This a a very concerning case because it highlights a very high level of falsification of records which, depsite attempts at interventions by the Trust aparently remain stubbornly common. Assuming that these falsified observations, or at least some of them, would have been recorded by registered healthcare professionals, most likely nurses, this is a breach of professional standards. I wonder if the Trust has reported anyone to the NMC or other registration body as a result of this?- Posted
