    With the latest UK government figures showing that there have been nearly 150,000 deaths where COVID-19 was mentioned on the death certificate, it’s understandable why some people compare the pandemic with a war. Indeed, daily life in the NHS is now peppered with military language: the frontline, gold command calls, redeployment, buddy systems and 'moral injury' Moral injury can be defined as the distress that arises in response to actions or inactions that violate our moral code, our set of individual beliefs about what is right or wrong. In the medical literature, moral injury has historically been associated with the mental health needs of military personnel, arising from their traumatic experiences during active service. Moral injury is generally thought to arise in high-stakes situations so it’s no surprise that the term has gained traction in healthcare settings over the course of the pandemic, given that healthcare staff have been faced with extreme and sustained pressure at work. In many ways, working in the NHS over the past year has felt like being some sort of circus acrobat, contorting ourselves to balance various competing realities: the desire to provide high-quality care for all our patients in the context of limited resources, looking after our own health needs alongside those of our patients, trying to make peace with the responsibility we feel towards our loved ones while still upholding our duty of care to patients. If we fail to deliver, particularly in high-stakes situations where we think things should have been done differently, it can shake us to our core. Our moral code transcends the relatively superficial responsibilities of our professional role: it gets to the heart of who we are as human beings.     A witness to an inquiry into deaths at England’s largest mental health trust has been intimidated by “cruel and calculated pressure”, with messages described by the man leading the investigation as “truly shocking”. In a statement at the start of hearings into the quality of care at Southern Health Foundation Trust, inquiry chairman Nigel Pascoe QC said one witness had received threatening telephone calls, messages and emails, which he said were “totally unacceptable, damaging and deeply disturbing”. Mr Pascoe said the inquiry had been told Beth Ford, whose job title at the trust is service user involvement facilitator, had been intimidated by members of the public. Ms Ford, who has autism, was admitted to hospital for her mental health earlier this month as a result of the abuse, but has now returned home. It’s the latest incident to hit the controversial inquiry, which has itself faced fierce criticism from the families of five patients who died between 2011 and 2015. The families have pulled out of the inquiry and accused the investigation and NHS England of bullying them and going back on promises to properly investigate the deaths of their relatives. Maureen Rickman, whose sister Jo Deering died in 2011, told The Independent she didn’t believe any of the main families were involved in intimidating witnesses.     Many doctors from black, Asian and minority ethnic backgrounds say key risk assessments have still not taken place, or have not been acted on. About 40% of UK doctors in the UK are from BAME backgrounds, yet 95% of the medics who have died from coronavirus were from minority backgrounds. The NHS said last June that its trusts should offer risk assessments to staff, but hundreds told a poll for BBC News that they were still awaiting assessments or action. Of 2,000 doctors who responded, 328 said their risks hadn't been assessed at all, while 519 said they had had a risk assessment but no action had been taken. Another 658 said some action had been taken, with just 383 reporting their risks had been considered in detail and action put into place to mitigate them. One of those who responded was Dr Temi Olonisakin, a junior doctor in London who has Type 1 diabetes. She had her risk assessment early on in the pandemic. "It was as comprehensive as a side A4 paper can be," she says. "I think for a lot of people it felt more like a tick-box exercise, and one that could be used to say: 'We've done what we need to do to make people feel safe' - but I'm not sure in reality that's how people felt."     Accident and emergency staff at Bristol Royal Infirmary do not feel equipped to deal with the violence they face from the public, inspectors have found. Health watchdog The Care Quality Commission (CQC) rated the department as "requires improvement" following an inspection in February. It said "urgent action" must be taken to protect staff and patients. University Hospitals Bristol and Weston NHS Foundation Trust said it was working on improvements. Amanda Williams, CQC's head of hospital inspections, said: "We were... particularly concerned to find high levels of violence and aggression against staff from patients in the department and to learn that staff did not feel adequately trained to deal with this." "Staff need to be given the appropriate training and support to ensure they feel safe and to enable them to defuse tension and prevent violence from escalating."     