The management team of the Gender Identity Development Service (GIDS) in London has been 'disbanded', documents reveal. It comes weeks after the clinic, run by the Tavistock and Portman NHS Trust, was judged 'inadequate' by the Care Quality Commission. Watchdogs said staff were afraid to raise concerns about patient safety for fear of 'retribution' from bosses. A report said: 'Staff did not always manage risk well. New training for NHS maternity staff to boost babies safety

Hundreds of senior midwives are to be given new training to help improve culture and leadership across 126 NHS trusts. Patient safety minister Nadine Dorries said a new £500,000 maternity leadership programme would be rolled out later this year aimed at giving senior staff running maternity wards the skills and knowledge they need to boost culture and safety. Its one step towards improving the working relationships between midwives and obstetricians and follows the damning report by the Ockenden inquiry into decades of poor care at Shrewsbury and Telford Hospitals Trust. The report, published last month, highlighted leadership on maternity wards as a key factor in cases at the trust which led to preventable baby deaths and cases of neglect over many years. Announcing the fund, Nadine Dorries said: “The shocking and tragic findings of the Ockenden Review highlighted the importance of strengthening maternity leadership and oversight as well as fostering more collaborative approaches within maternity and neonatal services. “I’m pleased to announce a new training programme for NHS maternity leaders, which will empower nurses, midwives and obstetricians to get the best out of their teams, and deliver safe, world-class care to mothers and their babies.” Read full story Source: The Independent, 12 January 2021 Government press release -
News ArticleOne of the largest hospital trusts in England has been downgraded by the care watchdog amid safety fears and criticism that bosses did not act on staff concerns. The Care Quality Commission (CQC) said it found bedpans covered in faeces, urine and hair during 10 visits to wards at University Hospitals Birmingham Trust in June. Staff in A&E told inspectors they were put under pressure to nurse patients in corridors. At one stage 20 ambulances were queuing outside Heartlands Hospital with patients waiting outside. The CQC said staff felt “disconnected from leaders” who didn’t show an understanding of the pressures they were under. Consultants to the regulator staff were experiencing fatigue and they felt executives at the trust “were no longer interested in staff welfare”. In its inspection report, the CQC said staff did not always clean equipment and said labels for when items were last cleaned were being applied incorrectly. News ArticleThe Care Quality Commission’s (CQC) outgoing chief inspector of hospitals has called on integrated care system leaders to be ‘courageous’ in putting quality first. Speaking at the HSJ’s Patient Safety Congress, Ted Baker implored ICS leaders to not focus solely on financial and operational targets, although he also acknowledged “there is a lot of pressure to meet [those] targets”. In his speech yesterday, he said: “It’s often taken really courageous leaders to put quality first ahead of financial targets and operational targets… You have to be courageous to do that and I think some of the leaders of the ICSs, they need to be that courageous. “They need to focus on quality and safety within an [ICS] and not, if you like, go down the kind of NHS path of focusing on financial and operational targets. “If we can do that, we can have a really transformative effect on integrated care across [the] system. Service rated ‘good’ despite ‘weaknesses in culture’

