    Since the start of the pandemic, the number of people waiting for NHS treatment in England has grown to 6.1 million - the highest level since comparable records began – and is expected to rise further as those who have missed out on care come forward. In February 2022, the government published the NHS elective recovery plan which aims to increase NHS treatment capacity to 30% above pre-pandemic levels by 2024−25. However, there are many uncertainties around the future demand for care and the number of ‘missing’ patients who will eventually return to seek NHS care. This tool developed by the Institute for Fiscal Studies (IFS) allows you to simulate how waiting lists may change over the next four years under different assumptions.

Its performance against these was deteriorating before the COVID-19 pandemic and has worsened since it began. Millions of patients’ care was disrupted, meaning backlogs increased.     The pandemic has severely disrupted cancer services in England with major consequences for survival rates for lung, breast and colorectal cancer. This paper from the Institute for Public Policy Research examines the impact of the pandemic on cancer pathways, highlighting widespread disruption across screening, referrals, diagnostic and treatment services. The authors also highlight that the 'missing patient' backlog is difficult to predict and that there is a lack of qualified staff to increase capacity and aid service recovery.

    While the NHS delivered a remarkable amount of elective treatment during the pandemic, the pressure of caring for large numbers of patients seriously unwell with COVID-19 has led to the waiting list for elective care reaching the highest level since current records began. This analysis from The Health Foundation looks in detail at the impact of the pandemic on the waiting list for elective care in England. It highlights that: 6 million fewer people completed elective care pathways between January 2020 and July 2021 than would have been expected based on pre-pandemic numbers the backlog of elective care is not evenly distributed across England patients living in socioeconomically deprived areas faced more disruption and delays than those in England’s least deprived areas. It also looks at the difficulty in predicting how long the backlog will take to clear and how much it will cost. One unknown factor that complicates this task is 'missing' patients - those who did not or could not seek care during the pandemic. These patients may present at a healthcare setting requiring more urgent, intensive treatment as a result of missing out on earlier intervention.

    Many elective orthopaedic procedures were cancelled due to the Covid-19 pandemic and the number of patients on waiting lists for surgery is rising. This study looks at the disparities between inpatient and day-case orthopaedic waiting list numbers and the ‘hidden burden’ that exists due to reductions in elective secondary care referrals. The authors looked at elective procedures at a single District General Hospital in the UK between 1 April and 31 December 2020 and compared data with the same nine-month period from 2019. The study found: a 52.8% reduction in elective surgical workload in 2020 the total number of patients on waiting lists had risen by 30.1% in just 12 months inpatient waiting lists have risen by 73.2%, compared to a 1.6% rise in the day-case waiting list new patient referrals from primary care and therapy have reduced by 49.7%. The authors highlight the disparity between inpatient and day-case waiting lists and predict an influx of new referrals as the pandemic eases. They call for robust planning and allocation of adequate resources to deal with the backlog.

They wanted to understand how waits, delays and cancellations impact on people and their families, particularly those living with long-term, multiple and chronic conditions. The findings have compelling insights into issues around communication, receipt of referral, contacting health and care teams, estimations around length of wait, updates on delays, access to support and self-management and much more. COVID-19 has led to many cancellations and delays, and it is clear that waiting will be an important feature of healthcare in England in years to come. Health inequalities also impact access to care and treatment and the experience of waiting. This makes the findings in Patient. Noun. Adjective even more important. "Health and wellbeing deteriorates whilst being on a waiting list. For instance, recent polls found that half of those with experience of joint replacement surgery said their physical health deteriorated and one third said their mental health deteriorated while they were waiting."

    The ongoing impact of COVID-19 on health services across Europe has in most cases led to significant reductions in cancer screening, testing and diagnosis. The resultant delays in diagnosis are impacting cancer treatment and survival and are likely to do so for many years to come. Responses in individual countries and for individual tumour groups have differed, but there are common challenges in all countries. Some solutions go above and beyond the obvious actions that all countries are taking, and there are examples of how the system has reacted so far that provides the basis for further discussion on building lasting resiliency into healthcare systems and preparing for post-pandemic recovery. This report, published by IQVIA, highlights some of the approaches already being taken, as well as suggestions for what should be done going forward. It considers different stakeholders – from local pharmacies to national and international organisations – and their roles, as well as multi-stakeholder collaboration and cooperation. It aims to highlight initiatives adopted in some countries that can be shared more widely.

The guidance strikes a balance between prioritising covid response and recovery efforts and advancing the broader service transformation objectives of the NHS Long Term Plan.
