    The Health and Social Care Secretary Steve Barclay has today appointed Dr Henrietta Hughes OBE as the first ever Patient Safety Commissioner for England. Adding to and enhancing existing work to improve the safety of medicines and medical devices, the appointment of a Commissioner is in response to the recommendations from Baroness Cumberlege’s review into patient safety, published in 2020. Dr Hughes will be an independent point of contact for patients, giving a voice to their concerns to make sure they are heard. She will help the NHS and government better understand what they can do to put patients first, promote the safety of patients, and the importance of the views of patients and other members of the public. Health and Social Care Secretary Steve Barclay said: "It is essential that we put patient safety first and continue to listen to and champion patients’ voices. Dr Henrietta Hughes brings a wealth of experience with her as the first ever Patient Safety Commissioner to improve the safety of medicines and medical devices and her work will help support NHS staff as we work hard to beat the Covid backlogs."     The NHS's approach to tackling children’s mental health is “threatening to overwhelm the social care system”, the president of the Association of Directors of Children’s Services has warned. Steve Crocker believes the NHS is “not doing a very good job” for children, describing how children are typically now waiting four months for a mental health assessment and over a year for treatment as being “simply not good enough”. He admitted he was being “deliberately provocative” around children’s mental health at the opening of the ADCS conference yesterday, as he wants to see “more collaboration” from the new Integrated Care Systems (ICSs), which were put on a statutory footing this month. Mr Crocker warned delegates that under the ICS reforms, there is an “ongoing risk that the needs of children are sidelined by the ongoing pressure in acute adult services”. “The House of Lords amendment ensuring each ICS has a children’s strategic lead was a welcome development, but does it go far enough?” he asked. Mr Crocker told LGC: “Children's mental health should be a priority for every ICS in the country. I can't imagine any reason why any ICS would not do that."     Hospital leaders say they have been pressured to deliver more ‘corridor care’ as a result of efforts to ease the ambulance handover crisis. Due to the collapse in ambulance response times over the last year, hospitals have been told to receive patients from ambulance crews more quickly, to enable those crews to respond to new incidents in the community. This can mean patients being kept on trolley beds in corridors, with a lack of appropriate staff to care for them. Tracy Bullock, chief executive of University Hospitals of North Midlands, told HSJ her trust almost eradicated “corridor care” before the pandemic. But she added: “There have been discussions about going back to corridor care, but we have resisted that, as it brought significant patient safety and staff wellbeing issues… although these never received the same airtime as ambulance waits as they are unseen and only impact on the acutes. “The terminology has now changed and instead of corridor care it’s ‘cohorting’, and the space is not necessarily a corridor but a designated space for ambulances to drop more patients off.” She said this only works with enough staff, “otherwise you end up with the same safety issues that we had delivering corridor care pre-pandemic”.     Steve Barclay has been named as the new health secretary following the resignation of Sajid Javid, who stepped down after saying he had lost faith in Boris Johnson's leadership. He starts as secretary of state at a time when the NHS and social care in England are under serious pressure. Amanda Pritchard, the head of NHS England, has warned that the next two years could be even tougher for the health service than the two years since the start of the pandemic. NHS Providers, which represents hospitals and other NHS trusts, described the problems Mr Barclay faces on his first day in the job as "big and pressing".     Sajid Javid has resigned and been replaced by Steve Barclay as health and social care secretary. Mr Barclay, previously chief of staff to the prime minister, was reported as having been given the role on Tuesday evening. Mr Barclay said: “It is an honour to take up the position of Health and Social Care Secretary. Our NHS and social care staff have showed us time and again - throughout the pandemic and beyond - what it means to work with compassion and dedication to transform lives. This government is investing more than ever before in our NHS and care services to beat the Covid backlogs, recruit 50,000 more nurses, reform social care and ensure patients across the country can access the care they need.” Mr Barclay’s appointment came hours after Mr Javid — health secretary since June last year — announced his resignation, saying in a letter to Boris Johnson that he “can no longer, in good conscience, continue serving in this government” and that Mr Johnson has lost his confidence.     