Nurse leaders are lobbying government to update "fundamentally flawed" guidance on personal protective equipment. The Royal College of Nursing says the existing recommendations are based on out-of-date evidence. One nurse told the BBC she had not been allowed to wear a higher-grade mask, despite having to go into the homes of patients with Covid. The nurse, who wishes to remain anonymous, wanted to use what's known as an FFP3 mask that filters out infectious aerosols. But she says her employers insisted on following national guidance, that most health staff should wear thinner surgical masks, instead. This comes at a time of mounting concern among many healthcare organisations that personal protective equipment (PPE) is inadequate. A new report for the Royal College of Nursing (RCN) criticises the official guidelines for PPE as "fundamentally flawed" because they're based on out-of-date evidence. It says that the risk of infection by aerosols is not given enough emphasis and that key research papers highlighting the possibility of airborne transmission have not been considered. The RCN's report concludes that key research papers on aerosols appear to have been ignored and that the terms used to search for new papers were likely to be "biased against" those on airborne transmission. The lead author of the study, Prof Dinah Gould, says she is "very disappointed" at the review for not taking into account the latest science. "A year into the pandemic, the review needs replacing.     Paramedics in London have started wearing body cameras after a 34% jump in the number of violent attacks on ambulance crews. A trial of the technology is being rolled out across the capital in areas where workers are thought to be more at risk based on past incidents. Paramedics can press a button to start recording if patients or the public become aggressive or abusive towards them. London Ambulance Service told The Independent there had been an increase in physical assaults in recent years. Attacks jumped from 468 in the financial year 2018-19 to 625 in the year 2019-20, a 34% rise. Gary Watson, based at Croydon Ambulance Station, will be one of the first staff members to wear a camera. He was violently assaulted by a drunk patient three years ago. He said: “We need these cameras. We get up every day to help people, not to be severely beaten. “Wearing these cameras should act as a deterrent and if it doesn’t then at least there will be evidence which will hopefully mean tougher sentences for criminals.” Read full story Source: The Independent, 23 February 2021

    The NHS has been warned not to use or distribute a batch of high-grade face masks after government officials warned they may not meet safety standards. The Department of Health and Social Care (DHSC) said it had “received urgent information” suggesting the items did not meet UK government specifications. Hospital bosses and others including GPs have been told to stop using the FT-045A FFP3 masks, quarantine any remaining supplies and then track down any they might have sent on to other health providers through mutual aid schemes. DHSC said it was pausing further distribution while a rapid review was carried out. Some 1.12 million masks are affected, the department said. DHSC’s warning document told health bosses: “Please be assured that we are working with regional teams and providers to fit-test staff to alternative models and provide of them with alternative masks in the quantities needed. We have a good supply of other FFP3 masks.” Rob Harwood, consultants committee chairman of the British Medical Association, told the BBC: “Healthcare staff working on the frontline do so facing the grave risk of contracting Covid, and deserve the best protection from this deadly virus." “Many will be incredibly concerned to learn that some masks designed to offer enhanced protection, as well as gloves, may not meet standards, potentially placing them at greater risk.” Read full story Source: The Independent, 24 February 2021

    Nearly 20 major healthcare bodies are appealing to the Prime Minister for better personal protection against coronavirus. They say at least 930 health and care workers have died of COVID-19 and more are experiencing long-term effects. In a letter, they say measures to stop airborne spreading are "inadequate" and call for urgent improvement in masks and other defences against variants. The government said it was monitoring evidence on airborne transmission and would update advice "where necessary". The organisations involved represent a wide range of health professionals, from doctors and nurses to dieticians and physiotherapists. Their approach to Downing Street follows repeated efforts to raise the issue with others in government. With health and care workers at three to four times greater risk of becoming infected than the general public, the plea to Boris Johnson is to make an "urgent intervention to prevent further loss of life". It says current policies focus on contaminated surfaces and droplets - for which the best defences are hand hygiene and social distancing - but not on airborne transmission by tiny infectious aerosols.     