Summary

A Patient Safety Incident Investigation (PSII) is the most in depth and complex of the learning response tools under the Patient Safety Incident Response Framework (PSIRF). The George Eliot Hospital has created a condensed, bespoke PSII patient information booklet which we would like to share if anyone would find it useful to support meaningful engagement and involvement of people affected by patient safety incidents.

Content

The leaflet has been simplified and co produced with our patients and staff.

Thank you for sharing Matt, and my initial thoughts are this is a great and much needed document. I particularly like the format of page 2.

I am going to share the leaflet with colleagues in Wales and discuss how we can adapt to meet the requirements of our regulations etc.

I shall keep you updated on their feedback!
