    In addition to older individuals and those with underlying chronic health conditions, maternal and newborn populations have been identified as being at greater risk from COVID-19. It became critical for hospitals and clinicians to maintain the safety of individuals in the facility and minimise the transmission of COVID-19 while continuing to strive for optimised outcomes by providing family-centered care. Rapid change during the pandemic made it appropriate to use the plan–do–study–act (PDSA) cycle to continually evaluate proposed and standard practices. Patrick and Johnson describe how their team established an obstetric COVID-19 unit for women and newborns, developed guidelines for visitation and for the use of personal protective equipment, initiated universal COVID-19 testing, and provided health education to emphasize shared decision making.

    There is concern among patients, surgeons and health authorities regarding reported adverse patient outcomes following use of mesh in certain urogynaecological surgical procedures. The European Society of Coloproctology (ESCP) has conducted an extensive review of the surgical literature on the outcome of use of mesh in the pelvis of patients who have undergone bowel surgery and will shortly publish its recommendations. ESCP would like to hear from patients who have had both good and not so good experiences with colorectal surgery using mesh such as operations for rectal prolapse (rectopexy), or operations for advanced rectal cancer/inflammatory bowel disease who had mesh inserted to assist in skin closure of the back passage area. The survey is designed to capture the experience of patients who have had an operation that involved using mesh in the pelvis as a part of a colorectal (bowel) surgical operation. The survey is NOT designed to cover outcomes following urogynaecological operations for prolapse or urinary incontinence. The use of mesh as part of abdominal wall hernia repair is also not included.

    Labour has urged the NHS and Matt Hancock to pause their plan to share medical records from GPs to allow time for greater consultation on how the idea would work, saying that maintaining patients’ trust must be paramount. In a letter to the head of NHS Digital and the health secretary, the shadow public health minister, Alex Norris, said Labour backed the principle of improved data collaboration but shared the concerns of some doctors’ groups. The Royal College of General Practitioners warned NHS Digital a week ago that plans to pool medical pseudonymised records on to a database and share them with academic and commercial third parties risked affecting the doctor-patient relationship. NHS Digital needed to explain the plans better to the public, the group said, as well as outlining how people could opt out. The British Medical Association (BMA) has also called for a pause to the General Practice Data for Planning and Research scheme. Another group, the Doctors’ Association, said it was worried it would “erode the doctor/patient relationship, leaving patients reluctant to share their problems due to fears of where their data will be shared”.     n the UK, while most primary care contacts are uncomplicated, safety incidents do occur and result in patient harm, for example, failure to recognise a patient’s deterioration in health. This study by Cecil et al. determined the patient and healthcare factors associated with potentially missed acute deterioration in health. Differentiating acute deterioration from self-limiting conditions can be difficult for clinicians, particularly in patients with sepsis, urinary tract infections, or long-term conditions. The findings of this study support the call for longer GP consultations and caution against reliance on telephone consultations in primary care; however, more research is needed to understand the underlying mechanisms.

