News ArticlePregnancy support helplines are experiencing a massive spike in distressed pregnant women asking for urgent help as charities warn coronavirus upheaval is placing pregnant women at risk. Frontline service providers warn mothers-to-be are anxious about whether they will be denied pain relief options and be separated from their newborn babies due to them being put in neonatal units. Birthrights, a maternity care charity, found enquiries to its advice line in March were up by 464 per cent in comparison to March last year. Women getting in touch also raised concerns about home birth services being withdrawn, midwifery-led birth centres shutting their doors and elective caesareans being discontinued due to the COVID-19 crisis. Baby charity Tommy’s experienced a 71% surge in demand for advice from midwives on its pregnancy helpline last month. The organisation warned coronavirus turmoil is placing pregnant women at risk after their midwives answered 514 urgent calls for help in April which is a sizeable rise from the 300 enquiries they would generally get. Jane Brewin, the charity’s chief executive, said: “Antenatal care is vital for the wellbeing of mother and baby – but the coronavirus outbreak means that many don’t know who they can ask for help, or don’t want to bother our busy and beloved NHS." “Although services are adapting, they are still running, so pregnant women should not hesitate to raise concerns with their midwife and go to appointments when invited. Continuity of midwife carer (CMC), a key component of good quality midwifery care, results in better clinical outcomes, higher care satisfaction and enhanced caregiver experience. However, CMC uptake has tended to be small scale or transient. McInnes et al. used realist evaluation in one Scottish health board to explore implementation of CMC as part of the Scottish Government 2017 maternity plan.

High-risk pregnancies could be missed due to pandemic, experts warn

Experts have raised fears that high-risk pregnancies may be missed due to the coronavirus pandemic, leading to a potential rise in stillbirths and neonatal deaths. During a session of Westminster’s Health and Social Care Committee, Gill Walton, the Chief Executive of the Royal College of Midwives, said there was a “fear” among pregnant women presenting themselves to maternity services during the COVID-19 outbreak. Former health secretary Jeremy Hunt, who chairs the committee, said one of the most important elements of maternity safety was to identify higher-risk pregnancies early “so that interventions can be made to prevent stillbirths, complications, or even the death of a baby”. Mr Hunt added the President of the Royal College of Obstetricians, Dr Edward Morris, had told him he is “worried that some higher-risk pregnancies may be being missed” because of fewer face-to-face appointments and missed scans. Asked whether she shared that concern, Ms Walton told MPs: “I do share that concern. Some of that is related to the fear of the pregnant population and presenting to maternity services during the pandemic." In this podcast from General Broadcast, Clinical Lead Daimon Wheddon and the Norfolk and Norwich University Hospital's Dr Caroline Barry, sit down to talk about ReSPECT. The ReSPECT process provides a written record of patients' treatment preference and focuses on treatments to be considered as well as those that are not wanted or would not work. People with a complete ReSPECT form do not require a separate DNACPR form although it is important to be aware that a patient can have a ReSPECT form and still be in favour of cardiopulmonary resuscitation. ReSPECT aims to inform decision making for ambulance clinicians when the patient may lose capacity to make decisions for themselves. -

Millions of patients 'avoiding calls to GP' during COVID-19 pandemic

Four in ten people are not seeking help from their GP because they are afraid to be a burden on the NHS during the pandemic, polling by NHS England reveals. The findings – from a survey of 1,000 people – are the latest in a wave of evidence that fewer people are seeking care for illnesses other than those related to coronavirus during the pandemic. GP online reported on 20 April that data collected by the RCGP showed a 25% reduction in routine clinical activity in general practice, and figures from Public Health England (PHE) and the British Heart Foundation show that A&E attendances overall and patients going to hospital for heart attacks are down 50%. Warnings that patients' reluctance to come forward could put them at risk come as leading charities warned that suspension of some routine GP services during the pandemic could also lead to a 'future crisis' if control of conditions such as asthma and COPD deteriorate. Professor Carrie MacEwen, chair of the Academy of Medical Royal Colleges, said: 'We are very concerned that patients may not be accessing the NHS for care because they either don’t want to be a burden or because they are fearful about catching the virus. 'Everyone should know that the NHS is still open for business and it is vitally important that if people have serious conditions or concerns they seek help. UK doctor invents digital flashcards to help COVID-19 patients understand staff

