    The latest NHS workforce figures have shown that a record number of staff voluntarily resigned from their jobs during the first quarter of this financial year. According to the data, almost 35,000 NHS workers resigned voluntarily, which was up from 28,105 during the same period in 2021, and 19,380 in 2020. It is also higher than in any equivalent first quarter over the last 10 years. The most common reason for leaving during quarter one of 2021-22 was ‘work-life balance’, with almost 7,000 NHS workers citing this as their reason for leaving their jobs. Close to 2,000 NHS workers also left in the same period in search of a ‘better reward package’, with almost 1,000 reporting ‘incompatible working relationships’.     A “perilous” shortage of homecare workers is the biggest reason thousands of people are languishing longer in hospital than needed, driving up waiting lists and making people sicker, figures reveal. Almost one in four people unable to be discharged – sometimes for weeks – were trapped in hospital because they were waiting for home care, as agencies hand back contracts because staff are quitting owing to low pay, leaving 15% of jobs vacant. A fifth of people unable to be discharged were also waiting for short-term rehabilitation and 15% were waiting for a bed in a care home, according to analysis of data obtained using freedom of information requests and public records by Nuffield Trust and the Health Foundation. It estimated that in April this year, one in six patients were in hospital because of delayed discharge, and the discharge of patients with a hospital stay of more than three weeks was delayed by 14 days on average. “People are ending up in hospital for malnutrition and dehydration, problems which, even if you supported people a little bit at home, would stop,” said Jane Townson, the director of the Homecare Association. “More providers are having to turn down work than usual and some are having to hand back people because they can’t do it.” Read full story Source: The Guardian, 3 October 2022

    Record numbers of nurses are quitting the NHS in England, figures show. More than 40,000 have walked away from the NHS in the past year - one in nine of the workforce, an analysis by the Nuffield Trust think tank for the BBC revealed. It said many of these were often highly skilled and knowledgeable nurses with years more of work left to give. And the high number of leavers is nearly cancelling out the rise in new joiners that has been seen. There were just 4,000 more joiners than leavers in the year to the end of June. But a Department of Health and Social Care spokesman said progress was being made and the government was already halfway to meeting its target to increase the numbers of nurses working in the NHS in England during this Parliament by 50,000.     Health service trusts in England are to be given additional funding to recruit nurses from overseas amid record staff shortages and increased demands. For nurses recruited between 1 January and 31 March 2023, trusts will be able to claim £7,000 per overseas nurse from NHS England. This is up to £4,000 higher than the financial support on offer during 2021-22. The move was unveiled by NHS Employers on its website last week and confirmed to Nursing Times by NHS England. NHS Employers said the additional funding reflected the rising costs of flights, accommodation and preparation costs for the nursing and midwifery objective structured clinical examination (OSCE). The OSCE forms part two of the Nursing and Midwifery Council’s test of competence and is a practical exam in which overseas nurses and midwives are tested on their clinical and communication skills. Responding to the move, Unison’s head of health, Sara Gorton, said: “Extra cash to tackle the chronic staffing shortages in the NHS is essential.” She warned that, until NHS staff vacancies ware addressed, there “will be a need for overseas recruitment”. “But it has to be done in the best interests of the individual workers,” she added. Ms Gorton highlighted ongoing concerns about unethical recruitment of nurses from overseas and the poor treatment many report facing. “Sadly, overseas nurses are still being exploited by unscrupulous care and health employers,” she said. “This is no way to treat those who come to offer the UK their help.” Read full story (paywalled) Source: Nursing Times, 27 September 2022

    Tinkering around the edges, the King's Fund said. A few short-term fixes, according to the Health Foundation. And a plan that will have minimal impact, the Royal College of GPs added. These were just a handful of the reactions from those involved with the NHS. And they were not even from organisations usually at the front of the queue when it comes to criticising government policies. So why has Therese Coffey's first announcement as Health Secretary for England received such a negative response? The fact is the problems the health and care system are facing are deep-rooted. Much is made of the impact of the pandemic but the health service was already struggling before Covid hit. The pandemic has simply exacerbated the situation. At the heart of it all is a lack of staff. Addressing this is not easy and cannot be done overnight. It takes five years to train a doctor, three a nurse, which is why there is a big push