This tool is based on the Surgical Safety Checklist developed by the World Health Organization (WHO) in 2009. It should be used at three key transitions in care:
Before anaesthesia is given
Immediately prior to incision
Before the patient is taken out of the operating room
The checklist is not intended to be comprehensive, and additions and modifications to fit local practice are encouraged.