The outgoing chief investigator of the national safety watchdog has described his frustration with the organisation’s ‘ambivalent’ relationship with NHS England. Keith Conradi, who is due to retire from the Health Safety Investigation Branch in July, said he did not think he had “ever really spoken to any of the hierarchy in NHS England”. He added “their priorities are elsewhere”. In an interview with health commentator Roy Lilley for the Institute for Health and Social Care Management, Mr Conradi also described HSIB’s relationship with NHSE as “ambivalent”. “It wobbled along that sort of line and got worse as time has gone on,” he said. “At the very start I had a chat with the permanent secretary of the Department of Health and said we would be better off in the department than NHS England. He disagreed and felt that we’d be too close to [then health secretary] Jeremy Hunt, and particularly at that time that would have a negative effect.” Mr Conradi was also critical of the decision to ask HSIB to take on investigations into maternity care early in its life. He said he was “shocked” that it happened so quickly “when we hadn’t really got going”. He continued: “We hadn’t developed a method of doing normal national investigations and suddenly we were being asked to do maternity ones.     A drive to ‘transform’ access to urgent, emergency and planned care will be added to the goals of the NHS long-term plan, a document leaked to HSJ has revealed The long-term plan for the NHS was originally published in January 2019. Last September, NHS England said it was reviewing the commitments made within the plan, with senior officials warning that many of them could not be met after the damage of the pandemic. HSJ has seen a document prepared for the most recent meeting of the NHS Assembly which sets out NHSE’s approach to the refresh. Strategic developments expected include better joined-up community based and preventive care, transform access to urgent, emergency and planned care, improve care quality and operations, and tackle health inequalities, improve population health and develop a sustainable health service through greater collaboration.     An ambulance trust has been placed in special measures after the Care Quality Commission (CQC) rated its leadership ‘inadequate’ and said staff felt unable to raise concerns without fear of reprisal The CQC inspected South East Coast Ambulance Service Foundation Trust after being contacted by staff with concerns about bullying and harassment, inappropriate sexualised behaviour and a leadership team which failed to address concerns. Many of the concerns echo those raised in 2017 in an independent review into a “culture of fear” at the trust, shortly after it was first placed in regulatory special measures. It was taken out in 2019 but has now been placed back in the equivalent “recovery support programme” on the CQC’s recommendation. CQC director of integrated care Amanda Williams praised staff who had contacted the regulator. She said: “While staff were doing their very best to provide safe care to patients, leaders often appeared out of touch with what was happening on the front line and weren’t always aware of the challenges staff faced. Staff described feeling unable to raise concerns without fear of reprisal – and when concerns were raised, these were not acted on. “This meant that some negative aspects of the organisational culture, including bullying and harassment and inappropriate sexualised behaviour, were not addressed and became normalised behaviours."     Systems and processes in place around patient safety failed in terms of the work of a Belfast-based neurologist, an inquiry has found. Dr Michael Watt was at the centre of Northern Ireland’s largest ever recall of patients, which began in 2018, after concerns were raised about his clinical work. More than 4,000 of his former patients attended recall appointments. Almost a fifth of patients who attended recall appointments were found to have received an “insecure diagnosis”. The final report following the Independent Neurology Inquiry found that problems with Dr Watt’s practice were missed for years and opportunities to intervene were lost. It makes 76 recommendations to the Department of Health, healthcare organisations, General Medical Council and the independent sector. “While one process or system failure may not be critical, the synergistic effect of numerous failures ensured that a problem with an individual doctor’s practice was missed for many years and, as this inquiry finds, opportunities to intervene, particularly in 2006/2007, 2012/2013, and earlier in 2016 were lost,” the inquiry found.     Henrietta Hughes has been named as the government’s preferred candidate for the role of Patient Safety Commissioner. Sajid Javid, the Secretary of State for Health and Social Care, has today, 20 June 2022, invited the Health and Social Care Committee to hold a pre-appointment scrutiny hearing with Henrietta. Henrietta is a practising GP with a background in women’s health who was the National Guardian for the NHS until 2021. In addition to her clinical work, she is an appraiser for NHS England and Chair of Childhood First. She was selected following an open public appointment process to appoint the first Patient Safety Commissioner. Following the select committee hearing, the committee will set out its views on the candidate’s suitability for the role.     Just over half of senior ethnic minority leaders have considered leaving the NHS due to experiencing workplace racism a survey suggests. The survey was carried out by the NHS Confederation’s BME Leadership Network and its 123 respondents included chief executives, directors and senior managers. Responses were collected from network members online before three roundtables were held with senior ethnic minority leaders to understand their experiences and the challenges they have faced in relation to discrimination. The survey found: 51% of respondents said they had considered leaving the NHS in the past three years because of their experience of racist treatment while working. More than 20% said they had experienced verbal abuse or abusive behaviour targeting racial, national or cultural heritage five times or more in the last three years. 