    Patients who had longer consultations with their GP were less likely to subsequently self-refer themselves to hospital due to a worsening of their condition, a study has found. The study, which looked at factors associated with potentially missed acute deterioration, said this might be because GPs with more time to assess patients ‘are more likely to recognise deterioration and refer the patient for secondary care’. A longer consultation might also allow GPs more time to provide advice, such as telling the patient to contact the practice again if their condition worsens, the British Journal of General Practice study found. The researchers defined a potentially missed acute deterioration as a patient having a self-referred admission to hospital after being seen by a GP in the three days beforehand. They found that 116,097 patients had contacted a GP three days before an emergency admission, with patients with sepsis or urinary tract infections more likely to self-refer. GP appointment duration was associated with self-referral, with a five-minute increase in appointment time resulting in a 10% decrease on average in the odds of self-referred admissions. Patients having a telephone consultation compared with face-to-face, previous health service use, and the presence of comorbidities were all also associated with self-referred admission, according to the research.     A group set-up following the Winterbourne View scandal is urging more people with learning disabilities to attend their annual health check-up. Healthwatch South Gloucestershire said regular health checks could prevent people from dying unnecessarily. It formed after BBC Panorama exposed abuse of patients at Winterbourne View hospital 10 years ago. Only about 36% of people with learning difficulties are believed to have an annual GP health check-up. The Local Democracy Reporting Service (LDRS). said the lack of regular, medical observations contributed to them having a life expectancy of 20 years lower than in the wider population. Healthwatch South Gloucestershire, a regional, independent health and social care champion, has created a checklist to encourage more people to attend appointments to help them improve their life expectancy. Vicky Marriott from the group said: "It is our unrelenting mission to listen and share people's lived experience so that the information informs how health and social care services improve. "We recently listened to people with learning disabilities and their families and developed with them an accessible info-sheet packed full of easy-to-read explanations about the lifesaving benefits of annual health checks."     Thousands of hospital patients were allowed to return to their care homes without a Covid test despite a direct plea to the government from major care providers not to allow the practice, the Observer has been told. As the crisis began to unfold in early March 2020, providers held an emergency meeting with department of health officials in which they urged the government not to force them to accept untested residents. However, weeks later, official advice remained that tests were not mandatory and thousands of residents are thought to have returned to their homes without a negative Covid result. The revelation will heap further pressure on the health secretary, Matt Hancock, who has admitted some care residents returned from hospital without a test. It comes after Dominic Cummings, the prime minister’s former senior adviser, last week accused Hancock of misleading the prime minister over the policy, during his unprecedented evidence in parliament. Some 25,000 people were discharged to care homes between 17 March and 15 April, and there is widespread belief among social care workers and leaders that this allowed the virus to get into the homes.     NHS staff at GP surgeries are facing unprecedented abuse and aggression from patients, while stressed doctors are increasingly suffering from mental illness, because of an appointments system “in meltdown”, family doctors’ leaders have revealed. The scale of the deep crisis in GP surgeries was revealed in an emergency summit of more than 60 NHS doctors, dentists and administrative staff in Salford, which the Guardian attended, triggered by a recent rise in verbal abuse. "Patients are short-tempered and not happy waiting for anything … They want letters. The latest one was a request to speak to a GP because he needs a letter to confirm anxieties that cause him a problem in long queues – because he wants to take his son to [Southport amusement park] Pleasureland and does not want to queue," says Jan Crowshaw, a GP manager It comes after a recent poll by the body representing GP surgery staff across the UK found that 75% of them face abuse every day, including assaults, threats, racism and sexism.     Rising numbers of patients needing care and a shortage of GPs is threatening to overwhelm the system, doctors and patient groups are warning. It comes after the pandemic has caused severe disruption to GP practices for more than a year. Analysis of NHS England data by the Health Foundation found more than 28 million appointments were booked in March, among the highest recorded. Doctor's leaders say what they are being asked to achieve is "undoable". The analysis of NHS data in England carried out by the Health Foundation for the BBC also that that between 2019-20 and 2020-21: The total number of appointments dropped by 10% - meaning 31 million fewer consultations with GPs and practice nurses Major shift from face-to-face to remote consultation, with the proportion seen in practices dropping from 79% to 54% The number of patients referred by GPs for urgent cancer check-ups dropped by 15%, putting lives at risk The escalating situation has prompted patient groups to call for an urgent review of access to services, amid reports that patients are struggling to get through.     Ensuring safe vascular access is a fundamental part of the care of many hospital patients with up to 90% of inpatients requiring intravenous access for delivery of fluids and medication or blood sampling. Historically vascular access has been carried out by anaesthetists, radiologists and medical consultants. But an HSJ roundtable heard that introducing specialist teams to assess patients for vascular access, and then insert, care and maintain devices has many advantages from both an organisational perspective and that of the patient.