A coronavirus patient’s terrifying hospital experience inspired an NHS doctor to create a flashcard system to improve communication with medical staff wearing face masks. Anaesthetist Rachael Grimaldi founded CARDMEDIC while on maternity leave after reading about a COVID-19 patient who was unable to understand healthcare workers through their personal protective equipment (PPE). Her system enables medical staff to ask critically ill or deaf coronavirus patients important questions and share vital information on digital flashcards displayed on a phone, tablet or computer. The idea went from concept to launch on 1 April in just 72 hours and is now being used by NHS trusts and hospitals in 50 countries across the world. Having read an article about a critically ill intensive care patient terrified when they couldn’t understand what the healthcare team were saying through their personal protective equipment (PPE), Rachael Grimaldi, an NHS anaesthetist, was inspired to create a simple communication tool: CARDMEDICTM. Update from Rachael: Since writing this blog, we have been accepted onto The Hill Accelerator Program, run by Oxford University, Oxford University Hospitals NHS Foundation Trust and Oxford Brookes University. We have also been selected to be a part of the Healthcare UK Digital Health Offer for Export for 2020. Healthcare UK is a joint initiative of The Department for International Trade (DIT), The Department of Health and Social Care (DHSC) and NHS England, which promotes the UK healthcare sector to overseas markets. We now have over 30,000 users in over 100 countries, alongside 11,500 app downloads. Version 2 of the app is out now, with a 'free notes' section so the patient and healthcare staff can communicate directly. Text can either be dictated or typed into the notes box, the language auto-detected and then translated into one of 10 languages available and read aloud to the patient if necessary. We are working on enhancing accessibility through the addition of sign language videos and images / illustrations / makaton signs. An independent academic evaluation by University of Brighton using simulated patients demonstrated 25% increased confidence in understanding a healthcare worker in PPE with CARDMEDIC than without. Overall, confidence improved by 28% to 95%. Results likely significantly higher for those unwell/communication needs.- Posted

Content ArticleThe medical symptom 'iceberg' and 'trivia' were defined in terms of people's own perceptions of their symptoms and their subsequent referral behaviour. The data were collected by household interviews of patients registered at a health centre and included information on personal and environmental characteristics. Bivariate and multivariate analysis was used to explore associations between those who were part of the symptom 'iceberg' or 'trivia', and factors which might have caused such incongruous referral behaviour.

News ArticleOnce COVID-19 seeps into care homes, it is a monumentally difficult job to protect the residents, writes Sky's Alex Crawford. We will look back at this appalling, tragic episode in our global history, and our children and grandchildren will ask us: "Did that really happen? Did you really leave the most vulnerable of our society - the elderly, the infirm, the defenceless, the muddled, sick and weak - in care homes, shut away from their closest relatives? Did you leave them to be ravaged by a deadly virus, and do very little to help them?" Because that is what's happening right now. There are elderly people - many with Alzheimer's, many with dementia, many frail - in thousands of residential homes up and down Britain, and they are very much at risk. Content ArticleCARDMEDIC was inspired by a news article on a patient surviving COVID-19 after an admission to a UK Intensive Care Unit (ICU), where he described feeling terrified of not being able to understand what his healthcare providers were saying, due to the limitations of communicating through Personal Protective Equipment (face masks, visors, hoods etc). CARDMEDIC is a collection of communication flashcards designed to break through the Personal Protective Equipment (PPE) barrier, improving transfer of vital information from frontline healthcare professionals to unwell and critically ill patients. They are also used as an upskilling training tool, for example by healthcare professionals and volunteers working outside their usual realms of practice. They are simple and succinct, using basic language to share information and describe the plan of action. Set out in alphabetical order, it should be easy to find what you’re looking for. You may wish to use the “HELLO MY NAME IS…” card at the start of every patient interaction to introduce yourself and set the scene for using the flashcards. The flashcards can be used in electronic format on either the patient’s or hospital’s phone / tablet / smart device, or printed, laminated, annotated and re-used – write on, wipe off. CARDMEDIC are continually expanding their database, so please get in touch with suggestions or comments, as well as ideas for further flashcards.