69% had experienced this behaviour from other leaders or managers within their organisation at least once in the same timeframe. 57% had experienced it from leaders or managers in another organisation at least once over the same period. Joan Saddler, NHS Confederation’s director of equality and partnerships, said the NHS was at risk of losing “committed, highly skilled and motivated talent to institutional racism and discrimination”.     One of the trusts worst affected by coronavirus has been issued with two warning notices and rated ‘inadequate’ for leadership, following a Care Quality Commission inspection. The regulator raised serious concerns about the safety of Countess of Chester Hospital Foundation Trust’s maternity services, as well as the oversight and learning from incidents. It also found staff were experiencing multiple problems with a newly installed electronic patient record, while systems for managing the elective waiting list were said to be unsuitable. In maternity services, the inspectors flagged severe staff shortages and a failure to properly investigate safety incidents. They said there were three occasions during the inspections when the antenatal and post-natal ward was served by only one midwife, despite the interim head of midwifery saying this would never happen. Inspectors also highlighted five incidents last year where women had suffered a major post-partum haemorrhage, involving the loss of more than two litres of blood and which resulted in an unplanned hysterectomy. The CQC said two were not reported as serious incidents, and where learning had been identified from the others, action plans were not being completed on time. The CQC said it was only made aware of the incidents by a whistleblower, while internal actions agreed in December 2021 had still not been implemented two months later.     The chief executive of one of the first teaching trusts in the country to have eliminated two-year waiters for elective care has said there is ‘no magic to it’ and it can be replicated elsewhere. Since the beginning of April, University Hospitals of Coventry and Warwickshire Trust has reported zero patients waiting over two years for their elective treatment – ahead of NHS England’s target of July 2022. According to the latest data, there are now 42 trusts that have eliminated 104-week waits and UHCW is the largest trust to have done this. UHCW chief executive Andy Hardy said that in order to achieve this the trust had been “relentless” in its focus on waiting times and had set up “bootcamps” to help managers understand how referral to treatment works. Mr Hardy said in an interview with HSJ: “It really does come down to a laser-like focus on waiting times, both at an executive level, down to a group level, and down to speciality level. It can be replicated. There’s no magic to it.” He said: “We use data to drive our organisations away from bad decisions and I have a weekly access meeting with the chief operating officer to look at where we are against all access targets, but obviously we focus on waiting times."     Ministers, including the health and social care secretary, are bashing managers and exploiting the culture wars to try to ‘explain away’ the crisis in the NHS instead of facing up to the problems that 12 years of Conservative-led government have created, NHS Confederation chief executive Matthew Taylor has told HSJ. During a wide-ranging and exclusive interview with HSJ, MatthewTaylor also raised concerns about the Messenger review and called on local system leaders to speak out if they thought NHS England was setting unrealistic financial targets The Confed CEO told HSJ: “When you’ve been in government 12 years, to acknowledge the scale of the problems which now exist, problems which clearly reflect decisions made across those 12 years, is a hard thing to do politically. “If you can’t recognise that the fundamental reasons [underpinning why] we face this yawning capacity gap are to do with, particularly, the decade of austerity, but [also] other failings to address capacity issues like workforce and capital, then you have to look for other culprits and you end up manager bashing and talking about wokery, because it becomes a way to explain away the reality that the patients and the public see.” Read full story (paywalled) Source: HSJ, 14 June 2022

    Coordination of waiting lists and elective treatment across health systems and regions should be ‘far more systematic’, and could have happened earlier, chief executives of some of the hardest hit trusts have told HSJ. In interviews for the HSJ Health Check podcast, the CEOs of King’s, Croydon, Chester and Sandwell and West Birmingham hospital trusts spoke about their experience in the pandemic and what could be learned from it. These included the need for faster decision making; resources for deprived and diverse areas, which are often hardest hit; the need for basic staff facilities such as parking and eating areas for staff; longer-term attention to the wellbeing of staff who were most affected; and to give time for trusts to recover. On elective care, the CEOs highlighted how the length of lists and waits, and the NHS’s ability to keep up, are now much worse in some areas than others. Some of those with the longest waits and lists at present – such as Countess of Chester and Birmingham – were also heavily hit by Covid; for others this is not the case. There were moves, particularly later in the pandemic, for patients who were on the elective waiting list of one trust to be treated at another, for example if they needed urgent treatment and faced harm if delayed, while other hospitals were still able to treat less urgent cases. Combining lists, often known as “shared patient tracking (or treatment) lists”, could also mean capacity being managed more efficiently across providers.     