Memory issues or problems recognising and interpreting the objects around them can cause the person frustration or create safety issues. Dementia UK have produced a leaflet with tips and guidance on how to make the home more safe for someone with dementia.

    NHS England has said GP practices must start opening their receptions and allow people to book initial face-to-face appointments — scrapping the controversial ‘total triage’ rule — for the first time since the early days of the coronavirus pandemic. Since last spring, NHSE guidance, supported by the profession, has said that face-to-face appointments must generally only take place after a phone, video or digital consultation. Many GP practice receptions have been closed to people wanting to make routine appointments. This rule was kept in place throughout last summer, despite covid circulation being low, and some health systems made the approach one of the pillars of planned post pandemic transformation. “Embedding total triage” was encouraged in 2021-22 operational rules for the NHS. The shift was seen by some as part of a move to a potentially more effective and efficient way of working. But there have been concerns about access and complaints in the media.     Patients have come to avoidable harm after a large private provider failed to deliver thousands of medicine prescriptions, according to a report from the Care Quality Commission. Healthcare at Home, which is based in Staffordshire but provides NHS-funded care and medicine supplies to patients’ homes across the country, has been rated “inadequate” and placed in special measures. A report published today said inspectors found more than 10,000 patients missed a dose of their medicine between October and December 2020 due to problems caused by the introduction of a new information system.     Patients awaiting a diagnostic test are to be assessed according to risk of becoming disabled as the service tries to prioritise in the face of huge backlogs. NHS England guidance released yesterday said local teams should categorise diagnostic waits on a four-point scale so those in most urgent need are seen first. It said this would mean, “recognising that for less urgent or routine diagnostics, some patients may experience a delay”. The diagnostics data for February showed 1.15 million people waiting for a test, compared to 1.08m in February 2020 – however, the proportion of people waiting more than 13 weeks rose from 0.6% in 2020 to 28.5% this year. The number of people on the list waiting more than six weeks also increased five-fold over the year. No more than 1% of patients are supposed to wait longer than six weeks for a diagnostic test, under government waiting time standards. The NHS England guidance puts diagnostics on the same footing as elective treatment, which has been organised according to clinical priority – P1 being the most urgent P4 being the least — since shortly after the pandemic hit last year.     Virtual wards, at-home antibiotic kits and using artificial intelligence in GP surgeries are among new initiatives to be trialled as part £160m funding to tackle waiting lists in the NHS. NHS England announced the funding to aid in the health service’s recovery after the pandemic, after figures last month revealed the number of people waiting to begin hospital treatment in England had risen to a new record. A total of 4.7 million people were waiting to start treatment at the end of February - the highest figure since records began in August 2007. But NHS England said indicators suggest operations and other elective activity were at four-fifths of pre-pandemic levels in April, which is "well ahead" of the 70% threshold set out in official guidance. It said it is working to speed up the health service's recovery by trialling new ways of working in 12 areas and five specialist children's hospitals. The so-called "elective accelerators" will each get some of the £160m as well as extra support for new ways to increase the number of elective operations, NHS England said. Tens of thousands of patients in the trial areas will be part of initiatives including a high-volume cataract service, one-stop testing facilities and pop-up clinics to allow patients to be seen and discharged closer to home. Other trials over the next three months include virtual wards and home assessments, 3D eye scanners, at-home antibiotic kits, "pre-hab" for patients ahead of surgery, artificial intelligence in GP surgeries and so-called "Super Saturday" clinics, bringing multi-disciplinary teams together at the weekend to offer more specialist appointments.     Every adult who has tested positive for COVID-19 in Scotland is to be invited to take part in a major new study into the effects of long Covid. Researchers hope to identify how many people in Scotland continue to be unwell after having the virus. The Covid in Scotland Study (CISS) will ask people what their symptoms are and how it affects their lives. Those taking part will be asked to use a phone app to answer questions about their health before and after Covid. Early estimates suggest as many as 6,000 people in Scotland were experiencing long term symptoms after the first wave of the pandemic but it is not known what that tally is now.
