Hill Dickonson: Risk management and learning from inquestsRecent high-profile inquests have demonstrated the breadth of Prevention of Future Deaths reports (PFD reports). These reports also gain significant media attention. These are addressed to a range of public, private and third sector bodies and frequently organisations that were not involved in the inquest itself.
So what does this mean for your organisation?
Our experience at inquests has indicated that the right risk and governance procedures have an important role to play in demonstrating and assuring the coroner and mitigating the likelihood of a PFD report being issued to your organisation. In the event that your organisation does receive a PFD report, it is very important that it has sufficient risk and governance processes in place to ensure that lessons can be learnt from tragic incidents.