on international recruitment at the moment. To free up GP appointments, pharmacists are being asked to take on some of their workload, while funding rules are being relaxed to allow GPs to use more of their money to recruit senior nurses. But there is nothing in the plan about where these new senior nurses are going to come from, which is why the Royal College of GPs has been so dismissive. It is a similar story for hospitals services, where accident-and-emergency waits, ambulance response times and the backlog in routine treatments such as knee and hip replacements have all worsened in recent years. Coffey is also introducing a £500m fund to get thousands of medically fit patients out of hospital as soon as possible. Local areas will decide how to spend the money and it could allow hospitals to pay for extra help at home for patients who need it. But it amounts to little more than a sticking plaster and is an approach already used to relieve the pressure during the pandemic.     One in four people could be left without a GP within a decade, medics say. The forecasts from Doctors’ Association UK suggest 16 million people in England could be left without access to a family doctor, amid growing staffing shortages. Today the new Health Secretary is expected to set out plans to boost access to GPs, following warnings that public satisfaction is the lowest on record. Research by the Health Foundation suggests that the NHS will lose up to 8,800 full-time equivalent GPs by 2030 if current trends continue. On Wednesday, Doctors’ Association UK said this could leave one in four people without access to a GP. Dr Lizzie Toberty, GP lead for the Doctors’ Association UK, said the workload of a family doctor now placed “unrealistic demands” on them. She said: “GPs will cut their hours, quit the NHS, or quit the country.     Adult social care in England is in serious crisis, Tory council leaders have warned the government, as it faces a £3.7bn funding gap and a growing staffing shortage that has brought many local care providers to the brink of collapse. The intervention by the County Councils Network, which represents 36 mainly Tory-run authorities, comes amid widespread local government concern over the increasing fragile state of social care. Care costs have accelerated recently, fuelled by unexpected wage and energy inflation. “We face the perfect storm of staffing shortages, fewer care beds, and higher costs – all of which will impact on individuals waiting for care and discharges from hospital,” said Martin Tett, the Tory leader of Buckinghamshire county council. Cathie Williams, the chief executive of the Association of Directors of Adult Social Services, said: “Too many people are missing out on vital care and support – we estimate that over half a million people are waiting for assessments, care, or reviews. With over 165,000 staff vacancies, this is only set to get worse. ” A government spokesperson said: “The health and social care secretary is focused on delivering for patients and has set out her four priorities of A, B, C, D – reducing ambulance delays, busting the Covid backlogs, improving care, and increasing the number of doctors and dentists.     Ms. Martinez is a midwifery student in Tulancingo, Mexico, working in an underserved community. “There is a health care house, but there are no permanent staff,” she explained. “In my community there are many youth pregnancies, and there are no dedicated health staff who could care for women or take care of teenagers.” This shortage is partly due to a widely held misconception that midwifery is an antiquated profession, she indicated. “I met with doctors and nurses who questioned me: Why was I studying this midwife career? They didn't see room for that.” Thursday is the International Day of the Midwife, a moment to recognise the enormous contributions of midwives to health care around the world. “Not only do their capable hands bring new life into the world, they are champions of sexual and reproductive health and rights, providing voluntary contraception and other essential services, while supporting childbearing women emotionally,” said Dr. Natalia Kanem, UNFPA’s Executive Director, in her statement marking the day.     A shortage of nurses across the world, including in countries that provide nurses for international recruitment, has created a global health emergency, according to the latest report from the International Council of Nurses. The report, Recover to Rebuild: Investing in the Nursing Workforce for Health System Effectiveness, lays out the devastating impact that the Covid-19 pandemic has had on nurses around the world. It urges that investment in a well-supported global nursing workforce is needed if health systems around the world are to recover and be rebuilt effectively. It also warned against reliance on the “quick fix” of international recruitment instead of investing in nursing education, as this was contributing to staff shortages even in countries with a long tradition of educating nurses to work in higher income countries. The report, co-authored by the organisation's chief executive, Howard Catton, and nursing workforce policy expert Professor James Buchan, includes the findings of workforce surveys from more than 25 countries, including the UK, as well as other research.