Sajid Javid’s claim that the number of NHS roles dedicated to promoting equality and diversity should be cut is incorrect and not what the government-commissioned review into NHS management recommended, according to its author. The review by General Sir Gordon Messenger and Leeds Teaching Hospital chair Dame Linda Pollard was published Wednesday. Speaking to the Daily Telegraph on Tuesday evening, the health secretary said: “In my view, there are already too many working in roles focused solely on diversity and inclusion, and at a time when our constituents are facing real pressures around cost of living, we must spend every penny on patients’ priorities. “As this report sets out, it should be the responsibility of everyone to encourage fairness and equality of opportunity which is why we must reduce the number of these roles.” Speaking later to HSJ, Mr Javid was asked if there was any area of NHS management cuts should be made. He said: “I would like to see fewer managers in terms of diversity managers and things, because I think it should actually be done by all management and all leadership, and not contracted out as some kind of tick-box exercise.” However, when HSJ spoke to General Messenger he said: “The report does not recommend the reduction of EDI (equality, diversity and inclusion) professionals. “What it does say though, is that if one successfully inculcates equality, diversity and inclusion to every leadership’s responsibilities then that becomes an accepted, instinctive, understood part of being a leader and a manager at every level then the requirement for dedicated EDI professionals should reduce over time."     A number of hospitals are insisting that patients keep wearing masks despite instructions from NHS chiefs to drop the rules. National coronavirus guidance which insisted on face coverings has now been scrapped, with health officials leaving it to local organisations to draft their own policies. However, several hospitals have called on patients and staff to continue to wear masks and face coverings on their sites. Sajid Javid, the Health Secretary, has repeatedly called on NHS trusts to drop restrictions in hospitals which are limiting operational capacity. Last month, he threatened to name and shame hospitals that do not lift social distancing measures and restrictions on visitors. A letter from health chiefs said that patients visiting accident and emergency (A&E) departments, hospital outpatient appointments and GP surgeries no longer needed to wear masks “unless this is a personal preference”. Hospitals have now begun issuing guidance for their local communities, with a number saying they intend to keep insisting on people wearing masks. The Sheffield Teaching Hospitals NHS trust issued a notice to patients and staff saying: “We are still asking patients, visitors, staff and anyone working at one of our hospital or community sites to continue to wear a mask, gel hands and social distance while in our buildings despite the lifting of national restrictions." “This is to keep vulnerable people as safe as possible.” Read full story (paywalled) Source: The Telegraph, 8 June 2022

    All the NHS’s 1.5m staff in England should tackle discrimination against disadvantaged groups, not just bosses and specialist diversity teams, a major review has concluded. NHS trusts will need fewer equality, diversity and inclusion (EDI) teams if action against discrimination does become “the responsibility of all”, according to the report. The review of NHS leadership said the health service should adopt a different approach to equality issues in order to overcome the widely recognised disadvantages faced by certain groups of its own staff, which include lower pay and chances of promotion among Black and ethnic minority doctors compared with white medics and low BAME representation in senior managerial ranks. The inquiry, undertaken by Genl Sir Gordon Messenger and Dame Linda Pollard, was commissioned last year by Sajid Javid, the health secretary. The report concluded that: “Most critically, we advocate a step-change in the way the principles of equality, diversity and inclusion are embedded as the personal responsibility of every leader and every member of staff. “Although good practice is by no means rare, there is widespread evidence of considerable inequity in experience and opportunity for those with protected characteristics, of which we would call out race and disability as the most starkly disadvantaged. “The only way to tackle this effectively is to mainstream it as the responsibility of all, to demand from everyone awareness of its realities and to sanction those that don’t meet expectations.” Read full story Source: The Guardian, 8 June 2022