One in five female doctors are concerned about whether their personal protective equipment (PPE) is the correct size as fears grow over more infectious strains of coronavirus. Exclusive data from the British Medical Association, which polled over 7,000 UK healthcare workers, found a fifth of female doctors were not at all confident their PPE is “fully fit-tested” or adjusted to tally with their requirements. Just 13% of men said the same. The professional organisation for doctors argues PPE is generally designed to fit the “size and shape of male bodies” even though women make up 75% of NHS workers. Helena McKeown, chair of the BMA representative body, told The Independent the situation is not improving due to a lack of action from the government as she warned there is a dearth of different sized PPE for a range of body shapes available. Dr McKeown, who is a GP, said: “Without properly sitting face protection, these doctors are putting themselves at risk. Poorly fitting PPE makes performing simple tasks more difficult and at worst exposes health professionals to dangerous infection." “We’ve had the pandemic a year and we know a one size fits all approach does not work.     Care home staff were without personal protective equipment (PPE) early in the pandemic because the government prioritised the NHS, MPs have said. The Commons Public Accounts Committee said care homes received only a fraction of the PPE needed compared with the health service. It said social care "was only taken seriously after the high mortality rate in care homes became apparent". The government said it worked "tirelessly" to provide PPE. The report from the Public Accounts Committee said many healthcare workers were put in an "appalling situation" where they had to care for people with Covid-19 or suspected Covid-19 "without sufficient PPE to protect themselves from infection". It said the social care sector did not receive "anywhere near enough" to meet its needs.     NHS hospitals are defying official rules to give nurses and doctors masks with greater protection amid fears over the spread of coronavirus within hospital wards. An analysis of the latest NHS data by The Independent shows more than 35,000 patients were likely to have been infected with coronavirus while already in hospital between 1 August and 31 January. NHS England has estimated as many as 20% of infections could be due to spread within hospitals. Outbreaks at some hospitals have seen whole teams of doctors or nurses affected, in some cases leading to wards having to be closed. The Independent has learnt several hospitals are now supplying higher grade masks to staff working in general wards, despite Public Health England saying only surgical masks are needed.     Dr Katherine Henderson, a senior A&E consultant in London and President of the Royal College of Emergency Medicine, says physical and verbal attacks have increased in recent months. Speaking to the Guardian, she says: “It is a sad reality that in recent months there has been a rise in abuse directed towards healthcare workers, but this abuse is not something new to frontline staff or emergency departments. It was bad before the pandemic, but there’s a changed atmosphere now. “During the pandemic people were being very positive about healthcare workers. But now the public are frustrated that services aren’t getting back to normal. Maybe people who weren’t the source of abuse before are now being the source of abuse. Abuse may be physical or verbal, it may be through social media, or it may be racial or misogynistic. “People are being angry – very angry – with us. They are angry about long waits, about having to stand outside emergency departments in queues, about delays in ambulances coming, including to take their relative home from hospital.     The abuse of staff at GP surgeries has "no place in the NHS", a healthcare boss has said, following complaints it has risen during the pandemic. Dr Joanne Watt, GP chairwoman of the Northamptonshire Clinical Commissioning Group (CCG), said she understood patients' frustrations as surgeries battle with increased demand. But she said the reports of abuse were "extremely upsetting and demoralising". A receptionist told the BBC she had "never seen this level of abuse". Claire, who works at Harborough Field surgery in Rushden and has been employed by the NHS for 34 years, said staff were being verbally abused on a daily basis and it was "becoming the norm and it shouldn't be". "We work within the rules we're given. It's very upsetting, we've been reduced to tears," she said. The latest NHS staff survey found one in three staff claimed to have experienced at least one incident of bullying, harassment or abuse from service users, their relatives or other members of the public, in the year to March 2021.     