    Brexit has worsened the UK’s acute shortage of doctors in key areas of care and led to more than 4,000 European doctors choosing not to work in the NHS, research reveals. The disclosure comes as growing numbers of medics quit in disillusionment at their relentlessly busy working lives in the increasingly overstretched health service. Official figures show the NHS in England alone has vacancies for 10,582 physicians. Britain has 4,285 fewer European doctors than if the rising numbers who were coming before the Brexit vote in 2016 had been maintained since then, according to analysis by the Nuffield Trust. In 2021, a total of 37,035 medics from the EU and European free trade area (EFTA) were working in the UK. However, there would have been 41,320 – or 4,285 more – if the decision to leave the EU had not triggered a “slowdown” in medical recruitment from the EU and the EFTA quartet of Norway, Iceland, Switzerland and Lichtenstein. The dropoff has left four major types of medical specialities that have longstanding doctor shortages – anaesthetics, children, psychiatry, and heart and lung treatment – failing to keep up with a demand for care heightened by Covid and an ageing population.     Nurses across the UK will go on strike for the first time over two days in the fortnight before Christmas after ministers rejected their pleas for formal talks over NHS pay. The Royal College of Nursing (RCN) said its members would stage national strikes – the first in its 106-year history – on 15 and 20 December. Senior sources said the industrial action was expected to last for 12 hours on both days – most likely between 8am and 8pm. The unprecedented national industrial action will seriously disrupt care and is likely to be the first in a series of strikes over the winter and into the spring by other NHS staff, including junior doctors and ambulance workers. On Friday, RCN general secretary, Pat Cullen, said the UK government had chosen strikes over listening to nursing staff, adding: “If you turn your back on nurses, you turn your back on patients.” The RCN said that despite a pay rise of about £1,400 awarded in the summer, experienced nurses were worse off by 20% in real terms due to successive below-inflation awards since 2010. It said the economic argument for paying nursing staff fairly was clear when billions of pounds were being spent on agency staff to plug workforce gaps. It added that in the last year, 25,000 nursing staff around the UK had left the Nursing and Midwifery Council register, with poor pay contributing to staff shortages across the country, which it warned were affecting patient safety.     The NHS staffing crisis will be solved only if doctors and nurses get more flexible about their job descriptions and break down barriers between roles, according to Rishi Sunak’s health adviser. Bill Morgan argues that training times for doctors and nurses may have to be reduced, and suggests developing “sub-consultants” and entirely new medical professions, He wants ministers to create an Office for Budget Responsibility-style body to predict future workforce needs. The Treasury has held down the numbers of doctors and nurses Britain trains to prevent “supply-induced demand”, which encourages people to seek appointments that are not needed, Morgan argues. Chronic shortages of qualified staff are the biggest problem facing the health service, which has more than 130,000 vacancies. Morgan acknowledges that this means “some of the government’s key manifesto commitments will not be met”, citing the promise of 6,000 extra GPs. Sunak said this week that the government was “thinking creatively about what new roles and capabilities we need in the healthcare workforce of the future”.     NHS therapy services won’t be able to manage increased demand driven by the cost of a living crisis as they are already thousands of therapists short, The Independent has been told. NHS counselling services in England are not meeting therapy access targets due to a shortfall of 2,000 workers, according to sources. The findings come as a poll by the UK Council for Psychotherapy (UKCP) and YouGov, shared with The Independent, found that almost one in two adults felt the cost of living crisis was affecting their mental health. According to the survey of more than 2,000 adults, 25 to 34 years old were most likely to say the cost of the living crisis was impacting their mental health. Adam Jones, policy and public affairs manager for the UKCP, said “I think what we’re concerned about is the fact that already, there is a record level of demand for mental health services. We also know there are record rates of prescription for antidepressant medication as well. We’re concerned the capacity currently is already falling short. “So with the rising demand going forward, we’re concerned that services are going to be stretched, waiting time is going to go up, average number of therapy sessions received is going to go down He warned that although the NHS is focussed on training new therapists, there was already an existing workforce of psychotherapists and counsellors who don’t work in the NHS. “We’d like to see more targeted recruitment of psychotherapists and counsellors who are already trained, and so that most would only require a short adaptive training to be able to work in an NHS context.” Read full story Source: The Independent, 23 November 2022

