    A new study has found night shifts are "significantly associated" with health issues related to the heart, particularly atrial fibrillation, finding that women may be at a greater risk. The research, published in the European Heart Journal also found working night shifts is linked to an increased risk of coronary heart disease (CHD). “Night shift exposure also increased the risk of CHD (coronary heart disease) but not stroke or HF (heart failure). Whether decreasing night shift work frequency and duration might represent another avenue to improve heart health during working life and beyond warrants further study,” the paper said.     The Royal College of Midwives is calling for "common sense" from NHS trusts and boards on staff access to water and other drinks. The college is concerned that the health and wellbeing of midwives could be in jeopardy as a result of having limited opportunities to stay hydrated on long, hot shifts. .In new guidance to its members, the RCM sets out the importance of staying hydrated on shift and the potential implications of not doing so. These included an impact on decision making, memory, attention span, mood and tiredness. The document also debunks myths suggesting that having fluid bottles on units is a cross infection risk.

    Nursing and maternity staff have reported that they are unable to have water with them whilst working, meaning some are unable to drink water for 12 hours due to work pressures. The Royal College of Midwives in updated guidance are calling for "common sense" from NHS trusts and boards on staff access to water and other drinks, particularly as staff are having to wear more PPE than before the pandemic began. Dr Mary Ross-Davie, director for professional midwifery at the RCM, said: “Some trusts and boards have banned water bottles in clinical areas, which means that our members are often going 12 hours with no break and no water. We are appealing to those services to apply common sense, to recognise that this application of infection control is misguided and to look after the health and wellbeing of their staff.” Read full story (paywalled).     Patient safety incidents can have significant effects on both patients and health professionals, including emotional distress and depression. This, published in British Journal of Surgery (BJS) Open, study explores the personal and professional impacts of surgical incidents on operating theatre staff. This study, published in BJS Open, involved 45 face-to-face interviews, with participants including surgeons, anaesthetists, scrub nurses, ODPs and healthcare assistants. The authors state that the results indicate that more support is needed for operating theatre staff involved in surgical incidents. They also suggest that there needs to be greater transparency and better information during the investigation of such incidents for staff.

    This is the National Guardian's Office annual data report covering the 1 April 2020 to 31 March 2021. It analyses the themes and learning from the speaking up data shared by Freedom to Speak Up Guardians across this period. There are over 700 Freedom to Speak Up Guardians in the NHS and there were 20,388 cases raised with them in 2020/21.

    In November last year, Unison and RCN raised concerns with NHS Forth Valley chief executive Cathie Cowan amid bullying claims made by emergency department staff at Forth Valley Royal Hospital. In light of the reports of bullying, a review was commissioned and leaked details revealed junior doctors were left unsupervised on shifts, nurses and doctors had said they are 'battle-weary', and many senior staff members have resigned their posts. Karren Morrison, Unison Forth Valley health branch secretary has said: ‘Last year, concerns were being raised by our members who worked in, or who had previously worked in the ED. Staff talked to us about feeling bullied and intimidated, being frightened to speak up, concerns about the delivery of safe patient care, high staff turnover and other issues.’ Read full story (paywalled).     This systematic review, published in the International Journal of Environmental Research and Public Health, looks at different support resources in healthcare organisation that are available to healthcare professionals who have been involved in a patient safety incident. The authors identify a range of challenges to the implementation of these, including persistent blame culture, limited awareness of program availability, and lack of financial resources.

    A new survey by the BMA was conducted throughout the UK on moral distress from 18 March 2021 to 12 April 2021. It was found that those doctors who worked with Covid-19 patients and doctors who were from ethnic backgrounds, were more likely to say moral distress was a feeling they had experienced. It was also found doctors from ethnic backgrounds were also more likely to say moral injury resonated with their experiences. The results of the survey has raised concerns for staff wellbeing with the BMA asking that the government increase staffing to the NHS, and provide emotional and psychological support for healthcare workers.     What will the new world of work look like in the NHS post-pandemic is the crucial question being examined in an HSJ webinar. The webinar will bring together a panel of experts to discuss what skills NHS staff will need as the world of work changes, with the likelihood that some work that has been done face-to-face will be virtual.     Despite many investigations and inquiries into violent incidents in mental health settings, resulting in reports and guidelines, safety considerations for mental health staff continue to be an issue of serious concern. This report from the Royal College of Psychiatrists attempts to address the safety needs of psychiatrists. Psychiatrists from different disciplines are likely to be exposed to different degrees of threat to their safety, but no particular branch of psychiatry is immune. In the same vein, the context in which psychiatric practice is delivered will also have a bearing on the likelihood of violence, be it on in-patient units, in out-patient settings, in accident and emergency departments, in prisons and other custodial institutions, or in patients’ homes.

    Doctors in Wales have faced bullying and disciplinary action for raising concerns over working conditions and safety, a union leader has said. Dr Phil Banfield, of BMA Wales, said doctors who complained about work, both before and during the Covid pandemic, were seen as "troublemakers". He said there are worries bullying among staff will get worse as longer post-Covid waiting lists are tackled. The Welsh government said bullying of NHS staff was "entirely unacceptable". Dr Banfield, who is chairman of the BMA Welsh consultants' committee, said staff have faced the prospect of being victimised by colleagues, or even being forced to leave the Welsh NHS, for raising concerns over bullying or health and safety. He said: "Staff are quite good at raising concerns, but they don't raise concerns if they're going get in trouble for it, or they sense nothing is going to happen. What happens is you think 'I can't be bothered'. "Decent people develop a kind of learned helplessness and it means that people who keep raising concerns stand out."     When we put people on a pedestal, my experience is that they are less likely to be asked, ‘are you OK?’, writes Samantha Batt-Rawden, a senior registrar in intensive care medicine. Like many she has been touched by the groundswell of support from the public. But there’s a problem with this hero image, she says. "It’s not just that many NHS staff are feeling increasingly uncomfortable with being hailed as heroes for what they see as simply doing their jobs. Of course, we were going to step up to the plate when the COVID-19 pandemic hit. As doctors it was our duty. There was never any question. "But there’s something more than just feeling undeserving of the cape weighing heavily on our shoulders. The worst thing about being seen as a superhero? Very few think to ask if you’re OK.
