Content ArticleMartha Mills died from sepsis aged 13 after sustaining a pancreatic injury from a bike accident. The inquest into her death heard that she would likely have survived had consultants made a decision to move her to intensive care sooner. Her mother, Merope, has spoken about the failures in Martha’s care, and how she trusted the clinicians against her own instincts – they didn’t listen to her concerns and instead “managed” her. This report is a response to that call from Martha Mills’ parents to rebalance the power between patients and medics with one purpose only: to improve patient safety. It comes amidst significant evidence that shows that failing to properly listen to patients and their families contributes to safety problems in the NHS.

Content ArticleCo-production is a way of working that involves those who use health and care services, carers and communities, in equal partnership. It engages groups of people at the earliest stages of service design, development and evaluation. Co-production acknowledges that people with lived experience of a particular condition are often best placed to advise on what support and services will make a positive difference to their lives. In this blog post, Helen Lee from NHS England's Experience of Care Co-production Programme talks about work her team has been doing to put co-production at the centre of quality improvement, including the launch of a new suite of materials. These resources aim to encourage professionals and leaders to expect to hear a range of experiences and be curious and open minded to these views.

Generally, this philosophical term describes when a person is wrongfully discredited as a knower; and within the clinical space, epistemic injustice is the underlying reason that some patient testimonies are valued above others. The following essay, published in Philosophy, Ethics, and Humanities in Medicine, seeks to connect patterns of social prejudice to the clinical realm in the United States: illustrating how factors such as race, gender identity, and socioeconomic status influence epistemic credence and associatively, the quality of healthcare a person receives.

Content ArticlePrimary care services are the front door to the NHS - they are the first port of call when we feel unwell and the main coordinator of care when we are living with health conditions. The primary care team have an important role in making people feel welcomed, listened to and taken seriously. Yet we often hear examples about people who have not had their communication needs met within primary care. This includes people with sensory impairments, people with learning disabilities, autistic people, people living with dementia, people who don’t speak English fluently, people with low or no literacy, people who are digitally excluded, people living nomadically, people experiencing homelessness and many others. This report sets out the key issues faced by people with specific communication needs within primary care and what they feel would make the biggest difference, as well as key actions primary care leaders and teams can take to support inclusive communication.

But how might people use technology to have more control over their health and wellbeing? And do they want to? This report explores the role of technology in managing, improving and supporting health and wellbeing. The NHS Confederation, in partnership with Google Health, commissioned Ipsos to explore people’s behaviours, attitudes and beliefs about responsibility and control when it comes to their health, the role that health technologies play in this and their expectations about the future of healthcare. A survey of more than 1,000 adults in the UK – a third of whom live with one or more long-term conditions (LTCs) – and interviews with individuals with LTCs and who have frequent interaction with the health system, forms the centrepiece of this report.

Content ArticleIn healthcare, telling stories brings benefits to both storytellers and audience members, but also presents risks of harm. A reflective storytelling practice aims to honour stories and storytellers by ensuring there is time to prepare, reflect, learn, ask questions, and engage in dialogue with the storyteller to explore what went well and where there are learning and improvement opportunities. Healthcare Excellence Canada (HEC) is a pan-Canadian health organisation focused on improving the quality and safety of care in Canada. The HEC Patient Engagement and Partnerships team have co-developed these recommendations on how best to meaningfully share stories from those leading, providing and receiving care at Board meetings. This Case Study outlines the process HEC used to co-develop storytelling recommendations, focusing on a trauma-informed approach to create safe spaces for preparing, learning from and reflecting on stories, to clearly articulate their purpose, and to ensure the locus of control for storytelling rests with the storytellers.

Content ArticleCare home residents are particularly vulnerable to patient safety incidents, due to higher likelihood of frailty, multimorbidity and cognitive decline. However, despite residents and their carers wanting to be involved in safety initiatives, there are few mechanisms for them to contribute and make meaningful safety improvements to practice. This study aimed to develop a measure of contributory factors to safety incidents in care homes to be completed by residents and/or their unpaid carers.

Content ArticleThe Primary Care Patient Measure of Safety (PC PMOS) is designed to capture patient feedback about the contributing factors to patient safety incidents in primary care. It required further reliability and validity testing to produce a robust tool intended to improve safety in practice. This study led to a reliable and valid 28-item PC PMOS that could enhance or complement current data collection methods used in primary care to identify and prevent error.

Content ArticleShared decision making involves ensuring patients are able to contribute meaningfully to decisions about their care. Healthcare professionals ensure patients are informed of the options available to them and fully involve patients in making treatment decisions. This report by the Patients Association sets out the views of 1,416 healthcare professionals on shared decision making, expressed in an online survey in Spring 2022. Respondents included GPs, consultants, specialist nurses and practice nurses. The survey found that most healthcare professionals are positive about shared decision making, but feel their ability to practice it regularly is limited by the current situation in the NHS. Many said that lack of time, gaps in the workforce and large caseloads prevented them partnering with patients to make decisions about treatment and care together.

Content ArticleFinding out a patient's perspective about their care and treatment is a key part of shared decision making, but healthcare professionals do not always proactively do this in practice. This scoping review in the journal Patient Education and Counselling explored the extent to which the personal perspectives of patients are drawn out by clinicians during a consultation, as part of a shared decision making process. It reviewed studies in five databases about shared decision making. The authors found that studies reported low levels of healthcare professionals eliciting patient perspectives, The majority of content healthcare professionals and patients discussed related to physical health, with social and psychological topics mostly unaddressed.
