Summary
This guidance supports services to provide developmentally appropriate care for 0 to 25-year-olds.
It sets out proposed actions for integrated care boards, providers and clinical teams to enable safe and effective transition between services.
If adopted by systems, this approach will improve continuity of care, health outcomes and young people’s experience.
