    A significant number of doctors and other healthcare workers have developed post-acute COVID, including a large number who developed it as a result of workplace exposure to Covid-19. This paper examines the impact post-acute Covid symptoms have had on the medical workforce, personally and professionally. It provides a unique and valuable insight into the experience of UK doctors suffering from post-acute Covid. It has been informed by a UK survey of over 600 doctors suffering from the continuing effects of an infection with Covid-19, as well as wider research of the issues. The survey was undertaken by the British Medical Association (BMA) in partnership with Long COVID Doctors for Action. This is the first comprehensive survey of doctors with post-acute Covid health complications. This paper should inform the support needed by current sufferers of post-acute COVID in the NHS workforce, and help protect services and patients now and in the future.

    A major teaching trust is dominated by a “medical patriarchy”, while “misogynistic behaviour” is a regular occurrence, two investigations have discovered. Two reports into University Hospitals Birmingham Foundation Trust have been published. They are the outcome of an investigation into the trust’s leadership carried out by NHS England, and an oversight review by former NHSE deputy medical director Mike Bewick. They follow major concerns being raised over recent months about safety, culture, and leadership at the trust. The NHSE review said the trust “could do more to balance the medical patriarchy that dominates” the organisation. It noted consultants are invited to observe a chief executive’s advisory group meeting, but nursing, midwifery and allied health professional leaders are not.” On culture, NHSE said the trust should take steps to ensure staff can work in psychologically safe environments where “poor behaviours are consistently addressed” and to “eradicate bullying and cronyism at all levels of the organisation”.     A number of serious concerns were raised about the University Hospitals Birmingham NHS Foundation Trust, relating to patient safety, governance processes and organisational culture. The Trust has been under review by the Birmingham and Solihull Integrated Care Board (ICB), following a junior doctor at the trust, Dr Vaishnavi Kumar, taking her own life in June 2022. In response to these concerns, a series of rapid independently-led reviews have been commissioned at the Trust.  A follow up report into concerns raised about University Hospitals Birmingham NHS Foundation Trust has now been published showing the progress made against the recommendations made in the clinical safety (phase 1) report. It also collates the evidence from phase 2 and 3 of the review and assesses how the lessons learned can at this point be incorporated into the recovery and development plan that the Trust is already progressing. It also takes account of any other concerns that have arisen or been communicated to the review team.

    Staff sickness in the NHS in England has reached record levels. Figures for 2022 show an absence rate - the proportion of days lost - of 5.6%, meaning the NHS lost the equivalent of nearly 75,000 staff to illness. This is higher than during the peak pandemic years of 2020 and 2021 - and a 29% rise on the 2019 rate. Mental health problems were the most common cause, responsible for nearly a quarter of absences, the Nuffield Trust analysis of official NHS data shows. Big rises were also seen in cold, coughs, infections and respiratory problems, likely to be linked to the continued circulation of Covid as well as the return of flu last year. The think tank warned the NHS was stuck in a "seemingly unsustainable cycle" of increased work and burnout, which was contributing to staff leaving.     More than half of UK doctors have seen or experienced abuse by patients or their relatives in the last year, including incidents in which they have been spat at and threatened. Doctors have variously had their hair ripped out, been backed up against a wall and been racially abused, a survey and dossier of testimonies collated by a medical organisation has revealed. Long delays for care and staff shortages are cited as the main triggers for what NHS leaders say is an increased readiness by the public to be aggressive towards frontline staff. The research by the Medical Protection Society (MPS) found that 56% of the doctors questioned had experienced or witnessed a situation involving verbal or physical abuse over the last year. Almost half said incidents had occurred because of a lack of staff, while 45% blamed it on patients’ frustration at having to wait a long time to be treated. One doctor told the MPS how a “patient’s partner threatened to kill me as he felt his wife had waited too long to be seen”, while another said: “I had a handful of my hair ripped out despite the patient being in handcuffs and with the police.” Read full story Source: The Guardian, 22 June 2023