A trust’s maternity services were rated ‘good’ despite an independent report finding ‘weaknesses in the culture’ and ‘defensive and fractious’ behaviours, HSJ has learned. As previously reported, former staff at Sandwell and West Birmingham Hospital Trust had raised concerns with the Care Quality Commission (CQC) over what they described as a “toxic management culture” and “unsafe” staffing levels in the trusts maternity service. Particular concerns were raised around community midwifery services. This prompted an unannounced inspection by the CQC in May, which found “low morale and negative culture” in the services. However, the CQC ultimately concluded the trust was taking positive steps to address the problems and rated its maternity services “good” overall, as well as for leadership and safety. Some frontline staff in the service have questioned those findings, however, and pointed to an independent review which was conducted in the early months of 2021. This review, carried out by independent consultant Debbie Graham and seen by HSJ, concluded there was “evidence of weaknesses in the culture; evidenced in the behaviours of some staff which appears to go unaddressed; a lack of strong, visible leadership; a lack of a shared vision; the finding that some staff have a fear of ‘speaking up’; and poor communication systems.” Read full story (paywalled) Source: HSJ, 20 September 2021 -
News ArticleA retiring chief executive was “astonished” how many junior doctors had never met the senior directors of their hospitals — and stressed how being visible on the wards is “critical” to good leadership. Karen Partington, who has this month stepped down after 10 years leading Lancashire Teaching Hospitals Foundation Trust, said she had made it her mission to understand the feelings and motivations of frontline staff. In an interview with HSJ, she was asked if being visible and spending significant time talking to frontline staff is the most important bit of advice she would give a first-time chief executive. She said: “In my personal opinion, it’s critical. How can CEOs be compassionate leaders without understanding the daily pressures faced by the whole team?" “My executive team and I [would] meet regularly with our junior doctors and do a ‘you said, we did’ session, which really helped us to change their experiences for the better. But it was also an opportunity to ensure our frontline colleagues understood the environment they were working in as well. CQC criticises ‘ineffective leadership’ at mental health provider

Appletree Hospital in Durham, a unit that provides female patients needing mental health care, has received an urgent enforcement notice from the Care Quality Commission. According to a report published by the CQC today, the hospital had “ineffective leadership”, also warning staff were carrying out “inappropriate” restraint on patients and that restraint was used “as a first line intervention”. The CQC report revealed staff had also claimed managers accepted the use of “unnecessary” restrictive practices. A spokeswoman for Cygnet’s Appletree unit said: “We move forward with strengthened local leadership, including a new hospital director and safeguarding and quality improvement leads.” Read full story (paywalled). News ArticleSerious patient safety concerns have been raised about a third major specialty at a struggling acute trust, with inspectors also flagging wider leadership issues. The Care Quality Commission (CQC) has issued an immediate warning notice in relation to the stroke service at University Hospitals of Morecambe Bay Foundation Trust, following an inspection earlier this month. A full report will be published later this year, but the immediate issues have been outlined within various documents published ahead of the trust’s board meeting on 26 May. According to a summary within the papers, the CQC warning notice has flagged “a range of incidents… identifying poor care that requires investigation”, governance concerns around the grading of incidents, poor levels of training and competencies, and worrying delays around administering thrombolysis. EventuntilHow are we ensuring that patient and staff safety is being prioritised during the pandemic? Presenters: Helen Hughes, Chief Executive, Patient Safety Learning, Dr Abdulelah Alhawsawi, Director General, Saudi Patient Safety Center, Professor Ted Baker, Chief Inspector of Hospitals, Care Quality Commission It can be argued that staff safety has not been prioritised as it should have been in recent years. The Covid-19 pandemic has reinforced this. It has exposed risks to staff physical and mental wellbeing, with inadequate Personal Protective Equipment (PPE), intensely difficult physical and psychological working conditions, and, tragically, it has resulted in deaths from exposure to Covid-19 as a result of inadequate infection control. The importance of overarching leadership for patient safety in health and social care is a key concept. Ensuring that leaders embed staff safety into safety programmes is essential. Some of the core issues that the panel will be exploring: How are leaders ensuring patient and staff safety during the pandemic? Are we doing enough? How are we prioritising patient access to non Covid care and treatment? Are we doing enough to encourage just and learning cultures and staff speaking up for safety? Content ArticleThe UK’s current strategy is outlined in its “Living with Covid-19” plan, which has three key pillars: vaccines, testing and treatment. In April 2022, the Tony Blair Institute for Global Change undertook a critical analysis of this plan: what worked well, what didn’t, and what needed to change to manage Covid more effectively, and to protect society and the economy. The recommendations were incremental steps amounting to a responsible and proactive strategy. Unfortunately, as we see from the recent rapid rise in cases – most involving people who are being infected for the first time – as well as from the increasing hospitalisations and the rise in deaths, the strategy shows what living with Covid means in reality. To support the government, this paper sets out immediate and practical measures to safely guide the country through the latest Covid wave and to outline the longer-term shifts that will better prepare it for public-health emergency management in the future – including by restoring trust with the public.
