PHSO: Prioritising patient safety (Winter 2025)

Summary

Prioritising patient safety is a new quarterly blog series from the Parliamentary Health and Service Ombudsman (PHSO).

Each month, PHSO publishes between 70 to 100 of their casework decisions as a way to share learning that will help organisations improve their service and prevent mistakes happening again.

Through these blog, Tony Dysart, Senior Lead Clinician, will be highlighting some of the cases PHSO publish to share good practice and findings from the casework more widely.

This first blog focuses on two cases PHSO have looked into about maternity care and imaging.
