It is quite clear from all the reading that I have done on staff fatigue that there is much more that needs to be done to identify how best to manage the effects that staff fatigue has on patient safety. it is a personal responsibility as well as an organisational responsibility. Personal is how you manage your life and organisational is about appropriate skill mix, length of shifts and appropriate breaks within shifts, staff retention, having a just culture to enable staff to speak up about any issues that affects patient safety without reprisal etc.- Posted

This is a fabulous article Claire. The people and systems in healthcare must remember who they serve, and start working in partnership with patients, families, carers and the public in general. Patients with complex conditions are experts in their own bodies and if professionals listen and hear to what they have to say harm would be greatly reduced.- Posted

Hi Claire, What fabulous actors you and Chris make. As a PSP who is able to observe from the outside looking in it is evident today about the calamitous nature of the NHS and much needs to change if patients are to be kept safe. I am concerned whether the organisation and it staff do learn.- Posted

I liked your summarisation of the briefing. I am disappointed about the lack of discussion around healthcare staff education in modern times. I really believe this is key to the improvement of safety and quality in healthcare. We still have a disconnect between practice, theory and good leadership.- Posted
