    Since the severity of disease is closely related to the prognosis, the basic and essential strategies to improve outcomes that we should adhere to remain the early detection of high-risk and critically ill patients  This retrospective analysis of casein Jiangsu Province proved a good consistency between early screening of SpO2, RR, HR and early warning model. Therefore, a flowchart integrating early warning model and early screening procedure is recommended for high risk patients recognition and all patients’ screening to make it possible for early intervention.     This report from the AHSN Network shines light on ways we can do more to improve safety for residents of care homes. The publication showcases over 30 examples of projects delivered by England’s 15 Patient Safety Collaboratives (PSCs) and the Academic Health Science Networks (AHSNs) which host them. They include case studies in medicines safety, dementia, monitoring and screening, and workforce development.

    A team at South Tees Hospitals NHS Foundation Trust in Middlesbrough developed a programme to raise awareness of acute kidney injury (AKI) and to recognise and treat the condition promptly. Since the programme started there has been a sustained reduction (36%) in AKI cases within the surgical wards at Middlesbrough. This successful programme and pathway has been shared with seven other trusts in the North East of England. As a result of the AKI project and its links to CRAB Clinical Informatics Limited (C-Ci), other NHS Trusts (Imperial, Frimley Park, Wexham Park, North Devon, St Helen’s, Lincoln, Yeovil, Bartholomew’s, The Royal London and Southend) have now also been consulted, meaning this project has the potential for much wider spread. Commonly AKI starts in the community so the team is now focusing on strategies to support primary care to reduce AKI in the community and to harmonise AKI aftercare between hospital and community services. The South Tees Hospitals NHS Foundation Trust team was also highly commended in July 2017 at the national Patient Safety Awards.

    Rather than measuring how safe care is, the focus is often on measuring levels of harm in healthcare systems. This report by Healthcare Excellence Canada outlines findings from a research study which aimed to answer, “How safe is care from the perspective of patients, families, care partners, and care providers?” Through a literature review, interviews, focus groups and a World Café wthe study aimed to increase understanding of how patients and their care partners view safety. The Measuring and Monitoring of Safety Framework (MMSF) (Vincent et al., 2013b) was used to guide the study. The MMSF offers a broader, more comprehensive and real-time view of patient safety and helps shift away from a focus on past cases of harm towards current performance, future risks and organisational resilience. The report concludes that the MMSF represents a critical shift in how patients can enable safer care. Inviting patients and care partners to contribute meaningfully to safety will enhance healthcare providers’ view of harm and understanding of what it means to feel safe.

    There was a national roll out of ‘COVID Virtual Wards’ (CVW) during England's second COVID-19 wave (Autumn 2020 – Spring 2021). These services used remote pulse oximetry monitoring for COVID-19 patients following discharge from hospital. A key aim was to enable rapid detection of patient deterioration. It was anticipated that the services would support early discharge, reducing pressure on beds. This study from Georghiou et al. evaluated the impact of the CVW services on hospital activity. The study found no evidence of early discharges or changes in readmissions associated with the roll out of COVID Virtual Wards across England.
