I manage the out patient anticoagulant service at SRH and Worthing Hospitals , University Hospitals Sussex. We manage the out patient warfarin service for 2,000 patients, considering those suitable to switch to a DOAC/need for LMWH etc. Our team gives guidance to the teams for in patients on warfarin and we manage the peri-op bridging plans for all those on warfarin too. We have a weekly clinical MDT for complex patient management and advise when required our VTE prevention nurse. discussing HATS at a monthly meeting. We have 3 non medical prescribers in our team, soon fingers crossed a fourth. As our patient numbers on warfarin decline, I am very keen to keep our current staffing level and utilise the expertise of my team with the patient first approach in mind. Therefore we are keen to expand our clinic remit and review patients post discharge following 1st unprovoked VTE event. I would be very grateful to discuss with those that already offer this service to discuss/adopt appropriate pathways for this group of patients. Thank you for the invite to this hub.
