A team of scientific experts has joined forces from across the world to help fight the spread of misinformation about the COVID-19 vaccines. Together they have created a unique online guide, led by the University of Bristol, to arm people with practical tips combined with the very latest information and evidence to talk reliably about the vaccines, constructively challenge associated myths, and allay fears. With the race on to vaccinate as many people as possible soonest in the wake of a more virulent virus strain, they’re appealing to everyone, from doctors to politicians, teachers to journalists and parents to older generations, to understand the facts, follow the guidance, and spread the word.

With the UK's official COVID-19 death toll exceeding 60 000, and data from the Office for National Statistics indicating excess deaths of more than 70 000, the succession of grim milestones in 2020 has shown little sign of abating, despite optimism around COVID-19 vaccination. Every single COVID-19 death is a family devastated by loss. The Covid-19 Bereaved Families for Justice, a group of over 2000 people who have lost a loved one to COVID-19 in the UK, share their stories in this Lancet commentary, and are campaigning for lessons to be learned as quickly as possible to save lives as the pandemic continues and ahead of the wide-scale roll-out of COVID-19 vaccines across the UK. They call for an immediate public inquiry with a rapid review phase.

The British Society for Immunology (BSI) has created two video question and answer sessions about vaccines for COVID-19. They took vaccine questions and concerns from the public on their Instagram channel and put them to expert immunology scientists, Dr Megan MacLeod and Prof Sheena Cruickshank. In the videos, Megan and Sheena answer these questions and explore the details of when vaccines may become available, who will receive the vaccines, how long immunity might last to a vaccine, how herd immunity can protect us and lots more. It’s important to understand and address vaccine concerns that are prominent in public discussion and may lead to hesitancy to vaccination. By answering your vaccine questions, the BSI hopes to provide expert information to help everyone make informed decisions about vaccines and their health. Follow the link below to watch the videos.

This report sets out the progress and learning from the first phase of the COVID-19 pandemic in informing advice and recommendations to government and the social care sector. The Social Care Sector COVID-19 Support Taskforce was commissioned in June 2020, with this report seeing the completion of its work in August 2020. The taskforce was set up to oversee the delivery of two packages of support that the government had put in place for the care sector: the Social Care Action Plan and the Care Home Support Plan. In addition, the taskforce was asked to support the government's work on community outbreaks – areas of the country that needed particular help and intervention to deal with higher rates of infection – and advising and supporting local places to consider and respond to reducing the risk of infection in care homes and the wider social care sector. Its further remit was to provide advice on the requirements for the response to COVID-19 in the next few months, ahead of and into winter.

The COVID-19 pandemic has uncovered serious gaps in the healthcare systems of many nations. In particular, it exposes a fragmented global governance system that does not have the structures to coordinate the pooling and sharing of resources needed to combat pandemics. Since the early days of the pandemic, medical protectionism has emerged as nations scrambled for their own stocks of personal protective equipment and ventilators. COVID-19 vaccines could be the next example. Already there is a danger of a vaccine bidding war, with governments competing for a limited number of doses, well before a vaccine even reaches the market. It is imperative that more governments and pharmaceutical companies agree to shoulder the costs of vaccine research and manufacturing, and to share data and technologies. They need to commit to WHO allocation guidelines and cooperate globally to distribute vaccines fairly to those at greatest risk. A pandemic vaccine needs strong global governance behind it.

Sinead Heneghan is a GP based in the North West of England with a passion for reducing health inequalities. In this interview for Patient Safety Learning, Sinead tells us how she made sure COVID-19 vaccinations were prioritised for people with learning disabilities, when national guidance advised otherwise. She also explains how they took the opportunity locally to combine these face-to-face immunisation appointments with annual health checks, identifying unmet health needs that needed addressing.

Adverse events are among the most heavily scrutinised parts of the covid-19 vaccine process. But India’s system was woefully unprepared for this, leaving families confused, sowing vaccine hesitancy in communities, while robbing the system of valuable data, reports Priyanka Pulla in this BMJ feature.

The UK Health Security Agency (UKHSA) has undertaken a rapid evidence review looking at the effects of vaccination against Long COVID or post-COVID symptoms. The review includes 15 UK and international studies that were undertaken up until January 2022. An estimated 2% of the UK population have reported symptoms of long COVID or post-COVID syndrome, which can last for more than 4 weeks after their initial infection. The three most common symptoms are fatigue, shortness of breath and muscle or joint pain. Eight of the studies in the review looked at the effect of vaccinations administered before infection. Most of these studies suggest that vaccinated people (1 or 2 doses) were less likely to develop symptoms of long COVID following infection compared with unvaccinated people – in the short term and long term (4 weeks up until 6 months after infection).

The Collaboration for Change is a group of two UK universities, nine community organisations and two small and medium size enterprises, who have conducted research on how to improve vaccine uptake among ethnic minority groups. This report outlines the results of a co-produced research study by the Collaboration for Change that combines research, evidence and lived experience to understand and improve vaccine uptake in ethnic minority communities. The report highlights the following factors influencing vaccine uptake: Using trusted messengers to encourage vaccine uptake may be a useful strategy to increase uptake in communities with low vaccination rates A lack of trust in the organisations and individuals encouraging vaccine uptake impacts vaccine uptake across many ethnic minority communities People are less likely to accept the offer of vaccination if little culturally and linguistically appropriate information is available to them, especially when their concerns are not covered Alongside the report, the Collaboration for Change has produced some recommended strategies to improve vaccine confidence and uptake.

UK experts have issued an update on the timing of elective surgery and risk assessment after COVID-19 infection. Your operation may be delayed if you test positive for Covid-19. Studies of people who had COVID-19 just before or after their surgery show that they had more complications and an increased risk of dying. The risks of chest problems, blood clots or death are about 3 or 4 times greater for a full 7 weeks following COVID-19. These risks are increased even if the patient had no symptoms from COVID-19 (i.e. just a positive test).

This guidance aims to support the safe roll-out of COVID-19 vaccination programmes. Vaccination programmes include a number of work systems, such as manufacturing, filling and packaging for distribution, testing and approval, cold chain delivery, booking systems for vaccination appointments, local administration of the vaccine, and patient follow-up. The challenges and requirements for operating such complex programmes at speed may vary both within a country as well as between countries, but the guidelines offer 10 principles to support systems thinking for vaccination programmes that apply across settings. These human factors and ergonomic principles relate to the identification and description of work systems (Identify), the improvement of work systems and processes (Improve), and the continuous learning from experience to achieve sustainable change (Adapt).
