Summary
Good communication is integral to effective healthcare provision. Communication failures, or errors, between doctors and patients or between professionals can result in patients being harmed or receiving substandard care. To improve patient care it is therefore important to understand different types of communication failures and the factors which contribute to these communication failures.

This scoping review was commissioned by the Chief Nursing Officer for Scotland and the General Medical Council in order to identify and understand different types of communication failure that lead to patient harm, and to inform the development of a taxonomy of communication failures and contributory factors within medicine which would support further work in this field.
