    Unpaid carers in Northern Ireland are suffering from "shocking levels of poor health", according to the charity Carers NI. In a survey of more than 1,600 unpaid carers across Northern Ireland, more than a quarter of respondents described their mental health as bad or very bad. One in five carers said the same about their physical health. The survey also found some 40% had not had a break from caring during the previous year and 23% said support services in their area did not meet their needs. Tracey Gililand, from Portadown, cares for her two disabled sons and said families like hers have been all but forgotten since the beginning of the pandemic. "Carers are still having to ask for the full return of much-needed day care and respite services and it feels like we've been left to paddle our own canoes with no help," she said. "No one knows our struggles, the many sleepless nights and exhaustion during the day. The impact on carers' mental health. The isolation that families like us experience that no one else sees," Ms Gililand explained. Carers NI said it has called for a legal right to social care support for all unpaid carers, the appointment of an independent carers' champion to advocate for carers to government, and wider transformation of the health system. Craig Harrison from the charity said carers had been "driving themselves into the ground", and were physically exhausted and in a state of constant anxiety.     Almost 90% of those living with Long Covid in Ireland have not returned to their pre-Covid level of health, according to a new report. The study of 988 participants was carried out by APC Microbiome Ireland, a research centre based at University College Cork (UCC), in conjunction with Cork University Hospital and Long Covid Advocacy Ireland. It found that more than two-thirds of participants in the study continued to experience fatigue, memory problems, chest pain, stomach upset, and muscle pain. Those surveyed also reported that they were suffering from new symptoms that had not been present before catching Covid. These included tinnitus (38%), mouth ulcers (28%), new allergies (16%) and sexual dysfunction (13%). They said these prolonged symptoms can significantly impact their quality of life, affect their ability to work and cause significant disability.     This Health and Social Care Select Committee report examines the pressure currently facing general practice, which is leading to low morale, GPs leaving the profession and problems recruiting new GPs. In turn, patients are increasingly dissatisfied with the level of access they receive. The root cause of the situation is that there are not enough GPs to meet the ever-increasing demands on the service, coupled with patients presenting with increasing complexity due to an ageing population. The report outlines the Committee's assessment of the key issues, including the problems with reliance on locum doctors and lack of continuity of care, and outlines what the Government should do to equip general practice for the future.

    Staff at accident and emergency departments across Scotland have expressed “deep concern” at the daily “excessively long waiting times” that are forcing a record number of patients to wait more than 12 hours, according to a leading NHS consultant. Dr John-Paul Loughrey, vice-chair of the Royal College of Emergency Medicine Scotland national board, warned that while such long waits were once regarded as “never events,” they are now daily occurrences. Amid fears the delays will spike significantly over the winter months, especially with another wave of Covid-19 expected, Dr Loughrey said staff were already “burned out,” “exhausted,” and “overwhelmed with a system facing increasing strain.” The latest weekly data on A&E treatment shows that in the week ending 2 October, the number of patients waiting more than 12 hours had soared by 45% week-on-week. “There is deep concern among staff around the excessively long waiting times,” Dr Loughrey said. “The weekly data that show significant increases in long waits translates to real patients on the ground or in the community who are seeking urgent and emergency care. “The system is failing them. We know that long waiting times are associated with patient harm and even death.     Long Covid clinics across Australia are being inundated with requests for assessments from patients struggling with ongoing symptoms, an inquiry has heard. Doctors told the federal parliamentary inquiry into long and repeated coronavirus infections that they were struggling to keep up with demand as waitlists increased. At least 10 million Australians have been infected with Covid and it is estimated 3-5% will develop Long Covid at some point. “Our waitlist is increasing because what we’ve observed is that it can take some time for the recognition of post-Covid conditions, particularly with the fatigue-predominant types, to reach us,” Royal Children’s hospital Associate Prof Shidan Tosif told the inquiry on Wednesday. Patients are usually referred to specialist clinics through a GP and while there is no official cure, symptoms can sometimes be treated on a case-by-case basis. The inquiry by the House of Representatives health committee is investigating the economic, social, educational and health impacts of long Covid and repeat infections.     Doctors recruited from some of the world's poorest countries to work in UK hospitals say they're being exploited - and believe they're so overworked they fear putting patients' health at risk. A BBC investigation has found evidence that doctors from Nigeria are being recruited by a British healthcare company and expected to work in private hospitals under conditions not allowed in the National Health Service. The British Medical Association (BMA) has described the situation as "shocking" and says the sector needs to be brought in line with NHS working practices. Dr Jenny Vaughan of the Doctors Association UK said, "This is a slave-type work with… excess hours, the like of which we thought had been gone 30 years ago.     Many nurses also act as family caregivers, and this study in the journal Nursing Outlook aimed to examine the impact of family caregiving on nurses, their colleagues and the organisations they work for. Nurse caregivers and healthcare organisation leaders completed two surveys about views on family caregiving. The authors found that healthcare leaders perceived family caregiving to have a larger impact on the nurses’ health and work performance than nurses themselves. Family caregiving was also identified as a potential contributor to burnout, and the authors highlight that lack of workplace support for family caregiving may influence nurses decisions about leaving or reducing their role.

