    Health Education England (HEE) has created this accelerated preceptorship guide, for use with newly qualified practitioners (NQPs) and Health Care Professionals (HCP's) on the NMC Covid-19 temporary register in response to the COVID-19 pandemic. The guidance is focused on how health and care organisations pan-London can support newly qualified practitioners (NQPs) and health care professionals (HCPs) on the Covid-19 temporary register. This is a fast-changing and emerging situation and guidance may be subject to further development. Therefore, these principles should be used in conjunction with advice and guidance from the regulatory councils (e.g. Nursing and Midwifery Council (NMC), the royal colleges, trade unions and national HEE guidance.

    Patient safety is the number one priority in health care as safety is considered at every level of a healthcare organisation (e.g., building, equipment, communication, processes for medications, treatments, and surgical procedures). Addressing the welfare of patients can be challenging, yet for some of the most vulnerable patients (e.g., special needs, disabilities and mental and social health issues), even the most routine nursing requests can put them at a safety risk. Simulations provide an opportunity for nursing students and professional nurses with realistic experiences caring for individuals with unique needs, especially when safety is a major concern.

    Medical students who are employed in the NHS as part of efforts to swell staff numbers to tackle covid-19 should not be expected to “step up” and act outside of their competency, says the BMA in new guidance. This is the first set of guidance released by the BMA specifically for medical students, who have had placements and exams cancelled and are uncertain about how they might be employed in the NHS in the current crisis. It says that any employment should be voluntary and within the competency of the student, who should have adequate access to personal protective equipment. The BMA refers to General Medical Council guidance that states that plans are not currently in place to move provisional registration forward from the normal August date. It warns that there are concerns around the boundaries of practice and the level of supervision that students who take on roles in the NHS would have, which could lead to unsafe working practices.     Third year undergraduate trainee nurses will be invited into clinical practice to support the coronavirus effort, while routine care quality inspections are “going to need to be suspended”, the Chief Executive of NHS England has said. Speaking at the Chief Nursing Officer’s summit event in Birmingham this morning, Sir Simon Stevens told delegates NHSE was working with the Nursing and Midwifery Council to “see how many of the 18,000 [relevant] undergraduates are available”. It is understood they would be paid, and follows government moves to pass emergency legislation to relax rules around working in healthcare. Asked about Care Quality Commission inspections during the outbreak, Sir Simon said: “There will be a small number of cases where it would be sensible to continue for safety related reasons… but the bulk of their routine inspection programmes is clearly going to need to be suspended and many of the staff who are working as inspectors need to come back and help with clinical practice.” Read full story (paywalled) Source: HSJ, 11 March 2020

    The number of doctors entering GP training is higher than ever, yet the overall number of full-time equivalent GPs keeps decreasing. This is one of the reasons that patients report increasing dissatisfaction with their ability to access general practice, although they are satisfied with their care once they are seen. In this blog for the King's Fund, Abigail Heller, a current GP trainee discusses the results of a recent survey of 840 trainees about their career intentions. Abigail and many of the respondents hope to pursue other clinical or non-clinical interests alongside general practice, with interests ranging from expedition medicine to medico-legal work to give them the opportunity to broaden their skills However, despite this desire for a more flexible career, the trainees have concerns about an unmanageable workload. The intensity of the working day remains the leading factor in not wishing to undertake full-time GP work.

    Healthcare systems are operating in an environment that is increasingly moving toward value-based payments that reward good health outcomes and patient experience. An impediment to success in this environment, however, is that both health care delivery systems and health information are extremely complicated. The level of complexity stymies many people and hinders them from making informed preventive care and self-management decisions. Health systems are finding that they cannot achieve improved patient outcomes or experiences without improving how health care professionals communicate with and support patients. Health systems have begun to respond to the mismatch between patients’ capabilities and the health literacy-related demands of the healthcare system. A new term has emerged – the health literate organisation – that describes organisations that aspire to make it easier for people to navigate, understand, and use information and services to take care of their health. Health literate organisations, in turn, need healthcare professionals who have health literacy knowledge and skills, such as being able to communicate effectively, break down health goals into manageable steps, and connect people with the resources they need to be successful Harris et al. explores health literate care in this Commentary for the National Academy of Medicine.

    The Director of Medical Education (DME) at Oxford University NHS Foundation Trust developed a range of forums for junior doctor engagement with the trust, via representative groups, which meet, individually, with senior executives 10-12 times a year. These forums include a foundation year group, core medical trainee group, medical registrar group and more recently a surgical and anaesthetic group. Each group is chaired by a junior doctor, who sets the agenda and is responsible for organising the meetings. This structure emphasises that the forum’s agenda is focused on the needs and concerns of trainees, and encourages attendance and discussion.
