    The adoption of artificial intelligence (AI) by the NHS should be faster, and more frameworks should be in place to get emerging technologies to as many patients as possible, experts have told MPs. A number of senior figures from medicine and biotechnology gave evidence to the Health and Social Care Committee as part of its inquiry into cancer technology. Stephen Duffy, a professor of cancer screening at the Wolfson Institute of Population Health at Queen Mary University of London, told MPs there is “strong potential” for AI, particularly in areas such as reading mammograms for the breast screening programme. However, he warned that there will be “staff issues in terms of the number of staff needed to double-read mammograms”. He added: “Those issues aren’t going away. It seems to me that AI systems have already been shown to be very good in terms of detection of cancer on from mammograms, so they’re safe in that respect.     Using robots to assist in operations could make surgery more efficient and free up NHS beds, a report has suggested. The Royal College of Surgeons of England (RCS England) has published a guide: Robotic Assisted Surgery – a pathway to the future; exploring the potential benefits and challenges of the technology. It said the document “provides a structured pathway” for surgeons who want to transition to robotic-assisted surgery, which allows doctors to operate with more precision using interactive, mechanical arms. The report outlined “significant advantages” of using robots in surgery, including reduced post-op pain, fewer blood transfusions, more efficient use of anaesthetics and shorter hospital stays for patients. There are also benefits when it comes to patient safety, the college said, with platforms eliminating tremors and providing a magnified image of the surgical site.     Cheshire and Merseyside won Gold in the ‘Supporting Elective Recovery Through Digital’ category, at the HSJ Digital Awards, alongside technology partner C2-Ai, after transforming how waiting lists are managed with the help of an AI-backed waiting list model. The tool helps surgical teams identify previously hidden high-risk patients, and to make informed decisions on how, when and where to treat patients to achieve the best outcomes. NHS England, who commissioned the project, reported that within six weeks patient waiting lists dropped by nearly 30%, as well as a 66% decrease in intensive care needs for high-risk patients, saved about 2,500 hospital bed-days across 20,000 patients, and cut emergency admissions to the waiting list by 8%.     Smartwatches might help diagnose Parkinson's disease up to seven years ahead of symptoms, a study suggests. The UK Dementia Research Institute team at Cardiff University used artificial intelligence to analyse data from 103,712 smartwatch wearers. By tracking their speed of movement over a single week, between 2013 and 2016, they were able to predict which would go on to develop Parkinson's. It is hoped this could ultimately be used as a screening tool. But more studies, comparing these findings with other data gathered around the world, are needed to check how accurate it will be, the researchers say, in the journal Nature Medicine.     Adverse incidents arising from suboptimal healthcare are a major cause of worldwide morbidity and mortality. Arriving at an understanding of the conditions under which adverse incidents occur has the potential to improve the safety of healthcare provision. Staff working in the NHS have been contributing their experiences via a narrative data capture platform – SenseMaker – to help gain contextual insights on a wide range of topics under exploration by the NHS Horizons team. This blog by Rosanna Hunt (Senior Associate, NHS Horizons) in collaboration with Lizzy MacNamara (Junior Research Consultant, The Cynefin Co.) and Taj Nathan (Consultant Forensic Psychiatrist, Cheshire & the Wirral Partnership Foundation Trust) describes how the SenseMaker® platform could be used to extract staff experiences on the topic of patient safety incidents both reported and unreported by staff, and the facilitated conversations that would be needed to transform the data into actionable insights and commitment to change.