    More work is needed on understanding and addressing a lack of sleep in rail workers, a new study has argued. Researchers looked at the difference between when staff were on day shifts and when they were working at night. They discovered a “feast and famine” scenario where 41% reported getting six hours of sleep or less when working days, compared to 63% when working nights. The findings, published in the Applied Ergonomics journal, suggested that many staff weren’t getting enough sleep and having less than six hours was linked to feeling very sleepy during the day. More than one in ten shift workers also reported they had been awake for between 18 and 24 hours by the time they finished work at least once during the past week. This led to fears that their tiredness could have an impact on road safety if they were driving home from work. The report said: “Sleep restriction and sleep deprivation, even in the short term, are known to affect cognitive performance. For a safety critical industry, this data should raise a significant concern.”

    Healthcare settings are high-risk environments for fatigue and staff burnout. The Need For Recovery (NFR) scale quantifies inter-shift recovery, which contributes to cumulative fatigue and may precede occupational burnout. Advanced clinical practitioners (ACPs) are an established feature of the emergency medicine workforce in the UK, however, little is known about factors affecting their inter-shift recovery, fatigue or how NFR correlates with formal burnout inventories.

A joint team of British and Greek researchers analysed 170 previous observational studies of the links between burnout among doctors, their career engagement and quality of patient care. Those papers were based on the views and experience of 239,246 doctors in countries including the US, UK and others in Africa, Asia and elsewhere globally. They found that burned-out medics were twice as likely as their peers to have been involved in patient safety incidents, to show low levels of professionalism and to have been rated poorly by patients for the quality of the care they have provided. Doctors aged 20 to 30 and those working in A&E or intensive care were most likely to have burnout. It was defined as comprising emotional exhaustion, depersonalisation – a “negative, callous” detachment from their job – and a sense of reduced personal accomplishment. A joint team of British and Greek researchers analysed 170 previous observational studies of the links between burnout among doctors, their career engagement and quality of patient care. Those papers were based on the views and experience of 239,246 doctors in countries including the US, UK and others in Africa, Asia and elsewhere globally. This meta-analysis provides compelling evidence that physician burnout is associated with poor function and sustainability of healthcare organisations primarily by contributing to the career disengagement and turnover of physicians and secondarily by reducing the quality of patient care. Healthcare organisations should invest more time and effort in implementing evidence-based strategies to mitigate physician burnout across specialties, and particularly in emergency medicine and for physicians in training or residency.     A study of over 1,000 health and social care workers, conducted by Florence, the tech platform providing health & social care workers access to available shifts, found that almost a third of healthcare workers admit to feeling overwhelmed at least once a week, with 17% feeling burnt-out every day. A staggering 97% believe the cost-of-living crisis has caused further stress or burnout among healthcare professionals. It comes after more than half of healthcare workers (56%) admit to working more than 2-3 times a week over their contracted hours, with 7% working overtime every day. Not having enough staff is causing the most pressure in their role (50%), followed by low pay (39%) and high workload (35%). The study revealed nine in ten NHS and social care workers state chronic staff shortages are affecting the quality of care. Analysing this deeper, three quarters of respondents stated that the quality of care is already being ‘severely’ impacted as high vacancy rates sweep across the industry. Dr Charles Armitage, Former NHS doctor and CEO and Founder of Florence, observed: “If you’ve got fewer people there on-shift to look after people, the quality of care decreases because the people that are there are overstretched, they’re trying to do too many things and are suffering from severe burnout.     