    One in 10 health workers in England had suicidal thoughts during the Covid-19 pandemic, according to research that highlights the scale of its mental impact. The risk of infection or death, moral distress, staff shortages, burnout and the emotional toll of battling the biggest public health crisis in a century significantly affected the mental wellbeing of health workers worldwide. A study involving almost 20,000 responses to two surveys reveals the full extent of the mental health impact on workers at the height of the pandemic. Research led by the University of Bristol analysed results from two surveys undertaken at 18 NHS trusts across England. The first was carried out between April 2020 and January 2021 and completed by 12,514 workers. The second – covering October 2020 to August 2021 – was completed by 7,160. The first survey found that 10.8% of workers reported having suicidal thoughts in the preceding two months, while 2.1% attempted to take their own life in the same period. Some 11.3% of workers who did not report suicidal thoughts in the first survey reported them six months later, with 3.9% – about one in 25 – saying they had attempted to take their own life for the first time. Responses showed that a lack of confidence in raising safety concerns, feeling unsupported by managers, and having to provide a lower standard of care were among the factors contributing to staff distress.     The Right Honourable Sir Anthony Hooper was asked by the General Medical Council (GMC) on 5 September 2014 to conduct an independent review of how the GMC engage with individuals who regard themselves as whistleblowers. The terms of reference were: “To conduct a review of how the General Medical Council handles cases involving individuals who regard themselves as whistleblowers and who have appropriately raised concerns in the public interest. These are individuals: whose fitness to practise is being investigated or determined under the General Medical Council (Fitness to Practise) Rules 2004; or who have reported such a concern to the GMC.” This is the report by the Right Honourable Sir Anthony Hooper to the GMC presented on the 19th March 2015.

    A campaigning whistleblowing surgeon who wrote two books about his experiences has decided to leave the medical profession out of fear that he is being “hunted” by the NHS. Peter Duffy, a consultant urologist, is quitting work several years earlier than planned and intends to remove his name from the medical register. After a two year investigation the General Medical Council has decided to take no action against him. But he told The BMJ that he is worried that, after several investigations into his conduct, he remains vulnerable as long as he stays on the register. Duffy, 61, who blew the whistle on patient safety issues at University Hospitals of Morecambe Bay NHS Foundation Trust’s urology department, left the NHS nearly seven years ago. He claimed he was forced to resign from the trust for his own protection and won a claim for unfair constructive dismissal in 2018, when the trust was ordered to pay him £102 000 in compensation.     The number of NHS staff who feel able to raise concerns about clinical safety has fallen for the second year in a row, an analysis of NHS staff survey data has shown. A report by the NHS’s National Guardian’s Office, which represents local “freedom to speak up guardians” who help NHS workers to raise concerns, said that staff were increasingly disillusioned and that they believed that speaking up was “futile,” which had “worrying implications for patient safety.” Dr Jayne Chidgey-Clark, National Guardian for the NHS said: “It is not acceptable that two in five workers responding to the NHS staff survey do not feel able to speak up about anything which gets in the way of them doing their job. “These survey responses show us that there is a growing feeling that speaking up in the NHS is futile – that nothing changes as a result. When workers speak up about concerns, including the impact of under staffing and a crumbling infrastructure, their leaders themselves may struggle to be heard when trying to address these concerns. “I would add my voice to that of others that this urgently needs to be addressed". In this blog, healthcare entrepreneur Vanessa Webb makes the case that as a potential cause of harm to staff, regulatory inspections in public services including healthcare should be subject to Health and Safety Risk Assessments. There should be a systematic process to identify hazards, evaluate the likelihood and severity of harm, and determine appropriate controls to prevent or mitigate those risks.

