Content ArticleThe Health and Social Care Select Committee is currently holding an Inquiry into Delivering Core NHS and Care Services during the Pandemic and Beyond. It’s stated aim is to ‘give focus to these upcoming strategic challenges, and give those working in the NHS and care sectors an opportunity to set out what help they will need from Government in meeting them’[1]. In its call for evidence the Inquiry has specifically identified ‘meeting the needs of rapidly discharged hospital patients with a higher level of complexity’ as one of the issues it will cover [2]. This is a joint submission (see attachment) to the Inquiry by Patient Safety Learning and CECOPS which is focused on this specific issue.

Content ArticleFor some time now I've been looking to find out more about mental health services in Trieste, Italy. Then I met Vincenzo Passante Spaccapietra, co-host of the Place of Safety? podcast series. This has enabled me to learn more about the closure of the mental institutions in Trieste, Italy, and the work of Franco Basaglia. I was keen to find out what really took place, what this really means in practice and how we can adopt this model in the UK. We were delighted to have become involved and to have recorded a couple of podcasts. I recommend this resource to everyone interested in safe, compassionate, patient led mental health care.

Content ArticleThis report is aimed at people who are working with those who have a learning disability, in the role of commissioners or providers of services. It was produced on behalf of the Hampshire Safeguarding Adults Board by a multi-agency group and seeks to understand why people with a learning disability are at greater risk of choking, looking at what can be done locally in Hampshire to improve outcomes for people who are at risk of choking, in any care setting. The report makes a number of recommendations based on common sense and good practice.

Content ArticleSTOMP stands for: stopping over medication of people with a learning disability, autism or both with psychotropic medicines. It is a national project involving many different organisations which are helping to stop the over use of these medicines. STOMP is about helping people to stay well and have a good quality of life. Psychotropic medicines can cause problems if people take them for too long. Or take too high a dose. Or take them for the wrong reason. This can cause side effects like: putting on weight feeling tired or ‘drugged up’ serious problems with physical health.

Content ArticleThe Academic Health Science Network’s (AHSN) plan 'Patient safety in partnership' has been developed to support the NHS Patient Safety Strategy and sets out how England’s 15 AHSNs, and the Patient Safety Collaboratives (PSCs) they host, will work more closely with their local health and care organisations to improve safety both in hospitals and community-based services such as care homes.

It has previously been referred to as 'Cardiac and vascular pathways', but the original investigation was split. This is part one of the investigation and part two, with a focus on the clinical diagnosis of aortic dissection, is due to be published in Spring 2019.

Content ArticleDifficulty in swallowing—known as dysphagia—is a serious problem for some adults with learning disabilities and, in serious instances, can lead to death. Improving the safety of people with dysphagia is essential, and introducing individual patient management guidelines can reduce the risks associated with this potentially life-threatening condition. This document from the NHS National Patient Safety Agency outlines the issues facing adults with learning disabilities who have dysphagia and introduces support materials that can provide practical help for these people. The tools can be adapted for local use and for any adult who has dysphagia.
