Just a cup of tea – An introduction to the SEIPS framework (21 March 2023)

Summary

This video offers an introduction to the Systems Engineering Initiative for Patient Safety (SEIPS) framework, an approach that looks at work systems and processes from a systems-based perspective. SEIPS is the main model used within the Patient Safety Incident Response Framework (PSIRF) adopted by the NHS. This video includes an explanation of the model and a dramatisation of the process of making a round of tea in a staff room, illustrating the error traps and design issues present in the environment.

45 years ago at a major hospital a cleaner couldn't find a free plug for her vacuum cleaner - so took one out, plugged in, did her job and then plugged the original one back in.  Would have been fine if it hadn't been the ventilator.  The kettle / fridge exchange reminded me of this and that disconnecting plugs that need to be on the whole time was a periodically recurring event during my working career (as was turning off switches to save electricity without realising that some things do need to stay switched on - like the main server).
