    In this presentation Paula Goss, the founding member of Rectopexy Mesh Victims and Support, shares her experience of having a mesh implant. She describes the absence of informed consent during the procedure and the pain and complications she experienced following her surgery. This was shared at a Bristol Biomedical Research Centre workshop aimed at improving shared decision making for surgical innovation.

    In this article for The Guardian, Dr Kara Thompson, an obstetrician and gynaecologist working in the public hospital system in Geelong and Melbourne, Australia, argues that women must be given clear and unbiased information in order to make informed decisions about their birth preferences. She highlights the case of an information brochure about caesarean birth published on the website of a hospital in New South Wales, which presented incorrect claims about the relative risks presented by vaginal and caesarean birth. She outlines how the leaflet indicates that the way women are informed about birth choices is still subject to fear-mongering and shaming, and highlights the need for healthcare workers to respect maternal choice and autonomy.

    Before a patient undergoes a medical procedure such as surgery, they need to give their consent, having been informed about the details of the procedure and associated risks and benefits. However, in spite of the development of clear guidance on informed consent, many patients still feel nervous or unsure about asking questions and raising concerns during the consent process.     In this article for The Guardian, journalist Sirin Kale speaks to Janet Williams about the impact the epilepsy drug sodium valproate has had on her family. Janet took the medication to treat her epilepsy throughout her two pregnancies in 1989 and 1991, but had never been warned about the potential risks to her babies. Foetal valproate syndrome can cause spina bifida, congenital heart defects and developmental delays and is believed to have affected around 20,000 children in the UK. Both of Janet's sons were affected by the medication and require full time care as a result. Janet describes how being told about the risks would have enabled her to make an informed decision about whether to have children, and how her experience led her to help set up In-FACT (the Independent Fetal Anti Convulsant Trust) in 2012.

    In 2015, the ruling of the UK Supreme Court in Scotland in the case of Montgomery v Lanarkshire Health Board fundamentally changed the practice of consent. According to the Judges in this case, doctors are no longer the sole arbiter of determining what risks are material to their patients. They should not make assumptions about the information a patient might want or need but they must take reasonable steps to ensure that patients are aware of all risks that are material to them. The Royal College of Surgeons has developed guidance on consent that sets out the principles for working with patients through a process of supported decision-making, and a series of podcasts that illustrate those principles in practice.

    Patient choice has been a feature of the NHS in England for the last two decades, but patients' knowledge about what choices they have in accessing their NHS healthcare is variable. This report by the Patients Association and the Independent Healthcare Providers Network (IHPN) looks at the role patient choice can play in tackling the elective care backlog. The report found that: there are significant variations in waiting times across the country. patients do not need to travel long distances to access care more quickly. On average, a patient would need to travel just 13.2 miles to go from one of the worst performing providers to one of the top performers. the potential for reducing waiting times by accessing care through an alternative provider was backed up by both polling and focus group work, which found the public is enthusiastic about patient choice. people's awareness of their rights around where they receive their NHS treatment is low, suggesting the Government and NHS England need to do more to promote choice and make it easier for patients to understand the options available to them.

    On 19 October 2022, the long-awaited findings of Dr Bill Kirkup’s independent investigation into maternity services at East Kent were published. This blog outlines the response of the charity Birthrights to the investigation. It focuses on how breaches of mothers' human rights contributed to negative experiences of care and affected outcomes. Lack of informed consent, the use of disrespectful and discriminatory language and a failure to listen to mothers' concerns all contributed to many cases of avoidable harm. It argues that there is a desperate need for proper funding and real commitment to improving staff recruitment and retention, coupled with a culture shift in maternity care that embeds human rights at the centre of care.

    This article by Carrie Murphy looks at the practice of inserting a 'husband stich' or 'daddy stitch', where midwives or obstetricians make an unnecessary extra stitch when repairing episiotomies or tearing from birth. The belief is that it will make the vaginal opening tighter and therefore increase pleasure for the woman's sexual partner. The author highlights that this is a real practice that has been carried out on women for many years, and describes the ongoing impact it can have on women affected, many of whom don't realise they have been given too many stitches. This misogynistic and unethical practice can cause additional pain for women during sex. The women featured in this article state that they did not consent to the practice, being vulnerable after childbirth and in many cases unaware of what a 'husband stitch' was. Angela Sanford reports only finding out that she had a 'husband stitch' five years after birth at a cervical screening appointment where the nurse expressed concern. Murphy expresses her concern that the practice may still be carried out without women's consent, leaving them feeling violated and in pain.

    Sonia Sparkles is a senior manager in healthcare who is using her artistic skills to improve the way healthcare services communicate with patients. Her goal is to empower patients to feel at ease in healthcare settings and able to fully engage in their care. In this blog, Sonia describes how her own experience of being in hospital helped her see healthcare from a patient's perspective. While an inpatient, she felt disempowered, frightened and unable to ask the questions she wanted to. Having reviewed some NHS patient literature, Sonia realised that there was a need to find a way to communicate clearly with patients and invite them to share their concerns with healthcare staff. She produced a series of 23 posters as a starting point to get people thinking about how to communicate with patients in a simple, visual and empowering way.

    Hysteroscopy is a diagnostic gynaecological procedure traditionally requiring administration of general anaesthesia, but more frequently completed using local anaesthesia within a day-case (ambulatory) setting. Advantages associated with this transition include decreased completion times, fewer risks, and lower clinical costs. Numerous services advertise the procedure as being either pain free or low pain; however, it is estimated that 25% of patients report experiencing intense or intolerable pain. For severe pain, local anaesthetic can be administered, but this does not guarantee effective pain management. This research, published in the British Journal of Anaesthesia, found that very few patients feel no pain and a significant number felt pain of greater than 7/10. It also found a disconnect between the patient's experience of pain and the clinician's perception of it. This research paper is paywalled, but can be purchased via the link below.

    AHRQ's easy-to-understand telehealth consent form is part of AHRQ's Health Literacy Improvement Tools to help healthcare organisations, leaders and professionals improve health literacy. AHRQ's telehealth consent resources include a sample telehealth consent form that is easy to understand and how-to guidance for clinicians on obtaining informed consent for telehealth. The consent form includes provisions for healthcare providers that have curtailed in-person visits due to COVID-19. Clinicians can use the easy-to-understand language from the form when they are having the consent discussion and can use the form as a checklist to make sure they have covered all the information required by informed consent rules.
