britain, where a nationalised health service and sustained commitment from Chief Medical Officer Sir Liam Donaldson and other senior figures have brought patient safety to considerable prominence.

    Governments in England, Scotland and Wales recently withdrew covid sick leave for NHS staff. These changes to sick pay provision for staff on Covid-related sick pay is hard to understand at a time when Covid-19 infections are going up exponentially and many NHS organisations are reporting increasing numbers of staff off sick. Evidence is emerging that your chances of on-going issues (Long Covid) following a covid infection increase with each re-infection. Given this you might expect that NHS organisations were ensuring their infection control guidelines guaranteed staff were fully protected against Covid-19. However, in many Trusts this does not appear to be the case. Throughout the pandemic many NHS organisations seem to have focused on following Government guidelines about PPE requirements and ignored their obligations under Health and Safety Legislation. This has resulted in on-going shortcomings in protecting staff at work. This is discussed by Professor Raymond Agius and colleagues in a BMJ blog.

    Health inequalities are not inevitable and are unfair. Many people from different backgrounds across our society suffer health inequalities which can negatively impact the whole community, not just those directly affected. Birmingham and Lewisham African Caribbean Health Inequalities Review (BLACHIR) set out to urgently reveal and explore the background to health inequalities experienced by the Black African and Black Caribbean communities. Birmingham is home to 8% of the Black African and Black Caribbean populations in England and 23% of Lewisham’s population is Black African or Black Caribbean (ONS 2011). The main aim of the Review is to improve the health of Black African and Black Caribbean people in the communities by listening to them, recognising their priorities, discussing, and reflecting on the findings and coproducing recommended solutions for the Health and Wellbeing Board and NHS Integrated Care Systems to consider and respond to.

    The NHS is in trouble today due to an utter failure of leadership in response to the challenge of increasing demand, writes Dr John Carlisle. The Health and Social Care Act was the most extensive reorganisation of the NHS ever. The plan emerged in 2010 when Health Secretary Andrew Lansley began to prepare the new bill, just as the NHS public satisfaction with the NHS was at its highest ever in a polling series that ran back to 1983. Now, ask yourselves, what fool would interfere with any organisation that had such ‘consumer confidence’? Never has the phrase, ‘if it ain’t broke, don’t fix it’ been more apt, particularly for an organisation that employs over a million staff and whose work is critical, and is respected around the world. But Lansley, ignoring international experts like Dr Don Berwick, pushed his own theory (which is all it was) into practice and created the conditions for Jeremy Hunt and his Oxford chum, Simon Stevens, to run the organisation down. Needless to say, the act worked out badly for the poor NHS. By 2018 public satisfaction had dropped to its lowest in 11 years as Hunt and Stevens tried to cobble together the ‘new’ NHS. Just how did this happen?

    In response to the COVID-19 pandemic, the Department for Health and Social Care (The Department) began an unprecedented programme of Personal Protective Equipment (PPE) procurement buying items such as gowns, gloves and masks. It eventually purchased 37.9 billion items at a cost of just over £13 billion. Two years on from that initial procurement activity the Department is still having to manage many of the contracts that it signed. It has now received nearly all of the PPE that it ordered but it is in dispute with many suppliers over the quality of the PPE that has been supplied and is also looking at whether fraud was committed on certain contracts. Much of the PPE still resides in storage locations, both around the UK and in China, and the Department is looking at options for how it might now dispose of some of the stock that it deems to be excess. Responsibility for management of the PPE programme has now largely been transferred back to Supply Chain Co-ordination Limited, the NHS’s main procurement partner prior to the pandemic. This report makes a series of recommendations.
