Content ArticleThree years since we launched the hub, our award-winning platform to share learning for patient safety, we have seen it grow in members, content and impact. To date, the hub has received over 565,000 visits and had over 1 million page views. It now has over 3,400 members from 80 countries working in over 1,000 different organisations, and offers 7,500 knowledge resources, viewed by people from 221 countries. We continue to highlight serious patient safety issues, celebrate patient safety achievements, provide ‘how to’ resources on good practice and offer a safe space for staff and patients to share their experiences and discuss challenges. In this blog, we would like to celebrate just some of the work we are especially proud of and highlight where we’ve been making the case for change and the many ways the hub is making an impact.

‘Know, Check, Ask’ before you take medication urges Northern Ireland's Minister

Both patients and healthcare staff have a central role to play in ensuring the safe use of medicines, Health Minister Robin Swann has said. Minister for Health Robin Swann was speaking at an event to mark the roll out of the ‘Know, Check, Ask’ Campaign across all healthcare sectors in Northern Ireland. The aim of the campaign is to increase awareness and understanding about the importance of using medicine safely. The call for action of the campaign is for: Patients to Know Check Ask – Before you take it: KNOW your medicines and keep an up-to-date list. CHECK that you are using your medicines in the right way. ASK your healthcare professional if you’re not sure. Health Care staff to Know Check Ask – Before you give it: KNOW your medications. CHECK you have the right: patient, medicine, route, dose and time. ASK your patient if they understand and ask your colleagues when you are unsure. Minister Swann added “I want to encourage and help patients to be more curious about their medication, know what medication they are using, how to use it safely and feel able to ask their health care professionals questions about their medicines. News ArticleHealth professionals should not let fears about sharing personal data “stand in the way” of reporting patients at risk of ”being groomed into terrorist activity”, new government guidance has stressed. New guidance has been developed in response to concerns raised by clinicians about information sharing without consent for the purposes of the anti-terror Prevent and Channel programmes. It stresses that “fears about sharing personal data should not be allowed to stand in the way of the need to safeguard and promote the welfare of children and adults at risk of abuse or exploitation”. However, despite the need for clarity it describes how the decision for making a referral without someone’s informed consent should be subject to a “case-by-case” basis assessment, which considers whether the informed consent of the individual can be obtained, and if the proposed data sharing is legitimate, necessary, proportionate and lawful. News ArticlePhrases such as “cutting edge,” “game changing,” and “ground breaking” have no place in the description of new drugs by the government and NHS agencies, a therapeutics specialist and GP has warned. James Cave, editor in chief of the Drug and Therapeutics Bulletin (DTB), said in an editorial1 that the degree of hyperbole and omission of important information in government press releases and media statements “leaves patients and healthcare professionals with a limited and unbalanced view of a medicine.” In a letter to the heads of NHS England, the National Institute for Health and Care Excellence (NICE), and the Medicines and Healthcare Products Regulatory Agency (MHRA) he referred to a loss of objectivity in statements about new drugs over the past few years. Content ArticleThis guidance from the Irish Health Services Executive (HSE) aims to help healthcare staff improve venous thromboembolism (VTE) prevention in hospitals. Hospital-acquired blood clots, or VTE, are the most common preventable cause of in-hospital death. Assessing patients’ risk of VTE and bleeding and choosing the appropriate thromboprophylaxis such as medicines or compression stockings early in their hospital admission reduces their risk of developing a blood clot.

Content ArticleIn this interview for Pharmacy Update Online, Patient Safety Learning's Chief Executive Helen Hughes talks about how the hub was established to provide free, easily-accessible information about patient safety for everyone. "By everyone we mean literally everyone–the hub was designed by and for clinicians, patient safety experts, patients, family members, policy makers, academics–everyone. We wanted a knowledge repository, all in one place, that people could find easily and use to inform their campaigning, their work, their education.” Helen describes how the hub's audience and reach has grown over the three years since it was launched—the hub has had a million page views from people in more than 200 countries, and 450,000 unique users. Although it was started as a UK-based resource, over time more people around the world have found out about it. Helen also discusses Patient Safety Learning's work to make patient safety a core purpose of healthcare, and the vital nature of patient involvement in patient safety.

Content ArticleThe non-profit Patient Information Forum (PIF) has published a new one-page guide to Body Mass Index (BMI). The poster was developed following user engagement sessions with patients and healthcare professionals which highlighted the amount of misinformation surrounding BMI. It is free to download and share and can be used directly by patients or a resource for healthcare professionals. Welcoming the publication of 'BMI – What you need to know', Dr Juhi Tandon said: “As a GP for more than a decade, I still struggle to have the BMI conversation with patients. Discussing someone’s BMI can easily make them feel uncomfortable as they feel like they are being judged. It will be very helpful to share a clear fact sheet to help patients understand more about BMI in a non-judgemental way.”

Content ArticleThis blog on the NHS England website looks at how Written Medicine, a service that provides bilingual medication information, is helping to reduce healthcare inequalities and medical errors in London. Written Medicine’s software allows pharmacies and hospitals to translate and print medication information, instructions and warnings. Drawn from a dataset of 3,500 phrases, printed labels are available in fifteen different languages. The bilingual labels help patients take ownership of their treatment, giving them a better understanding of how to take their prescribed medication. The solution is helping to reduce errors, improve medication adherence and enhance patient safety and experience. The blog also looks at the experience of London North West University Healthcare NHS Trust (LNWH) using Written Medicine. A 2019 audit showed that the service was valued by patients and highly successful in increasing medication adherence through empowering patients.

Dr Penny Kechagioglou, Chief Clinical Information Officer and Deputy Chief Medical Officer at University Hospitals Coventry and Warwickshire, kindly shared her thoughts on digitising patient reported outcome measures in a blog for HTN. The UK digital transformation wave is mainly characterised by the roll-out of electronic health records and is an opportunity to transform patient care by collecting and analysing patient reported outcome measures digitally. A recent study at the European Society of Medical Oncology open journal (Modi, 2022) showed that patient reported outcome measures are predictive of cancer patient treatment response and quality of life for physical and mental parameters. The knowledge of patient reported outcomes (PRO) and experience (PRE) measures can be valuable in the monitoring of individual patient symptoms in clinic or remotely in the community and also for aggregating and interpreting population health data. This website from the Association for Young People's Health (AYPH) aims to provide useful data about young people’s health for healthcare professionals, researchers and other professionals working with young people. At its heart is a data compendium called ‘Key Data on Young People’s Health’ produced AYPH, which gives up to date national data on key health outcomes for 10-24 year olds. The Professional Record Standards Body (PRSB) has published the final draft standard for 111 referral, which defines the information that should be shared from 111 or 999 services when a person is referred on to another service. The standard applies to: all 111 and 999 service referrals to wherever the person goes next. referrals through 111 online, call handler or clinical assessment services and 999 services, and is not specific to any triage system. all age groups including children. The standard is UK-wide and was developed in consultation with a wide range of professionals from all four nations, including from 111 services, receiving services, IT suppliers and people who use services. It does not apply to transfers between 111 services (e.g. across a country border) or between 111 and 999 services.- Posted
