    Good to see this document link has been posted on here but I am suprised there has been no discussion yet. Nancy Leveson dissects Holnagels safety I and II publication in great detail with many strong points. I am inclined to side with Leveson and am concerned about how safety ii is being seen by some in the healthcare community as the new and best way forward. For example many recent job postings in the area are requiring as essential a thorough understanding of safety II.