Midwife numbers are reaching a dangerous level which could put lives at risk, as records show more staff leaving than joining the profession for the first time in a decade. As a record number suffer burnout and leave, the figures from NHS Digital for 2021/22 show almost 300 more staff abandoned midwifery than joined the service, with 3,440 leaving and only 3,144 coming in. Analysis of the data showed a record 551 resigned in 2021 because of a lack of work-life balance. Midwives working in NHS trust maternity units typically work 12-hour shifts, but many work longer for no additional pay to cover staff shortages and to keep services running. The Royal College of Midwives (RCM) says members are "at the end of their tether' and 'physically and emotionally burnt out" Joeli Brearley, chief executive of campaign group Pregnant Then Screwed, said: "We don't have enough midwives, and those we do have are underpaid, undervalued and overworked." "This is a problem that has been communicated to the Government repeatedly for years. It is putting the lives of women and their babies in danger and causing untold damage to their mental and physical health.     David Oliver is a consultant in geriatrics and acute general medicine who has worked in the NHS for 33 years. In this blog, he talks about his personal experience of running covid 'hot' wards during the different waves of the pandemic, describing the toll working in these conditions has taken on the health of him and many of his colleagues. He highlights the impact of looking after dying patients without adequate PPE, informing family members of patients' death over the phone, being responsible for many more patients than usual and witnessing colleagues die from Covid-19. The result has been burnout, mental health issues and low morale for a workforce that was already stretched before the pandemic hit the UK. David finally caught Covid-19 himself in March 2022 and he talks about how the virus—plus the cumulative effect of working under such strain for over two years—has meant he is not able to work and has been signed-off sick since mid-May.

    Hospital doctors are being sent home from daytime shifts and told to come back and work overnight in the latest stark illustration of the NHS’s crippling staff shortage. Medics are having to change their plans at the last minute because hospitals cannot find any others to plug gaps in the night shift medical rota and need to ensure they have enough doctors on duty. Hospital bosses are forcing last-minute shift changes on junior doctors – trainees below the level of consultant up to the level of senior registrar – because staff sickness and the scarcity of locum medics has left them struggling to ensure patients’ safety is maintained overnight. One trainee doctor in south-west of England told how they started their shift as planned at 8am. However, “by mid-morning the doctor that was meant to be working that night, that I would hand over to, had called in ill”. The doctor stopped working at 11am, drove home – an hour away – and came back to work the night shift at 11pm. “By the time I returned I had already worked for three hours and driven for three hours. That’s an extra six hours on top of a busy night shift of 12.5 hours,” they said. Dr Julia Patterson, the chief executive of EveryDoctor, said: “We are hearing of escalating problems with NHS doctors being forced to work unsafe, unfair hours." “Patient safety is of paramount importance to all doctors, but this situation is simply not sustainable.     This blog by doctors Clare Rayner and Amali Lokugamage argues that Long Covid rehabilitation needs a wider focus that goes beyond a purely biomedical paradigm to include complementary therapies and methods. The authors—who have both lived with Long Covid for more than two years—argue that although patients were the first to raise concerns about Long Covid, describe its symptoms and patterns and even research the condition, their narratives and voices are not being included in approaches to treatment. While the biomedical evidence surrounding Long Covid is currently limited, they highlight that there is much valuable lived-experience to be found in patient support and campaign groups, and that patients' knowledge should be drawn on to shape policy and guidance about the condition.