    Two new healthcare workforce surveys outline widespread reports of discrimination, racism and workplace violence in the USA perpetuated by patients and coworkers alike. Among the findings were acknowledgments from respondents that incidents of discrimination are rarely reported to management or law enforcement. Additionally, more than half of the respondents to one survey said that they believed that incidents of workplace violence have increased over the course of their tenure, while nearly half of the nurses who responded to the other survey said they believe “a culture of racism/discrimination” was present as early as in nursing school. “If we are to truly provide just and equitable care to our patients, we as nurses must hold ourselves accountable for our own behavior and work to change the systems that perpetuate racism and other forms of discrimination,” said Beth Toner, RN, director of program communications at the Robert Wood Johnson Foundation (RWJF).     Paramedics are being told to take a police escort to more than 1,200 addresses for fear of attack, The Times has revealed. The College of Paramedics said the figure was outrageous and called on courts to implement tougher sentences for assaults on paramedics. Ambulance services have marked hundreds of addresses after violence towards crew. Notes on addresses include “patient keeps axe under pillow — serrated knife hidden round the house and is known to be a risk”, “shoots/throws acid”, and “patient is anti-ambulance”.     Children presenting with 'high-risk' behaviours are being cared for in NHS paediatric wards that may put them and others at risk of harm, according to a new report from the Healthcare Safety Investigation Branch (HSIB). HSIB's interim report warns that the placement of children and young people with complex mental health issues on NHS paediatric wards can impact on the wellbeing of these patients and their families, and pose a risk to other patients and staff. The report emphasises that paediatric wards are designed to care for patients who only have physical health needs and not for those who are exhibiting high-risk behaviours, which include attempts to die by suicide, self-harm, attempts to leave the hospital without permission, and episodes of violence and aggression. Examples of children and young people being restrained or sedated in front of other sick and vulnerable patients, families feeling concerned for their and their children's safety during incidents, rooms being stripped down to remove any risk of self-harm or death by suicide, and paediatric staff being physically assaulted are cited in the report. Saskia Fursland, HSIB national Investigator, said,"We know that NHS staff are trying to provide a safe environment for their patients, but they are facing difficult choices in wards that are not designed to support children and young people displaying high-risk behaviours. Our ongoing investigation will take a longer-term look at effective design, adaptations and risk management in the wards.     There has long been an acknowledgment by ministers and NHS leaders that violence against staff by patients was an issue that needed addressing, with a strategy to tackle it announced nearly five years ago. The health service’s 2019 long-term plan included a pilot for the use of body-worn cameras by paramedics in a bid to “de-escalate” situations. The following year the Crown Prosecution Service announced an agreement with the police and NHS England to “secure swift prosecutions” of those who assault staff, and the maximum penalty for assaulting emergency workers, including doctors and nurses, was also doubled to two years. Despite these measures, there have been internal disagreements within NHS England about the best approach to the problem, which affected almost 15% of staff last year, according to the latest national survey of the health service workforce. The Guardian understands that senior managers in NHS England told staff in its violence prevention and reduction (VPR) team last April that prosecutions of those who assaulted healthcare workers and dismissals of abusive staff should be a last resort. Instead, the focus should be on improving the culture of the NHS and staff wellbeing. It is also understood that managers cautioned against using the term “zero tolerance” because they said it did not take into account that some people who abuse NHS staff might lack capacity, an apparent reference to mentally ill patients. Julie is a registered nurse in the UK and became unwell with Covid in May 2020 while working on the frontline, during the first wave of the pandemic. She now lives with Long Covid and POTS (postural orthostatic tachycardia syndrome). In this podcast series, Julie shares her journey and lived experience, the symptoms and how each impacts daily life, not only the physical issues but also the impact this has had mentally and emotionally.

    This mixed-methods study in the Journal of Multidisciplinary Healthcare examined how health staff in Indonesian hospitals perceived open disclosure of patient safety incidents (PSIs). The authors surveyed 262 health workers and interviewed 12 health workers. In the quantitative phase they found a good level of open disclosure practice, a positive attitude toward open disclosure and good disclosure according to the level of harm. However, in the qualitative phase they found that most participants were confused about the difference between incident reporting and incident disclosure. The authors concluded that a robust open disclosure system in hospitals could address several issues such as lack of knowledge, lack of policy support, lack of training and lack of policy. They also suggest that the government should develop supportive policies at the national level and organise initiatives at the hospital level in order to limit the negative implications of disclosing situations.