    A major acute trust says it plans to move away from its significant use of agency doctors from overseas, who have been reported to be working on terms and conditions far below their NHS-employed counterparts. East Kent Hospitals University Foundation Trust has a contract with the NES Healthcare agency to supply 47 “resident medical officers (RMO)” across its three main sites to cover trauma and orthopaedics, medical and surgical rotas. HSJ has been told of concerns that RMO's are reporting substantial overworking, and poor terms and conditions, although some of these claims are disputed by NES. East Kent chief medical officer Rebecca Martin has told HSJ: “The well-being of all our colleagues is one of our top priorities and we are working with the agency about how they cover the rota safely". “We are committed to providing a safe workplace environment, where RMOs feel comfortable communicating their feedback and we review working patterns to ensure adequate rest periods between shifts.     The Government is looking to hire a new cyber security chief for the NHS and Department of Health and Social Care (DHSC), at a time of heightened risk of cyber attacks against the health service. The DHSC last month issued a job advert for a “national chief information security officer”, who will sit within the digital policy unit of NHS England’s transformation directorate. It comes at a time when the risk of cyber attacks against the NHS is increasing. Earlier this summer, an attack on an NHS electronic patient record supplier impacted several providers, including a dozen mental health trusts, with some trusts still not having recovered their service fully.     GP surgeries across Scotland are at risk of collapsing because of staff shortages and increased demand, a senior doctor has warned. Dr Andrew Buist, chairman of the British Medical Association's (BMA) Scottish GP committee, told the BBC many practices were at "tipping point". More than a third of surveyed surgeries reported at least one GP vacancy – up from just over a quarter last year. About half of the GP surgeries in Scotland took part in the BMA survey. It showed 81% of practices said demand was exceeding capacity - with 42% saying demand substantially exceeded capacity. Dr Buist told BBC Scotland: "I worry that we're reaching a tipping point for some practices. "They lose one or maybe two doctors out of three, and the remaining doctors cannot continue so they return the contract and the practice may cease to exist. "That is a real concern in some parts of Scotland that that is happening and it's going to happen increasingly as the situation develops over this winter."     NHS bosses are increasingly paying premium rates for agency staff to plug holes in rotas, the BBC has found. Spending in this area rose by 20% last year to hit £3bn in England. For many shifts, bosses have been so short-staffed they have been willing to breach the government pay caps for these agency workers, most of whom are doctors and nurses. Separate data supplied by Labour showed some NHS trusts had paid as much as £2,500 to nurses to fill shifts. Out of 60 responses from trusts, 10 reported the most expensive shift cost over £2,000, and for another 13 it was between £1,000-2,000. The BBC spoke to one cancer doctor who was offered work for £130 an hour - well above the cap for his role - and described the fees available as "astonishing" and a reflection of "desperation by management". Chief executive of Colchester hospital, Nick Hulme, says a combination of factors is playing a role, including the need to open extra wards to cope with rising demand, as well as staffing shortages. Some people are even leaving their jobs, only to return later as agency workers. He says while many agency workers do fantastic work on the wards, he would still prefer to be able to use his own staff. "We know that's best for patient care, best for patient experience.     The biggest ever strike by nurses looks set to go ahead. The Royal College of Nursing is due to unveil the results of its ballot, which ended last week, in the next few days. The final results are being counted but RCN sources say a large majority of nurses have voted in favour of action in a dispute over pay. The RCN had recommended to its 300,000 members that they walk out. If strikes take place, they would affect non-urgent but not emergency care. The vote has involved a series of individual workplace-based ballots across the UK and if nurses do not back action at a local level it is possible some hospitals and services will not be involved. The government had appealed to nurses to "carefully consider" the impact on patients. But Pat Cullen, RCN general secretary and chief executive, said: "Huge numbers of staff - both experienced and newer recruits - are deciding they cannot see a future in a nursing profession that is not valued nor treated fairly. She added: "Our strike action will be as much for patients as it is for nurses. We have their support in doing this." Cabinet minister Oliver Dowden said the government had "well-oiled contingencies in place" for dealing with any strike action by nurses. Speaking on Sky News, Mr Dowden said essential services would be prioritised, "but of course there would be an impact as a result of a strike like that".     MPs have launched an inquiry examining workforce burnout across the NHS and social care, and the system’s ability to manage staff stress amid increased pressures during the COVID-19 pandemic. The House of Commons health and social care committee said it aims to produce a report showing the levels of staff needed in health and social care to tackle exhaustion and meet future challenges. The committee is calling for evidence on how workforce shortages impacted staff well-being and patient care during the pandemic and the areas that need to see recruitment most urgently. In the context of an ageing workforce, and alongside recruitment and retention challenges, efforts should be directed at developing a more inclusive profession rather than focusing on why people other than women do not become nurses. To attract more men, transgender people and those who identify as nonbinary, as well as women, the approach to nursing recruitment needs to change. The profession must develop a more inclusive culture and examine and promote the advantages that gender diversity can bring to nursing. This article from Quinn et al. explores the lack of gender diversity in contemporary nursing, briefly examines the history of gender in nursing, and considers how the profession might evolve into a more gender-diverse and inclusive workforce.
