Content ArticleThe creation of a national network of medical examiners (MEs) was recommended in the Shipman inquiry and was alluded to in the Mid-Staffordshire and Morecambe Bay public inquiries. The Parliamentary Under-Secretary of State for Health, Lord O’Shaughnessy, confirmed in October 2017 that a national system of medical examiners will be introduced from April 2019. The ME reforms set out in the 2009 Coroners Act will be implemented nationally in two phases. By April 2019, NHS trusts should set up non-statutory schemes, based upon the national pilots (particularly in Leicester, Sheffield and Gloucester), funded in part from cremation form fees, in preparation for the commencement of a statutory scheme in 2020/21. A National Medical Examiner will be appointed, reporting directly to the National Director of Patient Safety.

This qualitative study, by Tong et al., of home care patients and their caregivers, published in the International Journal for Quality in Healthcare, revealed concerns about safe care space and ability to address physical needs. These results demonstrate the need for continued focus on safety in home care.

Dr Clare Dollery reflects on a retained swab 'never event' in Churchill Hospital theatres. Incidents, such as operating on the wrong body part or leaving instruments inside patients are categorised by the Department of Health as 'never events'. Dr Dollery is Oxford University Hospital's Deputy Medical Director. The Surgical Grand Rounds lecture series, hosted by the Nuffield Department of Surgical Sciences at Oxford University, is the key educational meeting for consultants, juniors and medical students. Presentations revolve around clinical cases. -
Content ArticleIn February 2019, Claire, a Critical Care Outreach Sister, and Patient Safety Learning's Associate Director, had the great privilege to visit Rush University Hospital, Chicago, as a visiting nurse scholar to shadow the critical care outreach team. The objectives of this visit were to (1) collaborate to harness and direct the strengths of two teams from two different countries, in two unique settings; and (2) to promote best practices that facilitate early recognition and management of patient deterioration to improve patient outcomes. In this blog, Claire talks about this amazing opportunity to see how the UK and USA systems differ and how we can learn from one another.

Rockwood Clinical Frailty Scale

Frailty is a distinctive health state related to the ageing process in which multiple body systems gradually lose their in-built reserves. Around 10% of people aged over 65 years have frailty, rising to between a quarter and a half of those aged over 85 years. Older people living with frailty are at risk of adverse outcomes such as dramatic changes in their physical and mental well being after an apparently minor event which challenges their health, such as an infection or new medication. The Rockwood Frailty scale is a tool to aid clinicians in assessing frailty in adults. -

Content ArticleThis joint project with East Berkshire CCG was highlighted within the AKI Programme within Oxford Patient Safety Collaborative. Fewer residents are suffering urinary tract infections (UTIs) following the introduction of a hydration programme in care homes. UTIs are closely associated with dehydration. This project was designed to encourage residents to drink more fluids with the aim that this would lead to fewer UTIs requiring medication or hospital admission. This approach involved introducing structured drinks rounds seven times a day, designed and delivered by care home staff. The initial focus was in four care homes which had higher than average UTI admission to hospital rates.
