    Interesting article to highlight - as it does very well - the real-life consequences of disinformation and of underfunding healthcare treatments we might take for granted, such as vaccinations.

    Thanks for bringing this infographic together PatientSafetyLearning - very helpful for us all to be aware of such tactics, which are no doubt employed in other sectors as well, to silence those wishing to raise genuine safety concerns.

    Thank you - that was a helpful article to read and to understand the risks with the current system. Let's hope that organisations can appreciate the importance of reframing those 'too hard' recommendations into 'too important to ignore'.

    Thanks for the useful article highlighting an important issue. One topic briefly acknowledged towards the end of the article is that the collection of ethnicity data is so poor, both in the NHS and private sector. Whilst there should never be discrimination about treatment based on ethnicity, it would be immensely helpful to understand how conditions might affect different ethnic groups differently, plus whether there is any variation in treatment outcomes by ethnic groups. Without comprehensive collection, we just don't know and this is likely to cause disadvantage.

    This series of 3 articles rewards careful reading and re-reading. It's helpful to see how lessons from other work contexts (aviation, maritime etc.) may be applied to healthcare - and many of the same principles apply and are valid, from decision-making by primary actors (pilots or surgeons) to the impacts of fatigue, how rules are followed or not followed and potential consequences of the latter, and even how we understand safety and risk. Having had 10+ years in both aviation and now healthcare, I think there's still a lot of value in healthcare learning lessons from how safety is managed in aviation, where I experienced a forensically dispassionate examination of causes and effects, and a keenness to routinely collect, report and share all relevant information and data-points.