NHS Highland says it expects to pay £3.4m in settlements to current and former staff who have complained of bullying. Whistleblowers exposed a "culture of bullying" at NHS Highland in 2018. A Scottish government-commissioned review suggested hundreds of health workers may have experienced inappropriate behaviour. So far 150 cases have been settled since the start of a "healing process", costing the health board more than £2m. Whistleblower Brian Devlin told BBC Scotland the scale of settlements made so far was "heartening", but he added that he continued to have concerns about bullying at the health board. A group of Highlands GPs first complained of a culture of bullying at NHS Highland in September 2018. Staff said they had not felt valued, respected or supported in carrying out "very stressful work". Others told of not being listened to when raising matters regarding patient safety concerns and decisions being made "behind closed doors".     Leaked results from a national survey of NHS staff has revealed a sharp drop in those who believe their health and wellbeing is being supported by their employer. The People Pulse is a national, monthly survey launched in 2020. It enables provider and commissioner organisations to monitor the NHS workforce’s health and wellbeing. According to a snapshot of the results recorded between May and August seen by HSJ, there was a drop of 9.6 percentage points in “perceptions of wellbeing support”, with “positivity” sitting at 57.3%. Almost a quarter of the survey respondents reported a “negative” experience of health and wellbeing support. The survey results also revealed almost a third of respondents said they wanted to speak up about a specific issue during the pandemic, especially on issues of staff safety, health and wellbeing, but they did not because they feared repercussions or believed nothing would happen.     A trust’s maternity services were rated ‘good’ despite an independent report finding ‘weaknesses in the culture’ and ‘defensive and fractious’ behaviours, HSJ has learned. As previously reported, former staff at Sandwell and West Birmingham Hospital Trust had raised concerns with the Care Quality Commission (CQC) over what they described as a “toxic management culture” and “unsafe” staffing levels in the trusts maternity service. Particular concerns were raised around community midwifery services. This prompted an unannounced inspection by the CQC in May, which found “low morale and negative culture” in the services. However, the CQC ultimately concluded the trust was taking positive steps to address the problems and rated its maternity services “good” overall, as well as for leadership and safety. Some frontline staff in the service have questioned those findings, however, and pointed to an independent review which was conducted in the early months of 2021. This review, carried out by independent consultant Debbie Graham and seen by HSJ, concluded there was “evidence of weaknesses in the culture; evidenced in the behaviours of some staff which appears to go unaddressed; a lack of strong, visible leadership; a lack of a shared vision; the finding that some staff have a fear of ‘speaking up’; and poor communication systems.” Read full story (paywalled) Source: HSJ, 20 September 2021

    NHS leaders are being urged to tackle racist abuse of staff as new figures reveal that a third of black, Asian or minority ethnic workers in mental health trusts in England have experienced harassment, bullying or attacks by patients, relatives or members of the public. One in three (32.7%) BAME staff in mental health settings have experienced abuse, according to analysis by the Royal College of Psychiatrists. For BAME workers across the NHS, the rate is more than one in four (28.9%). One medical director told the Guardian that hot tea had been thrown at them “because of the colour of my skin”. A psychiatrist told how they were left traumatised after they were racially abused – then threatened with a knife. The college is calling on health leaders to take a stand against discrimination by setting up better processes in NHS trusts to record and understand data about bullying and harassment. Dr Adrian James, president of the Royal College of Psychiatrists, said the findings were deeply concerning.     The RCN has today launched a new independent and confidential help and support line for any members or staff who have been affected by the issues relating to the reports received. Pat Cullen, General Secretary and Chief Executive, said: "Anyone can be affected by sexual harassment. It’s not restricted to any gender, age, ethnicity, sexuality or role. If you have been affected, it is important that you are listened to, supported, and helped to work through any actions you wish to take. This 0800 number will allow members or staff to discuss any concerns about sexual harassment related to RCN activity. “All staff and members have a right to feel safe and we are taking all necessary steps to provide that security.     