    This report assesses why NHS hospitals are failing to deliver higher activity despite higher spending on the service and higher levels of staffing over the last couple of years. It argues that politicians need to urgently focus on capital investment, staff retention and boosting management capacity, and sets out key questions for policy makers to address if they want to solve the NHS crisis.     A new type of artificial-intelligence technology that cuts the time cancer patients must wait before starting radiotherapy is to be offered at cost price to all NHS trusts in England. It helps doctors calculate where to direct the therapeutic radiation beams, to kill cancerous cells while sparing as many healthy ones as possible. Researchers at Addenbrooke's Hospital trained the AI program with Microsoft. For each patient, doctors typically spend between 25 minutes and two hours working through about 100 scan cross-sections, carefully "contouring" or outlining bones and organs. But the AI program works two and a half times quicker, the researchers say. When treating the prostate gland, for example, medics want to avoid damage to the nearby bladder or rectum, which could leave patients with lifelong continence issues.     Artificial intelligence (AI) is set to be rolled out more widely across the NHS in a bid to diagnose diseases and treat patients faster. The Government has announced a £21 million funding pot that NHS trusts can apply for to implement AI tools for the likes of medical imaging and decision support. This includes tools that analyse chest X-rays in suspected cases of lung cancer. AI technology that can diagnose strokes will also be available to all stroke networks by the end of 2023 – up from 86% – and could help patients get treated faster and lead to better health outcomes.     A soft robotic device that moves through the body like a worm could make bowel cancer screening much more comfortable by reducing pain and and discomfort, according to the scientists developing it. Researchers at Imperial College London said they have created a new type endoscope that is soft, flexible, and capable of extending and curling on its own inside the body. Around 900,000 colonoscopies – which involves an endoscope being passed through the bottom – are performed every year in the UK, the majority of which are for screenings for bowel cancer. The procedure is invasive and more than 75% of patients develop significant pain, according to Nisha Patel, a consultant gastroenterologist at Imperial College Healthcare NHS Trust in London, who is running clinical trials with the device. She said: “We know patients experience either discomfort or pain during the procedure and this affects uptake of further procedures and patient experience. The experts are also hoping that compared to current endoscopes, which requires specialist skills, their self-propelling robotic version will be easier to use and could, in future, be deployed in GP surgeries or outpatient clinics.     The widespread adoption of effective hybrid closed loop systems would benefit people living with type 1 diabetes by improving the amount of time spent within target blood glucose range. Hybrid closed loop systems (also known as 'artificial pancreas' typically utilise simple control algorithms to select the best insulin dose for maintaining blood glucose levels within a healthy range. Online reinforcement learning has been utilised as a method for further enhancing glucose control in these devices. Previous approaches have been shown to reduce patient risk and improve time spent in the target range when compared to classical control algorithms, but are prone to instability in the learning process, often resulting in the selection of unsafe actions. This study in the Journal of Biomedical Informatics presents an evaluation of offline reinforcement learning for developing effective dosing policies without the need for potentially dangerous patient interaction during training.

But how do we improve access and uptake of these medicines in a way that is affordable, and that supports the already overstretched health and care workforce. This free online event from the King's Fund is an opportunity to consider some of the key challenges to access and uptake of innovative medicines in England. She is also Clinical Director for Frailty in East Kent and runs a large frailty virtual ward. Dr Crystal Oldman CBE, Chief Executive, The Queen's Nursing Institute. Crystal qualified as a nurse at University College Hospital, London. In 2017, Crystal was awarded a CBE in the Queen’s Birthday Honours List for services to community nursing and her leadership of the QNI. Crystal is an Honorary Professor at London South Bank University.     Pioneering new technology could help patients with non-healing wounds avoid infections and the need for antibiotics, scientists say. Wirelessly powered, environmentally friendly “smart bandages” have been developed by a team of scientists from the UK and France, with the University of Glasgow and the University of Southampton leading the research. The bandage could help improve the quality of life for people with chronic non-healing wounds as a result of conditions such as cancer, diabetes or damaged blood vessels, they said. Currently, wounds require painful cleaning and treatment. Researchers believe the technology could help to slow the rise of dangerous new strains of antibiotic resistant bacteria known as superbugs.     The Community Health and Wellbeing Worker (CHWW) model was devised in Brazil in the 1990s, where it is called the Family Health Strategy. There are over 250,000 CHWWs in Brazil, described as ‘the ears and eyes of the GP in the community’. They are full time members of the local primary care team and focus on a defined location, usually 200 households, keeping in regular contact with the residents. By visiting households at least once a month, the delivery of primary care becomes truly local and embedded into everyday life. This article describes a pilot of a CHWW model by the NIHR Applied Research Collaboration Northwest London. It discusses how the project was established and includes case studies from the pilot.

    People with diabetes account for one in three hospital inpatients, and this is projected to increase to one in five in the next few years. Often, people are in hospital for reasons other than their diabetes, so it is important that staff across all specialties understand the basics of diabetes care in order to ensure patient safety. D1abasics is an innovative project that aims to equip all healthcare professionals to support the basic diabetes healthcare needs of their patients. Developed by the diabetes team at University Hospital Southampton with funding and support from the charity Diabetes UK, the campaign includes resources such as posters, lanyards and prompt cards.     Investors are pouring billions into companies claiming they can analyse DNA to find the disease early. But some scientists question if they really work. A pioneering group of people in the US and UK who have elected to take part in a new form of cancer screening known as multi-cancer early detection tests (MCED). The tests use gene sequencing or other novel technologies to detect fragments of DNA expelled by cancerous cells which circulate in people’s blood, allowing the identification of multiple types of cancer from a single blood draw. They have been hailed as “revolutionary” and “cutting edge” by British and US health chiefs. Health bodies in both nations have set up MCED clinical trials in the hope that the tests can be rolled out to the population at large. The UK’s NHS is participating in a clinical trial of the Galleri test involving 140,000 patients. But not everyone is convinced the tests live up to the hype. Several health experts and scientists told the Financial Times that the tests could harm rather than help some patients due to risks associated with misdiagnosis, over-diagnosis and over-treatment.
