    A webinar with the lead investigator into events that led to the increased neonatal mortality rate at the Countess of Chester.
    The press has widely reported that Facere Melius was commissioned to investigate who knew what, where, and when. The report, titled 'Hidden in Plain Sight', was submitted to the public inquiry in November 2024. However, there is much learning for the wider NHS in the events at the Countess of Chester. Whilst the contents of the report remain confidential, there is some generic learning that can come from understanding the events and circumstances. This is an introductory webinar; a full developmental event can be booked by ICBs for their local system leaders or for healthcare providers' senior teams. All of the information used in these events is in the public domain.

    The Patient Safety Incident Response Framework (PSIRF) encourages investigations across the NHS to apply SEIPS. This 3 hour introductory workshop will focus upon using SEIPS in Healthcare.   Our SEIPS trainer Dr Dawn Benson has extensive experience of using and teaching SEIPS, as a Human Factors tool, in health and social care safety investigation.  She will be joined in these workshops sessions by clinical subject experts.
    The session will be limited to a small group to ensure in-depth learning.

    This GovConnect webinar focuses on addressing the critical issue of wrongful hospitalisation for neurodivergent youth. Often, misunderstandings of neurodivergent behaviours or lack of appropriate support lead to unnecessary hospitalisations, causing distress for young people and their families.