The Modality Partnership, one of England's biggest general practice groups told HSJ that its GPs are regularly seeing more patients each day than is safe, after the number of people going to see their GP surged in the wake of Covid-19. Data has shown the provider’s GPs had an average of 20 patient contacts per day during April 2020, which has now risen to to an average of nearly 50 patient contacts per day. Modality, which had drawn up a report on the situation were quoted as saying, "There is just so much to cover – I am worried about missing something.” One partner at Modality who is also quoted in the report said: “An increasing number of patients I see are broken, often in tears, and seeking help to cope with the new stresses of life.” Read full story (paywalled).     A new study has found stress in the workplace could be leading women suffering from warning signs of heart disease. These signs included work-related stress, sleep disorders, and tiredness - which are important but non-traditional risk factors for having a heart attack or a stroke. Dr Wagner, a neurology professor at the University of Zurich, said "Traditionally men have been perceived to be more affected by heart attacks and strokes than women, but in some countries, women have overtaken men.     A new report from the British Medical Association has published results showing 9 in 10 female doctors have experienced sexism at work in the UK, including unwanted physical contact, denial of opportunities and being asked to massage male colleagues in meetings. The findings showed 91% of female doctors have experienced sexism at work, almost one-third (31%) of female doctors had experienced unwanted physical conduct in their workplace, and two in five (42%) of female and male doctors who had witnessed or experienced sexism felt that they could not report it. A female GP has said "I was asked at an interview if I was planning on having children. I’ve had patients refusing to see me as they want to see a proper – i.e. male – doctor … Advised I was not pretty enough to cause a distraction in meetings so they could treat me like a bloke.” Danny Mortimer, the deputy chief executive of the NHS Confederation, which represents hospital trusts, said "NHS organisations are working hard to make sure their staff do not experience sexism, or indeed, any form of discrimination, and Amanda Pritchard’s recent appointment [as NHS England’s new chief executive] signals a more representative leadership.     NHS leaders and experts have warned healthcare staff will leave their roles in a "mass exodus" unless exhausted doctors and nurses are given better support. This comes as reports earlier this year showed many healthcare staff are suffering from burnout. “From April onwards we’ve seen a significant rise in mental health cases, and it shows no sign of stopping,” Steve Carter, director of consulting services at FirstCare, told a panel of MPs and peers on Tuesday. “We need to address the mental health issue quickly if we are to get through the winter.” Read full story.     According to reports, GPs and healthcare staff are receiving a torrent of abuse and threats of violence from patients, with some surgeries being sent hate mail, bomb threats and graffiti. This rise in abusive behaviour comes as GP's become overwhelmed with demand, often working many hours overtime with delays being able to get to appointments due to staff shortages. Professor Martin Marshall, chair of the Royal College of General Practitioners, said: “It’s entirely unacceptable for anyone working in general practice to be at the receiving end of abuse of any kind, let alone the threat of physical violence.     Midwives at Suffolk Hospital have spoken out in a whistleblowing letter describing problems in their department as ‘demoralising and heartbreaking’. In the letter, written by midwives who declined to give their names "for fear of retribution", describe constant staff shortages, a culture of blame and fear, a high pressure environment and substandard care, saying " We entered midwifery to be able to give women centred, holistic care. Instead it feels like we are being overwhelmed by the unmanageable and relentless workload, and as a result are giving substandard care which is demoralising and heartbreaking. We are all feeling like we are now desperate for change. This change is beyond what we can achieve ourselves so we urge you to please help us to generate it. It should not be accepted or tolerated for us to be forced into giving unsafe care entirely due to unsafe staffing". In response, Karen Newbury, head of midwifery at the trust, said: “We are working exceptionally hard to recruit additional midwives and we are very grateful for the flexibility and dedication of our staff in ensuring that we provide a safe and caring service – this was recognised by our Care Quality Commission inspection in April which found we managed safety well. We have recently completed recruitment so there will be at least two senior midwives on every shift to provide flexible and experienced support to our maternity teams, and we are working with colleagues regionally to recruit staff both locally and internationally as well as running a full student training programme.” Read full story.
