Content ArticleThe dangerous practice of sending people with a mental illness hundreds of miles away from home for weeks at a time continues in England, according to new analysis published by the Royal College of Psychiatrists. Despite Government pledges to end the shameful practice, known as inappropriate out of area placements, by March 2021, almost 206,000 days have been spent by patients out of area in the 12 months since the deadline passed. Being far away from home, with friends and family not being able to visit, can leave patients feeling extremely isolated and emotionally distressed with devastating, long-lasting consequences for their mental health. Not only that, but it comes at a huge cost to the NHS. The health service spent £102 million on inappropriate out of area placements last year – the equivalent to the cost of the annual salary of over 900 consultant psychiatrists. The Royal College of Psychiatrists is calling on the NHS to adopt a ‘zero tolerance’ approach to inappropriate out of area placements and to take urgent action to ensure all patients get the care they need from properly staffed, specialist services in their local area.

Content ArticleThis national data collection project has been commissioned by NHS England (NHSE) and is run by the NHS Benchmarking Network (NHSBN). The aim of the project is to understand the extent to which organisations are complying with the NHSE Learning Disability Improvement Standards, and to identify improvement opportunities. Compliance with these standards requires organisations to assure themselves that they have the necessary structures, processes, workforce and skills to deliver the outcomes that people with learning disabilities and their families and carers, expect and deserve. This project aims to collect data from a number of perspectives to understand the overall quality of care across Learning Disability services. Read summary reports from previous years of the NHS England Learning Disability Improvement Standards project.

Content ArticleThe mortality rates for people with autism spectrum disorder (ASD) are double those of the general population and researchers believe unmet mental health needs may be a factor. The researchers’ results were derived from an Australian-first University of New South Wales (UNSW) study, which analysed linked data sets on death rates, risk factors and cause of death for 36,000 people on the autism spectrum. While cancer and circulatory diseases are the leading cause of deaths in the general population, injury and poisoning – including accidents, suicide and deaths related to self-harm – were the most common causes for people with ASD. GP and autism advocate Dr James Best told newsGP he was not surprised by the results, but that they did confirm people with ASD have a different set of health risk factors.

WHO European non-communicable disease dashboard (January 2022)
Patient-Safety-Learning posted an article in WHO
Non-communicable diseases (NCDs) including cardiovascular disease, cancer, chronic respiratory disease and diabetes, are leading causes of morbidity, disability and mortality in the WHO European Region, causing nearly 90% of all deaths and 67% of premature deaths. The World Health Organization (WHO) Regional Office for Europe has released the WHO Europe NCD Dashboard, which provides graphical data on NCDs in the 53 Member States of the Region and makes comparisons between them. The dashboard enables analysis of a country’s health situation and its progress towards meeting NCD health targets. It includes standardised data from national and international registries and surveys collected by countries, WHO and other international organisations -
Content ArticleIn this blog, Dr Timothy Ferris, NHS England National Director of Transformation and Rachel Power, Chief Executive at The Patients Association, look at patient access to health records. Dr Ferris writes about NHS England's ambition that patients are able to see their GP health record "at the touch of a button" and Rachel explains why it's important that patients have access to their records. Three patients also share why they find digital access to their records so useful.

Content ArticleThis analysis by the King's Fund looks at the latest British Social Attitudes (BSA) survey, which revealed that public satisfaction with the NHS fell by 17 percent between 2020 and 2021. It discusses the 'halo effect' that affected public attitudes to the NHS at the beginning of the pandemic, and why this has faded since 2021. The article highlights the importance of addressing workforce issues, but states that returning the NHS to an 'even keel' will take a long period of time. In the meantime, the Government should prioritise managing public expectations of the NHS. It also highlights that although the survey shows great dissatisfaction with the care currently provided, the public appears to have upheld its faith in the core principles of the NHS.

Content ArticleThe Primary Care Patient Measure of Safety (PC PMOS) is designed to capture patient feedback about the contributing factors to patient safety incidents in primary care. It required further reliability and validity testing to produce a robust tool intended to improve safety in practice. This study led to a reliable and valid 28-item PC PMOS that could enhance or complement current data collection methods used in primary care to identify and prevent error.

Content ArticleA Learning Health System (LHS) is a model of how routinely collected health data can be used to improve care, creating ‘virtuous cycles’ between data and improvement. This requires the active involvement of health service stakeholders, including patients themselves. However, to date, research has explored patients being ‘data donors’ rather than considering patients as active contributors. This study in the journal Health Expectations aimed to understand how patients should be actively involved in a LHS.

Content ArticleRather than measuring how safe care is, the focus is often on measuring levels of harm in healthcare systems. This report by Healthcare Excellence Canada outlines findings from a research study which aimed to answer, “How safe is care from the perspective of patients, families, care partners, and care providers?” Through a literature review, interviews, focus groups and a World Café wthe study aimed to increase understanding of how patients and their care partners view safety. The Measuring and Monitoring of Safety Framework (MMSF) (Vincent et al., 2013b) was used to guide the study. The MMSF offers a broader, more comprehensive and real-time view of patient safety and helps shift away from a focus on past cases of harm towards current performance, future risks and organisational resilience. The report concludes that the MMSF represents a critical shift in how patients can enable safer care. Inviting patients and care partners to contribute meaningfully to safety will enhance healthcare providers’ view of harm and understanding of what it means to feel safe.

Content ArticleLast year, a report from the Health Foundation looked at quality measurement—including patient experience—across a range of healthcare services. It described a “measurement maze”, with multiple sources and numerous national bodies presiding over data collection. The potential usefulness of the data for trusts was “limited by the data being hard to locate online, with multiple spreadsheets to choose from and large Excel workbooks to download and navigate.” The Patient Experience Library wanted to find a solution to this, to find a way to organise all the data such that every Trust could get one-click access to everything it might need.. Miles Sibley, Director of the Patient Experience Library, explains what they did.

Content ArticleThere was a national roll out of ‘COVID Virtual Wards’ (CVW) during England's second COVID-19 wave (Autumn 2020 – Spring 2021). These services used remote pulse oximetry monitoring for COVID-19 patients following discharge from hospital. A key aim was to enable rapid detection of patient deterioration. It was anticipated that the services would support early discharge, reducing pressure on beds. This study from Georghiou et al. evaluated the impact of the CVW services on hospital activity. The study found no evidence of early discharges or changes in readmissions associated with the roll out of COVID Virtual Wards across England.