    A government review of health and care leadership has recommended a single set of ‘core leadership and management standards’ for NHS managers. The report by General Sir Gordon Messenger and Dame Linda Pollard calls for “consistent management standards delivered through accredited training”, according to a government statement this morning. The full document has yet to be published but the statement summarises the findings and says an “institutional inadequacy” has formed in the way leadership and management is trained and developed in the NHS. It says the report has produced seven recommendations, which have all been accepted in full by the health and social care secretary Sajid Javid, who said they must be taken forward “urgently”. Among them is a call for a more “effective and consistent” appraisal system to reduce variation in how performance is managed. This is after the review concluded a greater focus was needed on “how people have behaved [and] not just what they have achieved”. The recommendations do not include any registration system for NHS managers, despite calls from some over many years for more regulation of the roles, nor appear to include specific reform of the “fit and proper person” test, which has been discredited and under review.     A GP surgery that provides treatment to about 5,600 patients has been placed in special measures by a regulator. London Street Surgery, in Reading, Berkshire, was found to have "significant backlogs of test results and care-related tasks". The Care Quality Commission (CQC) found there was "poor identification of risks to patients" during an inspection in April. The surgery has been approached for comment. The regulator rated the surgery's safety and leadership as inadequate, and said it had insufficient processes to ensure services' safety and effectiveness. Repeat prescriptions and medicines were "not managed safely", which could have posed risks to patients, and there were "risks associated" with the storage of blank prescriptions, it found. Staff training was "not monitored appropriately" and inspectors found patients with learning disabilities were not provided with health checks to make sure their wellbeing was properly monitored.     Regulators have raised serious concerns over trainee doctors within the maternity department at one of the largest trusts in the country. The NHS’ training regulator said it had concerns over the treatment of trainee doctors within the obstetric and gynaecology department at University Hospitals Birmingham Foundation Trust, while some medics report being in ‘meltdown’. Reviewers raised an incident where a consultant had refused to respond to an obstetric emergency in A&E which had been requested by a junior doctor. “The panel unanimously agreed that Consultant presence was required without delay,” the report added. The latest review follows concerns in November 2020 and June 2021 when patient safety issues were also identified.     Ministers and NHS England have not sufficiently warned the public of the risk to patient harm posed by next week’s junior doctors strike, some of the NHS’s most senior trust chief executives have warned. The senior leaders contacted HSJ with their concerns after a group call between trust leaders and NHSE bosses on Thursday. The chief executives and medical directors, who spoke to HSJ on condition of anonymity, made a series of robust criticisms which focussed on the lack of awareness of danger presented by the junior doctor’s industrial action, a lack of thorough communication of that to the public, and the insistence that trusts negotiate strike agreement with the British Medical Association at a local level. One comment on the chat function stated: ”Public awareness of the impact of this strike seems far lower than for e.g. the ambulance strike, but from a an acute trust perspective this will have a much bigger impact on patient care and safety.     A trust chief executive has suggested an inquiry team looking at 2,000 deaths is lacking in “expertise” and has created a “disproportionate impression” of the problems at his trust. Essex Partnership University Trust is at the centre of a high-profile inquiry into the deaths of patients over a 20-year period, which was sparked after serious concerns were raised over specific cases. The inquiry, led by Geraldine Strathdee, a former national clinical director for mental health, is reviewing the cases of 2,000 people who died while they were patients on a mental health ward in Essex or within three months of being discharged. In a letter to the inquiry, obtained by HSJ through a freedom of information request, trust chief executive officer Paul Scott wrote: “The headline number of c.1,500 or c.2,000 deaths used in publicity by the inquiry is, in my opinion, not a fair reflection of the deaths that would be of interest to the inquiry.” Read full story (paywalled) Source: HSJ, 1 March 2023

    Three women who died under the care of a hospital's maternity unit may have survived if earlier recommendations had been implemented, a report has said. The cases occurred at University Hospitals of Derby and Burton (UHDB) NHS Foundation Trust over 16 months. A review by the Healthcare Safety Investigation Branch (HSIB) also found a culture of intimidation and bullying. The report found that although there was no common theme to the deaths - and four other life-threatening cases that occurred in the same period - processes and leadership had been inconsistent and fragmented. HSIB said "robust action planning and prompt addressing of the learning" from previous recommendations from other investigations "may have had an impact on the outcome for the women who received care during the seven events included in this thematic review".
